The expression of miRNA in HPV-associated and HPV-independent head and neck tumors

Head and neck cancers represent a group of tumors with two different etiologies. The first type is associated with the viral HPV infection, the second one is virus-independent and it is associated with smoking and alcohol consumption as two main risk factors. Numerous studies show that HPV-positive tumors are more frequent in younger patients, as well as that the prognosis and overall survival of these patients is remarkably better. Therefore, the modification of the treatment is considered. For this, however, specific, sensitive and clinically relevant biomarkers for accurate identification of tumor etiology is needed. Suitable candidates for such biomarkers are miRNAs, small non-coding regulatory molecules stable in archived samples, that have been shown as differentially expressed in human cancers and the expression pattern seems specific for tumors of different origin. The submitted thesis focuses on miRNA profiling in HPV-positive and HPV-negative tonsillar tumors and cervical carcinomas with the aim to find out the differences between regulation of important carcinogenetic pathways of tumors of viral and non-viral etiology. Tumor-infiltrating T cells and their role in adoptive cell immunotherapy of cancer

Cancer immunotherapy has become a leading treatment modality in metastatic diseases. Although this novel therapy has changed the therapeutic algorithms and patients' outcomes in multiple malignancies, certain proportions of patients still fail to respond to these approaches. In our studies, we aimed to address the main mechanisms of tumor resistance to cancer immunotherapy. We have systematically defined the main challenges in adoptive cell transfer. We have focused on two key mechanisms of the tumor resistance to immunotherapy: poor trafficking of adoptively transferred immune cells into tumors, and the death receptor-induced apoptosis of the tumor-infiltrating immune cells. In our work, we have gone beyond the tumor tissue and searched for the immune cell populations and novel targets that would help to challenge the two mechanisms of resistance. Our data uncovered the therapeutic potential of the paratumoral tissue compartments and, thus, provided new avenues on how to challenge solid tumors by immunotherapy.
Methods of Detection, Segmentation and Classification of Difficult to Define Bone Tumor Lesions in 3D CT Data

The aim of this work was the development of algorithms for detection segmentation and classification of difficult to define bone metastatic cancerous lesions from spinal CT image data. For this purpose, the patient database was created and annotated by medical experts. Successively, three methods were proposed and developed; the first of them is based on the reworking and combination of methods developed during the preceding project phase, the second method is a fast variant based on the fuzzy k-means cluster analysis, the third method uses modern machine learning algorithms, specifically deep learning of convolutional neural networks. Further, an approach that elaborates the results by a subsequent random forest based meta-analysis of detected lesion candidates was proposed. The achieved results were objectively evaluated and compared with results achieved by algorithms published by other authors. The evaluation was done by two objective methodologies, technical voxel-based and clinical object-based ones. The achieved results were subsequently evaluated and discussed.

Title of bachelor's thesis: A case report of physiotherapeutic care of a patient after surgery for subtrochanteric chondrosarcoma. Objectives: The objective of this thesis is a collection of theoretical and practical knowledge about chondrosarcoma and their application to a case study of patient, who is two years after the operation of chondrosarcoma on the proximal femur, where they have used a graft from the fibula bone on the right leg. Summary: The bachelor's thesis is split into two sections: general section and a specific section. The general section describes the pathophysiology of overall tumours and then in each part the topic is narrowed down to just chondrosarcomas. It gives information about the epidemiology, etiologic, classification and clinical picture of a patient with chondrosarcoma. Also it describes a way to prevent, cure and a following rehabilitation. The specific section contains a case study of a patient, who suffered this illness and it has been two years after the operation. The sections include the input and output kinesiology analysis, short-term and long-term plan of a therapy, the design of the therapy, the description of course of individual therapeutic units and a final evaluation of the effect of the therapy. Analysis of circulating markers in patients with solid tumors

Circulating cell-free DNA (cfDNA) and its tumour-derived circulating tumour DNA (ctDNA) fraction are considered an innovative prognostic and predictive biomarker in oncological diagnostics. Many studies have demonstrated higher levels of cfDNA concentration and integrity, as an indicator of the amount of ctDNA in cfDNA, in body fluids from patients with cancer diseases in comparison with healthy individuals, which suggest its potential as an effective biomarker for monitoring of the tumour dynamics. This study focused on optimisation and validation of measurement methods later used for analysis of cfDNA concentration and integrity in blood samples from patients with four different solid cancers. Two different commercial isolation kits have been tested in plasma and serum samples. Quantitative real-time polymerase reaction (qPCR) and PicoGreen dsDNA assay were optimized to effectively quantify low concentrations of cfDNA, subsequently compared to each other and to droplet digital PCR assay tested on selected samples. The concentration and integrity of cfDNA from plasma samples of breast, ovarian, colorectal and pancreatic cancer patients were evaluated. Tumor in vitro chemosensitivity and resistance assays (CSRA) using flow cytometry

In vitro chemosensitivity and resistance assay determine the sensitivity of a specific tumor after a specific treatment administration in an experimental setup. A heterogeneous population of cancer cells is exposed to various approved anticancer drugs in short-term ex vivo and their combination thereof. The effect of each drug is then determined based on the viability of specific tumor cells allowing for individual patient treatment using a precise combination of drugs. This approach is an example of the personalized medicine principle, which is focusing on the adjustment of diagnostic procedures and treatment of a specific patient. Therefore, its goal is to avoid treatment failure in patients with poor response to the statistically most effective treatments based on randomized clinical trials. The number of viable cells determined by the flow cytometry provides very accurate statistics for multiparametric analysis. A necessary prerequisite is the presence of dissociated cancer cells in a single cell suspension. This is different from cloning methods, where tumor colonies grow on agar media, or from histocultures, which are specific with its three-dimensional tissue cultivation. Immunogenic cell death

According to the danger model, the immune system is activated by endogenous molecules known as danger-associated molecular patterns (DAMP) that are externalized from the interior of a dying cell to the cell surface or released into the extracellular space. Due to the loss of plasma membrane integrity a necrotic cell death as well as several types of proinflammatory programmed cell death are considered to be immunogenic, whereas apoptosis, on contrary, has been initially defined as a tolerogenic type of cell death. However, under certain circumstances, the immune response can be initiated by an apoptotic cell after exnternalization of DAMP molecules by newly described secretory pathways. This phenomenon was observed on tumor cells as a result of some widely used therapeutic modalities and is known as immunogenic cell death (ICD). Nomenclature of selected types of cell death is part of this thesis. The aim of this bachelor thesis is to provide an evidence of the experimental support for ICD theory during in vivo initiation of the immune response. I will evaluate the correlation between ICD and the induced exposure of DAMP molecules on the surface of tumor cells or their secretion to the extracellular space.
