The evaluation of vestibulospinal and vestibuloocular reflex in patients during early postoperative period after vestibular schwannoma resection
Markvartová, Anna ; Čakrt, Ondřej (advisor) ; Chovanec, Martin (referee)
The thesis deals with the issues of balance disorders in patients after vestibular schwannoma surgery. The aim of this thesis is to evaluate the effect of targeted rehabilitation with biofeedback on balance disorders and subjective visual vertical compensation. During the period of one year, a group of 20 vestibular schwannoma patients were examined and treated in University Hospital Motol. The compensation of vestbuloocular refex was assessed by a tilt of subjective visual vertical. The level of vestibulospinal compensation was determined according to score of the Activities-Specific Balance Confidence Scale. Patients were examined before the surgery, after the surgery and after the rehabilitation. The Homebalance system, developed by Support center for application outputs and spin-off companies at the 1st Faculty of Medicine, Charles University in Prague, branch office in Kladno, was used for the therapy with visual biofeedback. Another aim of this thesis was to monitor the effect of vestibular prehabituation with ototoxic gentamicin application two months before surgery. The statistical analysis of data proved in the case of subjective visual vertical tilt a significant increase of deviation after surgery and a decrease after rehabilitation. According to the ABC questionnaire the confidence of...
Compensation of balance disoder in time in patients after vestibular schwannana resection
Hajná, Barbora ; Čakrt, Ondřej (advisor) ; Chovanec, Martin (referee)
Introduction: Disorders of balance and dizziness are characteristic symptoms in patients after vestibular schwannoma surgery and often have a negative impact on their quality of life. This thesis deals with impairment of postural stability particularly in patients at the long time period after surgery. The aim of this study is to observe the longitudinal development of vestibular compensation from acute postoperative period to time of several years after surgery. Another aim is to evaluate the short-term effect of vestibular rehabilitation in a long time period after surgical treatment, and to compare the results of measuring the subjective visual vertical, posturography, score of the Dizziness Handicap Inventory questionnaire, age and tumor size in these patients. Methods: Examined group consisted of 10 patients after resection of vestibular schwannoma (mean age 41.4 ± 12.1 years; 2 women and 8 men). All patients underwent intensive rehabilitation program in the acute phase after surgery. 7 patients did exercise with visual feedback, and 3 patients had conventional rehabilitation without visual feedback. In long time period after surgery all patients underwent one exercise unit with visual feedback. Patients underwent computerized posturography examination using force platform Balance Master® in...
New trends in cell and molecular biology of the head and neck cancer
Fík, Zdeněk ; Chovanec, Martin (advisor) ; Komínek, Pavel (referee) ; Ludvíková, Marie (referee)
Head and neck squamous cell carcinomas are still challenging despite progress in the oncological treatment. Study of the molecular biology allows to deeply characterize tumor properties and to predict the prognosis for affected patients. Nowadays there are many drugs clinically tested in the group of targeted therapy medicine Experimental work comprised both in vitro and in situ assays, being performed thanks to the collaboration between a number of departments of the 1st Faculty of Medicine of the Charles University in Prague, Academy of Sciences of the Czech Republic, Institute of Hematology and Blood Transfusion and Faculty of Veterinary Medicine of the Ludwig-Maxmillian University Munich. Galectin-1 is important inductor of the myofibroblasts/cancer associated fibroblasts. These fibroblasts are regarded as negative prognostic markers thanks to their capability of invasive cancer cells induction. On the other hand, Galectin-9 is not present in the carcinoma and in the case of dysplasia, its expression indicate aberrant features together with aberrant expression of keratin 14 and 19. Except from galectins using as prognostic markers, we focused on the galectins as a therapeutics instruments as well. Presented work with mutant variants of galectin-2 proved their effect on both pharmacodynamics and...
Evaluation of the effect of rehabilitation using visual biofeedback for patients after vestibular schwannoma resection
Jandová, Nikola ; Čakrt, Ondřej (advisor) ; Chovanec, Martin (referee)
The aim of this thesis was to evaluate the effect of rehabilitation with visual biofeedback at acute stage after vestibular schwannoma surgery and to evaluate if preoperative intratympanic aplication of gentamicin has influence on vestibular compensation of standing stability. The study was attended by 20 patients (12 women and 8 men) in the age from 28 to 63 years. Eight patients had vestibular schwannoma diagnosed in their left side, twelve in the right side. Rehabilitation program using interactive rehabilitation system Homebalance took place from the 5th postoperative day until hospital discharge, daily 15 to 20 min. To evaluate of therapeutic intervention objectively the device Synapsys Posturography System was used. Stabilometric examination was done before operation of vestibular schwannoma, after operation and after termination of rehabilitation with visual biofeedback. Statistic analysis showed significant increase of mean values of measured parameters CoP (sway velocity CoP, length of trajectory of CoP, area of the confidence ellipse CoP) postoperatively compared to preoperative values. After termination of therapeutic intervention there was any significant decrease of values of parameters CoP compared to postoperative status. Between group of patients with preoperative aplication of...
Quality of life in patients after vestibular Schwannoma surgery
Hajná, Barbora ; Čakrt, Ondřej (advisor) ; Chovanec, Martin (referee)
TVestibular schwannoma is a benign tumor that arises from the Schwann cells of the vestibular nerve. Unilateral hearing loss, tinnitus, facial and trigeminal dysfunction and vertigo are the most common symptoms. Surgical removal of the tumor is one of the treatment modalities of this disease. Surgical excision usually involves the complete vestibular nerve resection and there is also a risk of cochlear and facial nerve lesion. This thesis deals with changes in quality of life in patients after vestibular schwannoma sugery, which are associated with balance deficits, tinnitus and facial palsy.
The influence of unilateral vestibular dysfunction on spatial orientation in patients after vestibular schwannoma surgery
Šponarová, Lenka ; Chovanec, Martin (referee) ; Čakrt, Ondřej (advisor)
The aim of this diploma thesis is to evaluate spatial orientation in the patients after vestibular schwannoma surgery. In the theoretical part, there are summarized the facts about vestibular system; there is described the clinical picture of unilateral vestibular dysfunction and the field of vestibular schwannoma and spatial orientation in general. In the practical part, we performed the investigation of the spatial orientation using the Blue Velvet Arena System and posturography using the platform Balance Master® System. The purpose of the practical part was to find out, whether unilateral vestibular dysfunction in patients after vestibular schwannoma surgery has the impact on spatial orientation and to what extent is the postural stability influenced in patients identified for surgery and in patients after vestibular schwannoma surgery. Glycophenotype of head and neck carcinomas
Chovanec, Martin ; Smetana, Karel (advisor) ; Mandys, Václav (referee) ; Martínek, Jindřich (referee)
1 THESIS The thesis deals with the glycophenotype analysis of normal squamous cell epithelia, and head and neck squamous cell carcinomas. Many research projects focus on improving patients prognosis studying biological properties of the tumor. The alterations of carbohydrate motives occur during cell differentiation and carcinogenesis. Lectins are proteins that bind to specific carbohydrate structures. Animal lectins, including human lectins, consist of five subfamilies according to their binding specifity. Much attention is paid to the investigation of galectins - -galactoside binding proteins. Their carbohydrate recognitive function is not dependent on the presence of Ca2+ ions, contrary to the other lectin types. Galectins play important roles in many biological processes, e.g. in adhesion, intercellular interactions, immunomodulation, inflammation, cell growth, apoptosis, pre-mRNA splicing. Alterations in any of these might contribute to the malignant transformation and cancer progression. Endogenous lectins (galectins, Langerin, Manose receptor) were detected using immunohistochemistry, galectin-binding sites were localized by lectin histochemistry. Reverse lectin histochemistry was employed for the study of glycosaminoglycan binding sites. Human (squamous cell epithelia of the oral cavity, oropharynx...

Chovanec, Martin ; Fiala, Petr (advisor) ; Borovička, Adam (referee)
documentation. I decided to help solve this problém for one Czech company in the implementation of a specific project. Elements, that could affect the success of the project, are many. However, by using different models it is possible to try minimize some impacts of known risks. In manufacturing project, which especially focuses on custom projects, there is an essential part of the decision whether to accept not to accept the project. This company primarily endeavor to maximize profits, the project must be interesting. In the first part of this thesis I examine this problem. In the second part I do the project analysis, which was selected on the basis of multi-criteria evaluation of alternatives. Finally, this whole thesis is implemented into the reality and the company works with it.
