Eating Habits of People with Multiple Disabilities

This thesis aims at description of basic eating habits of children with multiple disabilities and mapping methods for acquisition of these habits. The thesis then describes the physiological way of eating as well as individual parts of the involved digestive system. The objective is to describe, on the basis of contemporary domestic and world literature, individual learning strategies of most elementary eating habits, like chewing or swallowing, and compare these theoretical knowledge with my practical experiences acquired by working as a personal assistant for children with multiple disabilities in their home and school environment. Special attention is paid to orofacial stimulation in relation to the acquisition of individual eating habits and the possibility of using the concept of basal stimulation to improve their quality. The paper proceeds with alternative ways of nutrition for the persons with multiple disabilities, respectively percutaneous endoscopic gastroscopy and nasogastric tube. The thesis includes three case studies of children with multiple disabilities documenting the way for increasing the quality of lives of people with multiple disabilities by using treatment methods; special attention is paid on the food intake.
Stimulation of severely disabled persons using the therapeutic concept basal stimulation

Thesis should readers familiar with the concept of the basal stimulation and with people with heavy users. The practical part of the work will process the results of the questionnaires in the basic school Sluneční in Hostinném. This investigation will focus on the technical readiness of the respondents to work with the concept of the basal stimulation and on the application of this concept into the educational process, people with severly disabilities. The aim of my work is to introduce the concept of basal stimulation as a very effective therapeutic method applicable to work with people with severly disabilities and to verify this by investigation.
Application Basal Stimulation in Intensive Care Unit

Precis This graduation theses discusses about application of the concept of Basal stimulation in the work of nurses in intensive care. A theoretical part explains the concept of basal stimulation, briefly outlining the history of this concept and describes the different elements. Elements' name are include somatic, vestibular, visual, olfactory, auditory, oral and tactile - haptic stimulation. The work also highlights the importace of biographical history of the client. Research part determines the level of knowledges about basal stimulation between nurses in the ICU (intensit care units) and its use in the ICU at University Hospital in Hradec Králové. Concept of basal stimulation in seniors with utilization of psycho-biographical model of care of Erwin Böhm.

Basal stimulation is a pedagogical and nursing concept supporting development in people with short-term or long-term limitations of perception, communication and movement. Within the concept of basal stimulation, nursing care is structured so as to support preserved abilities of an elderly patient. The aim of basal stimulation is to understand the needs of an elderly patient, adapt to his/her communication style, give him/her space for orientation and development of his/her personality. The model assumes work with biography, which is the cornerstone of psychobiographical care model. The model is a great asset for nursing care, especially its positive effect on the actual elderly patient since the model sees him/her as a person with one's own biography.The aim of the study was to determine how Erwin Böhm's psychobiographical care model is used within the concept of basal stimulation. Another aim was to find out whether nurses actively work with Erwin Böhm's psychobiographical care model and how nurses can take medical history according to Erwin Böhm's psychobiographical care model. Furthermore, the following research questions were posed: How can nurses work with Erwin Böhm's psychobiographical care model? How do nurses take medical history from patients according to Erwin Böhm's psychobiographical care model? How is the concept of basal stimulation used in homes for the elderly?Qualitative research using the in-depth interview method was used in order to carry out the research part. All interviews were recorded on a tape recorder and then transcribed verbatim without linguistic adjustments. The research was conducted in homes for the elderly of Saint Zdislava and Saint Anne. Subsequently, transcribed interviews were processed by open coding technique, using the paper and pencil method. Individual categories and subcategories were created based on the generated codes. In total, 8 categories were indentified. Everything is shown in clear diagrams for each category.The research shows that the respondents have very good knowledge of and experience with the concept of basal stimulation. An interesting finding was that the respondents can apply the concept of basal stimulation in nursing care very well and can themselves choose appropriate techniques of basal stimulation. The results were not so surprising for Erwin Böhm's psychobiographical care model. The respondents have limited information about and experience with this model. The research also shows that the respondents take biographical anamnesis well and can work with it. All respondents agreed that the collection of biographical anamnesis is one of the most important aspects in biography.The research shows that the respondents have general knowledge of basal stimulation and biographical anamnesis. On the other hand, the respondents lack experience with Erwin Böhm's psychobiographical care model. Given the lack of literature about Erwin Böhm's psychobiographical care model, the output of this Bachelor's thesis will be a brochure on Erwin Böhm's psychobiographical care model. The brochure should provide nurses with sufficient information about.
Application of the element of basal stimulation in the nursing practice

Abstract The use of elements of the basal stimulation in nursing praxis The actual state Nowadays this concept belong among very accredited and very used concepts in nursing praxis and pedagogy in the countries of Europe. This concept deals with all areas of human needs and is used especially in taking care of premature infants, children and adults with intellect and somatic disability, people with different acute and chronic diseases, patients after brain injuries and clients with dementia. The aim of the concept is to organize the conditions of health care users in order to support their development, autonomy, and provide them with a chance to live their life in a highest possible quality. Objectives of the work: In this bachelor thesis there were stated two goals. The first objective of my work was to find out how the nurses orientate in the area of the basal stimulation. The second one was to find out how the nurses use the basal stimulation in different departments. The methodology: To gain and to process all the data needed for the empirical part we used qualitative research and we executed the semi-structured interview with the surveyed respondents on chosen departments. The research group consisted of 16 respondents. All of it were nurses working on internal, traumatology, and rehabilitation inpatient department of South bohemian region and nurses working at a Hospice of St. Lazarus in Pilsen. Results: After executing the analysis of the received data two main categories appeared. In the first category: "Orientation of nurses in application in area of basal stimulation elements in nursing praxis" appeared 8 subcategories of results of interviews with nurses. It was found out that nurses on the chosen departments know the term "basal stimulation", some elements of BS, but only 3 of them completed the course of basal stimulation. Within the second category "the use of basal stimulation by nurses on the chosen departments" appeared 5 subcategories. First half of the surveyed nurses do not apply elements of BS and the second half do apply only some of them. It implies that if all the nurses would have finished the course of BS they could have used the BS and significantly improve the nursing care with the application of the most appropriate elements of BS. The conclusion: The research implies that the medical staff is inadequately informed about the concept of BS. The results confirm that the concept is applied mainly by nurses who have finished the course of BS. Other nurses apply only some parts of it which they know from observing the educated medical staff applying the BS in their nursing praxis or thanks to the information they have from school or other sources. There is therefore no chance of quality provision of BS elements and correct education and communication with family which is very important when providing BS. Subsequently, there was created a summary of "Ten of the basal stimulation commandments".
Spiritual needs in the new concepts of care

The work focuses on the spiritual needs of seniors and their saturation in modern concepts of senior care. The work is theoretically aimed to clarify how the spiritual needs of seniors are reflected in the modern concepts of care. The first chapter is devoted to the old age and aging, it describes the changes associated with aging, characterizes the term dementia and describes the societal view on aging. The second chapter deals with the characteristics of the basic human needs. In the third chapter the focus lies on the developmental tasks of aging and the seniors´ adaptation to age as a spiritual task. The fourth chapter describes the concept of spirituality and constitutes the fundamental spiritual needs of seniors. The fifth chapter focuses on selected modern concepts of senior care. These are the Sensory activation by Lore Wehner, psychobiografic care model by professor Böhm, the concept of basal stimulation and the validation method Naomi Feil. The final chapter assesses the significance of spiritual needs and their saturation within the mentioned concepts.
Using the Basal stimulation concept in the care of children with combined disability.

Objective of this bachelor's thesis whose name is Using the Basal stimulation concept in the care of children with combined disability was to describe the integration of Basal stimulation concept in care of children with combined disability in the Centrum Bazalka, o. p. s in České Budějovice. Next, find the benefits of Basal stimulation concept from parents of children with combined disability and their view of this. Map out the contribution of the Basal stimulation concept from the point of view of workers from Centrum Bazalka, o.p.s Methods used: To obtain necessary information for this thesis it was selected the qualitative research method. The method of a semi-structured interview with nurses and parents was used to obtain valid data collection. Respondents: The first research group consisted of four nurses working in the Centrum Bazalka, o.p.s. in České Budějovice. The nurses were selected because they work in the Centrum Bazalka, o.p.s. in České Budějovice. The second research group consisted of parents of children with combined disability. The parents were selected because their chidren attend the Centrum Bazalka, o.p.s. in České Budějovice. Results: By the method of open coding 15 categories were established, categories 1-8 for nurses and 9-15 for parents. Nurses know the work with Basal stimulation? concept and they apply it in their practice very good. All of the nurses have a cours of Basal stimulation concept. The contribution of Basal stimulation concept is acording of nurses in stimulation of the perception of children and in the better nursing care. In the opinion of the parents of children with combined disability is Basal stimulation concept clearly positive for their children. None of the parents has the cours of Basal stimulation concept. Conclusion: In the opinion of workers of Centrum Bazalka, o.p.s. in České Budějovice is Basal stimulation concept very positive thing. The capital result of this thesis is from nurses opinion better nursing care thanks of the Basal stimulation concept. In the opinion of the parents of children with combined disability is Basal stimulation concept clearly positive for their children and they recommend the concept of Basal stimulation for other parents too.
Use of basal stimulation at anesthesiology department

The theme ?The Use of Basal Stimulation at the Anaesthesiology and Resuscitation Department? was chosen in order to map out the use of this nursing method by the nurses and the staff who I cooperate with. The theoretical part deals with the environment at the Anaesthesiology and Resuscitation Department where the basal stimulation is used and also with special characteristics of the nursing care. Further, it deals with monitoring patients, causes of consciousness defects occurrence and kinds of methods of basal stimulation and their importance for patients. The basal stimulation is scientific, pedagogy and nursing concept supporting perception, communication and motor abilities of the human. The issue of basal stimulation at the Anaesthesiology and Resuscitation Department lies in skills and knowledge of the nurse and its timely application, which does not have to be used only on patients with the Apallic Syndrome. The basal stimulation can be applied in the nursing process also to patients with any degree of consciousness defection who are hospitalized at the Anaesthesiology and Resuscitation Department in order to reach patient´s orientation of better quality as he/she is waking up from the unconsciousness in the environment where he/she is located. This environment is often very stressful for the patient. The basal stimulation can be applied both to adults and children. These days the basal stimulation is predominantly used on patients with the Apallic Syndrome. The aim of this Bachelor Degree Work is to map out the use of the basal stimulation on patients suffering from the consciousness defect at the Anaesthesiology and Resuscitation Department form the nurse?s point of view. There were set three research questions for the practical part. Research question no. 1 ? How do nurses feel the use of the basal stimulation on patients suffering from the consciousness defect who are hospitalized at the Anaesthesiology and Resuscitation Department? Research question no. 2 ? How do nurses use the basal stimulation on patients suffering from the consciousness defect who are hospitalized at the Anaesthesiology and Resuscitation Department? Research question no. 3 ? What is the knowledge of nurses who work at the Anaesthesiology and Resuscitation Department on the basal stimulation? The set aim of the work was fulfilled, the questions were answered. In the research part of the work there was found out endeavour of nurses to apply the stimulation not only on patients suffering from the Apallic Syndrome but also on patients suffering from consciousness defects of different reasons. The work shows limitations in theoretical knowledge and consequently in practical skills of nurses in the sphere of basal stimulation application. The practical part facts will result in the nursing standard focused on providing the basal stimulation by nurses at the Anaesthesiology and Resuscitation Department. This standard will be available to the ward sisters in Písek Hospital and Strakonice Hospital too. It can be a suitable help for nurses who care for seriously ill people and it can contribute to better quality of the nursing care which they provide.
The use of the concept of basal stimulation in special primary school

This bachelor thesis deals with the application of the Basic Stimulation concept at special primary schools, i.e. schools serving for children suffering from serious forms of mental disability, in combination with potential other disabilities. The basic stimulation concept was created by the German special pedagogue Prof. Andreas Fröhlich in the 1970s, for the education of pupils suffering from serious mental or combined disabilities, at special schools. It was applied in nursing by Prof. Christel Binstein in the 1980s. In this area it also used in the Czech Republic for the first time at the beginning of this century, thanks to PhDr. Karolína Friedlová.The objective of this bachelor thesis was to find out in which way basic stimulation may be used at special primary schools and how it can be incorporated into the education programme. In accordance with this objective, my thesis contains a definition of the basic terms, a brief historical outline of the development of basic stimulation and its theoretical foundations, and subsequently the particular techniques that the concept makes use of. The most essential passage is the theoretical definition of possibilities of applying the concept in the special pedagogical practice, particularly at special primary schools. This part is completed with three case studies that present the real application possibilities of the basic stimulation concept at three special primary schools in scope of the whole Czech Republic.
Comparison of nursing care about apallic patients in domestic and institutional care from carer?s viewpoint

As a result of urgent medicine acceleration and neurosurgery care improvement individuals that had minimum chance of life saving in the past survive brain insults. Increasing occurrence of people with apallic syndrome is a consequence of this progress. This thesis compares domestic and institutional care of apallic people. It examines whether the care quality and complexity are comparable. As the domestic care is more economical for the government it has to be supported in the times of insufficient funds for health care. Quantitative research technique of non-standardized interview was used for data collection. The research sample consisted of six healthcare workers and six family members caring for apallic patients. One respondent had a son in institutional care, however she took him temporarily to domestic care. Questions for healthcare staff and for caring family members were prepared separately. The results show that apallic patients in domestic care achieve substantially better results. The physical load is comparable, mental load is higher for family members. The basal stimulation, which is extremely important for apallic patients, is purposefully applied in institutional care only, as it is provided by trained staff there. The results of this work were presented at the 4th National Student Conference organized by the Faculty of Healthcare Studies of the Pardubice University.
