Angiographic examination of lower limb arteries using CT and MR.

Angiographic examination of arteries of lower extremities by CT and MR methods. At present, CT and MR are frequently applied methods in the diagnostics of diseases affecting the arteries of lower extremities (LE). These are mini-invasive examination methods, replacing in the Hospital of Písek, a.s. the invasive DAS examination method. At present, the disease of arteries of lower extremities is one of the frequent indications bringing the patients with certain problems limiting him in the usual life and endangering his life in case of the present pathology without surgical treatment, to the CT and MR workplaces. In this thesis, I deal with advantages of the performance of the angiographic examination of the arteries of lower extremities by the method of the computer tomography and magnetic resonance at the radio-diagnostic ward of the Hospital of Písek, a.s. from the viewpoint of time demands, price of examination, processing and evaluating the acquired data. In the theoretical part, I describe shortly the anatomy of the arteries of lower extremities and their diseases bringing the patient to CT or MR workplaces for the performance of AG examination. The application of KL i.v. the characteristics, indications and contraindications of which are also included in this part is closely connected with AG examination. Last but not least I describe CT and MR theory. In the practical part, I deal with materials associated with the necessary appliances and computer equipment of the CT and MR workplace of the Hospital in Písek, a.s.. It is followed by the description of the performance of AG examination of the arteries of LE by Toshiba Aquilion at CT workplace and by Toshiba Excelart Vantage 1,5T at MR workplace. The result of work is the comparison of the applied amount of KL between CT and MR examination methods, financial demands of the KL applied into the blood stream and the price of the completed examination after KL application. Another task was the comparison of time demands of both examinations; I came to the satisfactory result proving the advantages of the CT examination method. Thanks to opinions acquired from two independent physicians I came to the conclusion that CT examination is more advantageous in the sense of processing and evaluation of the acquired data than the MR examination.

The influence od educational to maintenance of secondary prevention rules of myocardial infarction

This bachelor thesis deals with the effects of educational plans on the compliance with the principles of the secondary prevention of myocardial infarction. The theoretical background covers atherosclerosis, acute myocardial infarction, the secondary prevention of myocardial infarction and the educational process in nursing. Prior to elaborating the practical part of the thesis we had established two objectives. Objective 1: To find out whether the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Objective 2: To compare whether the patients educated without an educational plan comply with the principles of the secondary prevention of myocardial infarction in the same way as the patients with an educational plan. A qualitative form of research had been chosen for this thesis, semi-standardized interview in particular. The analysis of the data gained was subsequently carried out. Two objectives had been established in the thesis. Objective 1: To find out whether the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Through the analysis of the obtained data we found out that the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Objective 2: To compare whether the patients educated without an educational plan comply with the principles of the secondary prevention of myocardial infarction in the same way as the patients with an educational plan. Having compared the results we found out that the patients with an educational plan comply with the principles of the secondary prevention more consistently than the patients educated without an educational plan. The objectives of the thesis were met.

Patellofemoral pain syndrome and its affection by patellar taping

Diploma thesis "Patellofemoral pain syndrome and its affection by patellar taping" consists of two parts. The first general part summarises all present knowledge in the field of classification, diagnostics and therapy of this syndrome. The experimental part investigates a partial area of conservative therapy and a patellar taping technique according to McConnell. With the use of EMG records of a group of patients with patellofemoral pain syndrome it assesses the immediate effect of patellar taping on the pain intesity and timing of m. vastus medialis obliquus to m. vastus lateralis throughout three different movements. The role of GH/IGF-1 axis components in the etiopathogenesis of metabolic disturbances in type 2 diabetes mellitus and acromegaly

(EN) GH/IGF-1 axis components (GH, growth hormone receptor (GH-R), IGF-1, IGF-1 receptor (IGF-1R), IGF-binding proteins (IGFBPs)) participate in the control of glucose metabolism, inflammatory processes as well as cell proliferation and differentiation, including adipocytes and monocytes. The aim of the present study was to evaluate the role of local mRNA expression of GH/IGF-1 axis components in subcutaneous adipose tissue (SCAT) and peripheral monocytes (PM) in the development of insulin resistance and differences of adipose tissue mass in following groups of patients: obese females with and without type 2 diabetes mellitus and subjects with active untreated acromegaly. A total number of 66 subjects were included in the study: obese females without type 2 diabetes mellitus (OB), obese females with type 2 diabetes mellitus (T2DM), acromegalic patients (AC) and healthy lean control subjects (C). T2DM underwent 2 weeks of very-low- calorie diet (VLCD - energy content 2500 kJ/day). According to our results we suggest that decreased mRNA expression of IGF-1, IGF-1R, IGFBP-2 and IGFBP-3 in adipose tissue of T2DM subjects may contribute to changes of fat differentiation capacity and the increased IGF-1R mRNA expression in peripheral monocytes in these patients may play a role in the regulation of...

BK-polyomavirus infection in patients after simultaneous pancreas and kidney transplantation

Introduction. The aim of the study was to introduce a new BKV PCR protocol in our centre and to verify its accuracy as well as to assess the prevalence, risk factors of BK virus replication, course of BKV infection and therapeutic approaches in simultaneous pancreas and kidney (SPK) recipients in order to design a screening protocol. Methods. The results analysed by both Affigene® and Transplantation Virology, Basel PCR protocols were compared. Thereafter 183 SPK patients were examined to assess the prevalence of BK viremia, viruria and BKVN and to identify the risk factors of BKV replication. The cases of retransplantation after a graft loss due to BKVN were retrospectively described. Results. 100 of results were analysed according to the Affigene ® and Transplantation Virology, Basel PCR protocols with the accordance of 95%, Rho = 0,946, 95% CI: 0.920 - 0.963, P<0,0001, Bland-Altman plot analyses: bias Basel PCR protocol/Affigene® BKV trender: -0,1 (mean) *±1.96 SD: -1,6 - 1,3] for both methods. Point-prevalence was assessed in 183 patients; Viruria found in 17,3 %, viremia in 3.8% of patients. Effect of a comprehensive rehabilitation program of children with asthma bronchiale

Title: Effect of a comprehensive rehabilitation program of children with asthma bronchiale Objectives: The main objective of this thesis is to show the usefulness and necessity of physical therapy in children with bronchial asthma and how does it affect them. Another objective was to summarize the possible available methods with demonstrations of examples of individual exercises that can be used for this disease. The aim was also monitoring and evaluation of the patient's subjective feelings. Methods: In the thesis we carried out a single arm experiment, where we used comprehensive kinesiology analysis of individual subjects and the methods of lung function tests. From the lung function test, we chose the spirometry test by method of curve flow / capacity and also we used the peak expiratory flow monitoring using peak-flow-meter. We compared the subjective feelings of clients at the beginning and the end of the treatment program. In the period we watched the changes of value in individual measuring techniques. Neuromodulation in treatment of selected dystonic syndromes

Dystonia is a neurological syndrome characterized by the involuntary contraction of opposing muscles, causing twisting movements or abnormal postures (modified by Fahn, 1987). Writer's cramp is the most common form of task-specific focal dystonia. In the first study, patients with writer's cramp were evaluated for differences in cortical activation during movements likely to induce cramps (complex movements) and movements which rarely lead to dystonia (simple movements). Although complex patient movements during fMRI were never associated with dystonic cramps, they exhibited abnormally decreased cortical activity. This was not observed in simple movements and was unrelated to the character of handwriting or the presence/absence of visual feedback. Our results support the theory of dualistic sensorimotor system behavior in writer's cramp. As the somatosensory system is believed to be affected in focal dystonia, we focused on modulation of the primary somatosensory cortex (SI) induced by repetitive transcranial magnetic stimulation (rTMS) in the second study, in order to improve writer's cramp. I managed to find the unique theme of work, I have also become a big challenge. The main motivation was Considering that the work has clear clinical meaning and perhaps even impact. Diabetes mellitus (DM) is a serious disease significantly changing the current life of the patient. Good control and treatment disease requires patient discipline in lifestyle and respect of medical schemes. However, even with goodcompensation diabetes may progress and reduce the quality of life affected. This is mainly on chronic complications specific for this disease - that is affected eye, kidney and polyneuropatické. The theme of this work is just one the most serious chronic komlikací diabetes and diabetic nephropathy.
