Clinical and genetic predictors of drug dependency in inflammatory bowel disease
Ďuricová, Dana ; Lukáš, Milan (advisor) ; Keil, Radan (referee) ; Špičák, Julius (referee)
IN ENGLISH Drug dependency in inflammatory bowel disease (IBD), Crohn's disease (CD) and ulcerative colitis (UC), is a specific disease phenotype which determines disease prognosis and hence may be used as a prognostic marker for treatment management. Drug dependency in IBD has been well described in corticosteroid treatment and recently also in infliximab (IFX) therapy. The aims of this thesis were: 1) to assess the occurrence of IFX dependency in paediatric and adult patients with CD; further to search for clinical and genetic predictors of IFX outcome and to evaluate the impact of IFX dependency on surgical rate; 2) to assess in CD patients the outcome of the first course of 5-ASA monotherapy with emphasis on 5-ASA dependency and to define clinical predictors of 5-ASA treatment outcome. We found that 66% of children and 29% of adults with CD became IFX dependent. The high frequency in paediatrics is in agreement with previously published studies, while the finding in adult patients indicates a lower rate of IFX dependency in the only study to date. Perianal disease and no bowel surgery prior to IFX start were predicative of IFX dependency in paediatric patients. In adult cohort, 2 genetic variants LTA c.207 A>G and CASP9 c.93 C>T were associated with IFX outcome, whereas no relevant clinical...

The first part deals with a brief description of the disease, and further gives basic information on the preparation of monoclonal antibodies, their structure and mechanisms of action. The second part of this work is focused on biological treatment. Biologics are divided into groups according to their target structures. In each of them, basic features of the most important drugs applied in this indication as well as the results obtained are summarized. Collecting and comparing literature data has been the basic method of work. Foreign- -language medical and pharmaceutical articles published by periodicals and various clinical studies have been the most important sources.

Inflammation and peripheral metabolism of glucocorticoids
Leden, Pavel ; Kment, Milan (advisor) ; Hampl, Richard (referee) ; Kršek, Michal (referee)
10 Abstract Glucocorticoids play an important role in regulation of inflammation and their immunosuppressive effect is widely used for treatment of inflammatory diseases. The biological activity of glucocorticoids depends not only on their plasma concentrations, the number of receptors and the responsiveness of the target cells but also on the local metabolism of glucocorticoids that is predominated by 11β-hydroxysteroid dehydrogenase (11HSD). Two isoforms of 11HSD are known. The isoform 11HSD1 operates in vivo predominantly as a reductase that increases the local concentrations of glucocorticoids by reduction of their 11-oxo derivatives. The isoform 11HSD2 is a pure dehydrogenase that inactivates biologically active glucocorticoids to their inactive 11-oxo derivatives. The published data concerning peripheral metabolism of glucocorticoids during inflammation were obtained mostly in in-vitro studies. The aim of the thesis therefore was: (1) to study peripheral metabolism of glucocorticoids during trinitrobenzensulfonic acid (TNBS) induced colitis in rat. (2) to study peripheral metabolism of glucocorticoids in biopsies from human ulcerative colitis (3) to examine peripheral metabolism of glucocorticoids during dextran sodium sulphate (DSS) induced colitis in rat and (4) to study peripheral metabolism of...

Augustin Navrátil and his disident activities
Kordíková, Marta ; Hlavačka, Milan (advisor) ; Randák, Jan (referee)
Augustin Navrátil This thesis addresses the life and work of Augustin Navrátil (1928-2003), a Roman- -Catholic activist from the Moravian township of Lutopecny near Kroměříž. A. Navrátil has not been subject of a complex publication yet, although he is the author of the largest petition demanding religious - and with that going hand in hand also civic - liberties in socialist Czechoslovakia (1988, 600 000 signatures). Followingly, this is the first attempt to biographically outline the personality and motivations of this farmer, politician (member of the party Lidova strana), man who signed Charta 77 but also a husband and father of nine children. The aim is to cover his life from his birth, over his family background to the beginnings and heights of his social engagement. The text will mainly deal with 22 open letters in which he drew attention to various illegal practices and actions of the Communist power, as well as with the three petitions he authored (1978, 1985 a 1988) and the samizdat (self-published) journal Křesťanské obzory (Christian horizons), which he published with a group of others from June 1988 to July 1990. In contrast to most Czech dissidents, he was never sentenced to a prison term for his activities, but he had to involuntarily undergo repeated psychiatric treatment. He was...

Metabolic study of sibutramine
Link, Marek ; Wsól, Vladimír (advisor) ; Holčapek, Michal (referee) ; Nobilis, Milan (referee)
Obesity represents a serious problem especially in American and European populations. Pharmacotherapy in combination with a reduced calorie diet is recommended for obese patients as a multi-modal approach to weight loss. Sibutramine hydrochloride monohydrate represents one of the few established and well-proven agents available for treatment of obesity. It is sold as a racemic mixture under the trade-name Meridia, Reductil or Lindaxa. It acts as a monoamine reuptake inhibitor. The weight loss of patients induced by sibutramine is thought to be due to a combination of serotonin- and noradrenaline-mediated mechanisms that increase both satiety and energy expenditure. In organisms, sibutramine is rapidly demethylated to form metabolites M1 and M2. These metabolites contribute largely to the pharmacological effects of sibutramine and the pharmacokinetic characteristics of M1 and M2 were thoroughly studied in human plasma. Although sibutramine is widely used for the treatment of obesity almost ten years, the published information on the further metabolic fate of metabolites M1 and M2 as well as on the elimination of sibutramine from the body is almost exclusively limited to package inserts of the product. To address this issue we determined the routes of elimination of sibutramine in humans via urine. LC-API/MS...
