The role of the nurse manager in facilitation of adaptation process of new employed anaesthesiological and intensive care nurse
HUDCOVÁ, Lenka
Successful coping with adaptation process by a nurse newly joining an intensive medicine workplace is quite a complicated process. Mastering sophisticated instrumentation, specialist demands of nursing care provision and everyday physical load as well as psychical stress expose each newly coming nurse to a wide spectre of demands the unconditioned coping with which is a basic precondition of successful integration to an elite team. Each incoming nurse (not just at an intensive care workplace) should be particularly supported by her nurse manager in the far from easy adaptation process, who can substantially help a new nurse adapt to the new environment and become an equivalent team member. The Thesis is aimed at the role of a nurse manager in comforting an adaptation process of a nurse joining an ARD and an ICU. The aim of the Thesis was to map the role of a nurse manager in adaptation process of a nurse joining an ARD and an ICU and to find out how such a nurse manager helps a nurse joining an ARD or an ICU with adaptation. Further aims were to map the process of adaptation of a nurse joining an ARD or an ICU itself and to examine the factors that mostly influence the adaptation process of a nurse joining an ARD or an ICU. A combination of qualitative and quantitative research was applied to the research part of the Thesis. A semi-standardized interview conducted with ARD and ICU ward sisters in selected hospitals in Prague was chosen as the data collection technique for the qualitative part. A questionnaire designed for nurses working at ARDs and ICUs in selected Prague hospitals, who themselves were undergoing or have recently undergone such an adaptation process was chosen as the data collection technique for the quantitative part. The aims of the Thesis have been met. The research results clearly showed that the adaptation process of a new nurse is not only based on effective specialist training, but also in integration of the nurse into the existing working team. It is thus necessary for a nurse manager to take sufficient care to the adaptation process and help a new nurse become an equivalent member of the team, which will thus achieve the set goals of the department as well as the whole organization under her efficient guidance. The results of our research may thus contribute to better awareness of the individual motivation factors of the adaptation process among the involved public. Upon the information found about functioning of a nurse manager in the process of adaptation of a nurse joining an ARD or an ICU a thought map was created, which might help as inspiration or as a manual for execution of this highly demanding process itself.

TUČKOVÁ, Marie
Adaptation process of graduate nurses The topic of this thesis is ?Adaptation process of graduate nurses?. The first goal of the thesis was to map satisfaction of graduate nurses with the way of initial training, their adaptation process duration and guiding. A questionnaire was used for assessment of satisfaction among the graduate nurses. The following hypotheses were set: H1: Graduate nurses are satisfied with the way of their initial training conduction, H2: Graduate nurses are satisfied with their adaptation process duration and H3 Graduate nurses are satisfied with their adaptation process guiding method. The second goal was to find out how the adaptation process of graduate nurses runs and how it is evaluated by head nurses. An interview was used for evaluation of satisfaction among head nurses. Two research questions were set. 1: How is the adaptation process of graduate nurses conducted? and 2: How do head nurses evaluate the adaptation process of graduate nurses? A questionnaire designed for graduate nurses in České Budějovice Hospital and Český Krumlov Hospital was used as a quantitative research technique. 50 questionnaires were distributed in total and 43 (86 %) returned. The research interviews showed that the adaptation process of graduate nurses is conducted according to Standard No. 055 defined by the České Budějovice Hospital and in line with a document ?Training Plan?. It involves allocation of a trainer to a graduate nurse, continuous assessment of theoretical knowledge and practical skills by means of oral examinations. We found that head nurses evaluated the adaptation process positively. The Training Plan and the training duration are sufficient for the head nurses. The adaptation process is necessary for all new nurses according to the head nurses. 66.8 % of the questioned graduate nurses are satisfied with the course of their adaptation process. The hypothesis H1 remained unconfirmed. All the respondents (100 %) were satisfied with the adaptation process duration. The hypothesis H2 was confirmed. 79.4 % of the respondents gave positive evaluation of the adaptation process guiding method, 2.3 % are not satisfied and 18.6 % said they were not guided at all. The hypothesis H3 remained unconfirmed. The results of the research may be offered to the addressed hospitals as a feedback to the method and usefulness of adaptation process guiding in their facilities and as a base for improvement of the process.

The third part of the thesis deals with work methodology and the research sample. The research was divided into three subsequent stages from December 2010 to June 2011. In-depth interview was chosen as the data collection technique. The research sample consisted of 8 respondents working at anaesthetic-resuscitation department of the České Budějovice Hospital, namely of four respondents with several years of experience and four respondents in the adaptation process. The fourth part of the thesis starts with interviews with the questioned respondents from the first research stage. They are interpreted by means of categorization tables in the thesis. Material on FF monitoring for newly hired nurses, available in Annex 2 is elaborated in the second stage of the research. The third stage of the research is dedicated to responses of the respondents to the newly elaborated educational material on FF monitoring. They were obtained by means of semi structured interviews with the respondents. The research has shown that ARD nurses are not given any educational materials on FF monitoring of sufficient quality. We have found that each of the respondents would like to obtain good quality materials on FF monitoring at his/her start at an ARD. However the respondents said that they did not know what exactly the educational material on FF monitoring should look like. All the eight respondents agree that they like the newly elaborated material on FF monitoring for new nurses from both, the content and form point of view and they would like to use it in practice. The fifth part of the thesis is dedicated to a discussion. It analyses and describes the research and its results. The conclusion of the thesis summarizes the research and the newly obtained information. All the goals of the thesis and the research questions are answered there. The thesis also includes a list of resources, abbreviations used in the text, key words and moreover the annexes elaborated within the work on the thesis.
