Peritoneal lavage cytology and biochemistry in cases of upper GI tumours

Introduction: Intraperitoneal tumor seeding is a common form of recurrence after surgery for GI tumours. Early diagnosis and treatment may influence the course of disease. Hypothesis and objectives of the work The finding of free tumor cells and / or elevation of tumor markers in peritoneal fluid predicts intraperitoneal tumor recurrence. The results of these tests can be used to indicate HIPEC. Aim of this work is to determine the sensitivity and specificity of cytology and tumor markers levels and importance of these tests for early diagnosis and prognosis of recurrence. Another objective was to confirm the applicability of RT PCR for identification of free tumor cells. Patients and methods . Materials are sourced both during initial surgery (220 patients) and by the dispensary controls (25 patients). The peritoneal fluid was examined cytologically and biochemically too. 50 samples was exemined by RT PCR. Results Elevation of tumor markers or finding of free intraperitoneal tumor cells predicts recurrence. The prognosis of these patients corresponds to stage IV TNM classification, regardless of other parameters TNM. Low sensitivity of these tests (34% for cytology, 53% for tumor markers) does not allow its use as an indicator of HIPEC. Due to degradation of RNA we did not confirm the value of RT...

1 Abstract Legal regulation of employment termination The rigorous thesis focuses on the legal regulation of employment termination. The author set himself the goal to prepare a comprehensive analysis of the legislation in question, to draw attention to application problems and, in the end, to evaluate it in terms of de lege lata and de lege ferenda. The thesis is divided into 10 separate chapters, most of which are further divided internally. The introductory part is followed by a chapter on the sources of labour law. The next chapter describes labour law facts, including defects in labour law practices. The third chapter deals with the employment relationship, subjects, objects and briefly also with the creation of an employment relationship on the basis of an employment contract and appointment. In the fourth chapter, the author generally deals with the termination of employment. In accordance with the labour law theory, the author divided the following key chapters into termination of employment on the basis of legal conduct, further into termination of employment on the basis of a legal event, and finally into termination of employment in special cases. Possibility to influence chronic post-surgery inguinal pain using of self-fixating mesh in laparoscopic inguinal hernia repair

Introduction: Transabdominal laparoscopic (TAPP) approach in the therapy of inguinal hernia is a suitable alternative to classical open inguinal hernia repair mainly in the hands of an experienced surgeon. TAPP repair offers the possibility of gentle dissection with implantation of the mesh from posterior approach. Hypothesis and objectives of the work: The fixation of mesh through penetrating techniques using staples, clips or screws is associated with a significantly increased risk of developing a post-herniotomy inguinal pain syndrome (CPIP). The aim of the thesis is to review options of self-fixating meshes in laparoscopic TAPP procedure without additional fixation. Furthermore to evaluate effect of this technique on development of the chronic postoperative groin pain and also on frequency of hernia recurrence and mesh migration. Patients and methods: Data analysis included all patients, who underwent inguinal hernia surgery at our Surgical Department within the period from 1.10.12 to 31.12.14 and fulfilled the inclusion criteria. Standard surgical technique was used. Data were entered and subsequently analyzed on Herniamed platform.
