Title: Comparison of educational and compensation exercise programmes for ankylosing spondylitis patients Objective: The objective of the research was to determine the effect of two educational and compensation exercise programmes on the mobility of the axial system, the functional status and disease activity (including inflammatory process activity) in individuals with ankylosing spondylitis. Methods: The research group included men and women (average age of 35.42 ± 7.15 years), all out-patients of the Institute of Rheumatology in Prague. Title: Group exercise with elements of the method Pilates in individuals with ankylosing spondylitis - comparisons with the control group Objective: Find impact of group therapy with engaging Pilates method to movability of axial system, functional status, activity of disease and total health status of individuals with ankylosing spondylitis. Compare this method with compensation group motion program in individuals with ankylosing spondylitis. Methods: 26 individuals with ankylosing spondylitis of average age (38,25 ± 9,18) attending Institute of Rheumatology in Prague were chosen. Probands were split into two groups: experimental group, which were doing motion program with elements of Pilates (n=13), and control group, which were attending compensation group motion program in individuals with ankylosing spondylitis. It was empirical quantitative research, exactly comparative quasiexperiment, where was compared individual groups in between (inter-group) and, moreover, influence of individual motion programs (intra-group). The theoretic part describes the characteristics of AS as the main representative of spondylarthritides, including its diagnosis and testing methods. It also contains an overview of current approaches and strategies of a treatment. It focuses on biological treatment with TNF-α blockers. The practical part of the thesis illustrates the findings on the example of a patient with AS treated with biological therapy. It evaluates the subjective feelings in response to treatment and compares the results of treatment with standard therapy (i.e., without biological treatment) on another patient under otherwise similar conditions.

The theoretical part focused on the basis of rheumatology and general issues of ankylosing spondylitis, etiopathogenesis of disease, the course and especially non-pharmacological and pharmacological treatment. The special part deals with complete case report of patient with this disease, the course of therapy and their outcome.

The rehabilitative treatment insists foremost upon spinal flexibility maintenance which is herewith a part of the diseas progression prophylaxis. In case of disease acute phase, I carry elimination the complications associated with pains caused by fault motional stereotypes in particular. After pains alleviation, I focus on eliminating fault motional habits.
Application of backstroke to a patient with ankylosing spondylitis

Title: Application of backstroke to a patient with ankylosing spondylitis Objectives: To evaluate the effectiveness of the application of backstroke with chosen parameters to a patient with ankylosing spondylitis. To compare the effectiveness of this swimming exercise programme with a traditional exercise programme held in a gymnasium. This work investigates whether it is suitable to recommend backstroke swimming to patients with ankylosing spondylitis as an effective form of physical exercise for improving range of movements and reducing pain. Methods: The effectiveness of a 9 week long swimming programme was evaluated using a range of movements of the glenohumeral and coxal joints and of the spine. The presence of pain was measured by the Bath Ankylosing Spondylitis Diasese Activity Index. The same parameters were measured for another patient with the similar activity of the disease who took part in group exercises in a gymnasium. At the end of the programmes the parameters were compared and evaluated. Results: The swimming programme has improved the patient's posture and has strengthened back muscles. Pain was reduced in some parts of the back and also general pain measured by BASDAI. The range of movements of both glenohumeral joints improved in all measured directions. The range of movements...
Effect of exercise therapy to Morbus Bechtěrev patients

Title: Effect of exercise therapy to Morbus Bechtěrev patients Objectives: Aim of this Thesis is to collect and analyse the available information from Czech and foreign sources concerning the Morbus Bechtěrev disease. Aim of empiric part is to evaluate the effect of four-week spa therapy In the Lazne Bohdanec spa to the Morbus Bechtěrev patients. Methods: Method of work consists of the study of available sources, assembling of theory part, collection and analysing of data and their statistical evaluation. For this research was used the standardised questionnaire method with added questions needed for the evaluation of hypotheses. Results: By the analysing of 32 pairs of anonymous questionnaires was concluded that the effect of exercise therapy to patients is positive, but not permanent. The use of physiotheraphy procedures for treating inflammatory rheumatic disease selected from the perspective of patients.

Rheumatology is a part of internal medicine that deals with diseases of musculoskeletal system. It is primarily devoted to diagnosis and treatment of these diseases. Rheumatoid diseases primarily affect joints and muscles, but other organs can also be affected endocrine glands, the nervous system, internal organs such as the kidneys, liver, lungs, human gastrointestinal tract, and skin. The diseases are classified as follows: inflammatory, degenerative, metabolic bone and joint diseases, extra-articular rheumatism, septic arthritis, etc. The causes of these diseases are unknown, but internal and external factors play an important role. They may be infectious, immunogenetic factors and effects of the external environment such as drugs and chemicals. The aim of this work is to map the use of physical procedures in the treatment of inflammatory rheumatic diseases. To do this, the following research questions were formulated: 1. Which are the most commonly used physical methods in the treatment of rheumatic diseases? 2. What effects does the application of physical therapy have on the patient? 3. How does inflammatory rheumatic disease affect the patient's life? 4. Which procedures can the patient apply in the home environment? 5. Are patients satisfied with physical therapy? Qualitative methods were chosen for the research part of this work and a semi-structured interview was used as the technique for data collection. The respondents were asked 28 questions. The research sample consisted of 10 respondents. The respondents were chosen from rheumatologic surgeries in Tábor and Pelhřimov. They consist of 5 respondents with a diagnosis of Bechterew's disease and 5 respondents with a diagnosis of rheumatoid arthritis. The interviews were conducted as previously arranged in the respondents' homes or via email. The results show that in most of the respondents the disease occurred between the ages of twenty and thirty. The first symptom was a painful and swollen joint. They most often use disease modifying drugs, anti-inflammatory drugs, and analgesic drugs for the treatment. Examples of disease modifying drugs include Methotrexate, Sulfasalazine, Aflamil, and Arava. Aulin is among the most commonly used NSA. Enbrel was mentioned by the respondents as a preparation used in biological therapy. Among the regime measures which the respondents take to reduce symptoms, were most often mentioned home workouts, changes of diet, compresses, ointments, and other compensation aids. As regard physical therapy, the respondents were most satisfied with balneotherapy, namely with massages and warm packs. The most used method is magnet. Physical therapy primarily helps to ease pain and relax muscles and stiff joints. Nurses educate patients about drug effects, the changes that are to come in their lives and to which they need to prepare; about the importance of workouts, rehabilitation, and compensatory aids. The quality of the respondents' lives is substantially limited by pain, stiff joints, and severe fatigue. For these reasons, the respondents are unemployed and have to receive a disability pension. In the initial period, the respondents suffered from the feelings of anxiety, helplessness, hopelessness, and depression. The family plays an important role in this context, helping and supporting the respondents. According to the data obtained it can be concluded that physical therapy helps to customers with rheumatoid inflammatory disease to relieve from pain and stiffness in joints. The course of the disease can be influenced by regular workouts that can help ease the symptoms. In some cases, the symptoms disappear for some time. It is very important to appropriately inform customers about their disease, therapy, rehabilitation and the changes they will have to undergo in life. The doctors and nurses deal those problems in the rheumatic clinics and spas. This thesis points to common problems of patients with inflammatory rheumatic diseases.
The influence of physiotherapy for patients with ankylosing spondylitis treated in outpatient facilities and spa resorts

This Bachelor´s thesis is concerned with influence of physiotherapy at patients with Bechterew´s disease, out of them one group of research sample undergoes therapy in outpatient facilities and the second one undergoes all-embracing balneotherapy. In theoretical part is briefly described the stability of backbone, functional anatomy and kinezilogy of axial system. Next part is concerned to a detailed description of Bechterew´s disease, concerned with its categorization to the group of serum-negative spondylartritid, with definition and classification of the disease, its occurrence, genesis and progress, prognosis and clinical picture of the disease. The main aim of this thesis is to evaluate the effects of physiotherapy on patients with Bechterew´s disease who were treated in outpatient facilities and spa sanatorium. To achieve the goal was used the qualitative research, in which four participants took part, out of them two were treated outpatiently, namely in Rehabilitation and regenerative Borovany and in Outpatient clinic Medipont, s.r.o. Next two participants attended complex curative therapy in Berta´s spa Třeboň. The patients had different stadium of the disease in the range between II. V. stage of disability. During the introductory meeting was executed preliminary kinesiological examination, next they were asked to fill up the informed agreement and Functional index BASFI (The Bath Ankylosing Spondylitis Functional Index), which is used to considering the patient´s functional abilities to solve common daily activities and which reflects impact of the disease to the quality of patient´s life. After therapy´s ending it was executed final kinesiological analysis together with repeated filling out the Index, in order to evaluate success rate of the treatment in range of theirs functional skills. With participants were as well executed not structured interview, in which they communicate their subjective evaluation to the realized research and compared outpatient and spa therapy from their pont of view to their contribution to themselves. The results of preliminary and final researches and physiotherapy course are placed upon record in practical part of Bachelor´s thesis. The Bachelor´s thesis can be used as study material for students, medics and non-professional public as well to improve knowledge about influence of the physiotherapy at patients with Bechterew´s disease, as well to teach patients about possibilities of their treatment.
