The role of nurses and their impact on the care of patients with total hip arthroplasty.

Total hip replacement is one of the most extensively and highly effective operating methods in orthopaedics. The patients are brought back the self-sufficiency, life without the pain and noticeably motion restriction. Irreplaceable function here is a role of the nurse, who helps the patient to return back to his common life...The aim of this work was to find out, how the patients feel the role of the nurse and its influence to given nursing care before and after the total hip replacement as the less fulfilled. There were following research questions set: How the patients feel the role of the nurse before and after the total hip replacement? How the patients before and after the total hip replacement feel the interpretation of the nurse roles? How the patients before and after the total hip replacement feel the role of the nurse and its influence to given nursing care. Which of the nurse roles are felt by the patient as the unsatisfactory in postoperative care? The patients before and after the total hip replacement feel through the everyday nurse activities the nurse in her role of the caregiver and education giving person, following the nurse vindicator and the nurse bearer of changes. The patients feel the care thanks these roles as a quality, holistic and continual. Nevertheless was the role of the education giving named as the least fulfilled. In connection with these research results was created a manual, a Guide book for the patients before and after the total hip replacement. This Guide book was given to testing purposes. This Guide book was appreciated by the patients very positive despite a large amount of information, which were surprising for many of them. We gained some data, which gave the impulse to complete the information in the Guide.The created Guide book can´t replace the effective communication of the patient with the medical staff, which could be always the most suitable "source" of information.

Final bachelor's thesis on " Nursing care for a patient after a hip replacement" bachelor's thesis focuses on the problem of total hip replacement, as the disease coxoarthrosis to preoperative and postoperative care of patient after total hip replacement. The objective is to determine the level of preoperative and postoperative care of patient after total hip replacement. in selected hospital and assess the general standards of care in the preoperative and postoperative care. Another objective is based on information obtained from preoperative and postoperative situation to propose their own standards. In the bachelor's thesis I used the method of qualitative monitoring undertaken using a case study of nursing.

Bachelors thesis is directed at perceived quality of life patients after total hip replacement. It contains theoretical and research parts. Theoretical part is consist of two parts: first part is about characterization of total hip replacement, second is about quality of life and their meaning at National Health Service. Research part is concern of questionnaires research. It passed of one hundred patients at Medical spa Bohdaneč. It compared quality of life before and after operation.

Cílem této bakalářké práce bylo zpracování případové studie ošetřovatelské péče po totální endoprotéze kyčelního kloubu u pacientky F.J. (78 let) v jejím domácím prostředí. Pacientce byla implantována náhrada kyčelního kloubu pro zlomeninu krčku femuru. Práce má klinickou a ošetřovatelskou část. Na začátku klinické části je uvedena anatomie kyčelního kloubu, dále se zabývám zlomeninami proximálního femuru a jejich řešením. Následuje stručná historie totálních endoprotéz kyčelního kloubu, jejich typy a možné komplikace. V ošetřovatelské části je zmíněna charakteristika ošetřovatelského procesu s využitím jednotlivých fází ošetřovatelského procesu podle koncepčního modelu Marjory Gordonové. Informace o pacientce byly získány na základě rozhovoru s pacientkou a jejím přítelem, vlastním pozorováním, sledováním fyziologických funkcí a čerpáním z dostupné dokumentace. Jejich vyhodnocením jsem stanovila aktuální i potenciální ošetřovatelské diagnózy, včetn plánu ošetřovatelské péče, jeho realizací a hodnocením péče. Dále uvádím dlouhodobý plán ošetřovatelské péče a hodnocení psychického stavu pacientky. Edukace pacientky a následná prognóza ukončuje ošetřovatelskou část bakalářské práce.

Sciatic nerve traction palsy after surgery

Title of thesis: Sciatic nerve traction palsy after surgery Aim of thesis: The aim of this bachelor's thesis is to look at the origin of pheripheral palsy after a total hip replacement and to compile a detailed examination of a patient with sciatic traction palsy after surgery. Method: In the first theoretical part of this bachelor's thesis I am following the problems associated with peripheral palsy after a total hip replacement , it's etiology, risk factors, diagnosis and prognosis. Eventually, I focus on the rehabilitation treatment for peripheral paresis including physiotherapeutic methods and procedures. The second specialized part is an examination of patient with sciatic traction palsy after a total hip replacement. This includes anamnesis, differential judgement, initial examination, short term physiotherpeutic plan, procedure of therapy, final examination, evaluation of therapy effects and long term physiotherapeutic plan. It also describes the processes used in three week's therapy of the patient at The Department of Rehabilitation and Physical Medicine in Ústřední vojenská nemocnice in Prague. The issue of compensation of a heart valve from the perspective of the patient

A heart valve replacement is one of the most technically difficult cardiac surgeries. For the patient, it means a mental and physical health intervention. Heart valve surgery is currently the second most common cardiac surgery - accounting for up to 20% of the total number of cardiac surgeries. The goals and objectives of a heart valve replacement are to improve patients' quality of life and their medical prognosis. The heart valve replacement is carried out only in cases when we cannot perform the heart valve plastic surgery and thereby replace the entire heart valve with an artificial one. Heart valve prosthetics can be mechanical - patients with this kind of prosthetics are then warfarinized for life or they can also be made of biological material (bioprosthetics). Patients with bioprostehtics require only a three month long anticoagulant therapy. Temporary postoperative complications may occur such as dyspnoea, gastric and intestinal atony or aggravation of pulmonary functions. Among the rarer complications are rhythm disorders, thrombosis of prosthetic heart valve, and last but not least infective endocarditis. The patients, after the heart valve replacement, are at a greater risk of complications, therefore they are obliged to always have their patient at risk of infection endocarditis card with them (see appendix 4). After the operation the patients are informed about the lifestyle changes they are to follow within their daily routine, the information is delivered by doctors and nurses. The task of the nurses is to inform the patient about the lifestyle changes together with doctors. Good understanding and observance of these lifestyle changes help to improve patients' life after the heart valve replacement. This bachelor thesis is divided into an empirical and theoretical part. The theoretical part focussing on the problems connected with the heart valve replacement, valvular imperfections, perioperative nursing, patients' lifestyle changes and their quality of life.

Advanced Fabrication of Custom Orthopaedic Implants Using Selective Laser Melting Technology

This work describes advanced fabrication of custom orthopaedic implants using unconventional additive manufacturing technology - Selective Laser Melting (SLM). There was a main focus on custom knee replacement and certainly on its femoral component. The study investigated three general issues within the domain of the usage of additive manufacturing technology in medical application. First, there was an evaluation of process parameters influences of SLM fabrication method on surface and mechanical properties of titanium Ti6Al4V ELI specimens. This material was used because of its biocompatibility and its wide use within implant fabrication. Then, a proposal of the manufacturing strategy was carried out and the fabrication of customized knee femoral component prototype by SLM technology was done. The elaboration of the numerical chain prior the SLM implant fabrication, from patient's CT knee scan to final femoral replacement model, was also done. Then, a proposal of different 3-axis and 5-axis strategies of machining of the fitting femoral surface of bone prototype (3D printed from the powder) using CNC machines FV 25 CNC and TAJMAC ZPS MCV 1210 was projected and also, the 3-axis spiral machining was realised. The individual machining tool paths were generated by software Power Mill from Delcam group. Finally, these machining strategies were generated as a prior step before a machining of real patient’s bone, therefore the machining tests of cartilage and bone were done.
