Thesis title: People suffering from communication disorders in general practitioner's office Author: Mgr. Květa Trčková Department: The Department of Special Education Subject: Special Education Supervisor: Doc. PaedDr. Jiřina Klenková, Ph.D. The thesis focuses on the problematics of communication disorders and in what way general practitioners encounter it in their office, especially with specific number of their patients considering their age, sex, and active speech/language pathology care. The thesis is divided into theoretical and research part. The theoretical part describes characteristics of communication disorders which are encountered by a general practitioner on a daily basis. More attention is given to aphasia, as there is a subsequent qualitative research. The research part is divided into a quantitative research that verifies set hypotheses, and a qualitative research which shows the communication disorder from a more specific point of view by using five different case interpretations. The research file included 1054 patients who came in the general practitioners office during 22 months, usually repeatedly. There have been four hypotheses set for verification. Title: Casuistry of physiotherapeutic care of patient with diagnosis ischemic stroke. This bachelor thesis concerns in stroke. Thesis is divided into general and special part. General part consists of anatomy and physiology of cerebral blood supply, functions of cerebral lobes and remarkable ability of the nervous system - neuroplasticity. Second half of general part describes ischemic stroke - clinical state, pathophysiology, diagnostics, differential diagnostics and therapy. At the end of general part there is section devoting neurorehabilitation. In special part there is worked out casuistry of physiotherapeutic care of patient after ischemic stroke. This part of thesis works with data from practise which took place in Military university hospital from 7th January to 1st February 2013. The theoretical part explains some general information about a speech therapy and neurolingvistics. It characterises all the disruptions in communication skills. Case study of physical therapy care of patient after ischemic stroke

Title: Case study of physiotherapy treatment of a patient after the ischemic stroke Objectives: The main aim of this study was to verify my ability to work with the patient, his examination, making conclusions, proposed treatment and its subsequent implementation. Other aim was mapping of the literature on the subject. Methods: The first part of my bachelor thesis is theoretical and it is aimed on stroke. There are described anatomy of cerebral artery, knowledge about stroke, its clinical symptoms and treatment options in this part. The description of spasticity with its influence on therapy and its treatment and the techniques used in physiotherapy are also there. The Brocca aphasia is described in the last section. The second part is the case report of a patient P. V. with diagnosis: ischemic stroke with dextral symptomatology. This special part is based on two weeks' work with the patient in the hospital (Ústřední vojenská nemocnice - Vojenská fakultní nemocnice Praha). This work lasted from 9th January 2012 to 3rd February 2012. Nurses´and speech therapists´cooperation while treathing clients suffering from aphasia

The main part of this thesis is cooperation between a nurse and a speech therapist themselves. The relationship between a nurse and a doctor is very important, communication is a part of the cooperation. The communication between a nurse and a doctor is important in all possible ways written and oral, gestures, facial expression, and body language. Nurses are an important part of the doctor's work. Just few doctors have that in their minds, most of the doctors take the nurses for granted. A nurse contacts the speech therapist via telephone and tells him about the patient with speech disorder and his medical history. A speech therapist put the patient's record into the database and plans their physical examination. At the first session the speech therapist meets the patient and examines him. The biggest issue is that the patient is not able to communicate with the others that is why they often use nonverbal communication or body language. The patient is mostly nervous and aggressive due to the disorder. They do not understand the people around them and cannot express their needs. They realize they cannot speak properly. At this moment a psychologist may join the session. The psychologist talks to the patient and tries to solve their problems that could be depression caused by the communication disorder. The speech therapist prepares materials for the next session. The speech therapist hands in the materials to a nurse. These materials can be worked with the patient and the therapist or the patients and their family or the patients themselves. The aim of the quality research of this thesis was to outline the nurses and the speech therapists cooperation. Two research questions belong to this research. The first question is: What's the cooperation between speech therapists and nurses while taking care of patients suffering from aphasia? The second question is: What are the aids used from improving the quality of communication with the patients suffering from aphasia? For completing the aim we completed a half-structured interview. The interviews were done in the České Budějovice hospital. The research was quality-based, half-structured interviews with nurses. The interview consists of nineteen questions aimed on communication, nursing a patient suffering from aphasia and on the cooperation between a nurse and a speech therapist. The research was authorized by a supernurse and charge nurses at the department. The questions were asked based on the theoretical part of the thesis which was written based on technical literature and the internet. The research was accomplished in June 2015. Five nurses of each department were chosen to make the interview with. Each nurse was interrogated separately. The nurses, who completed the interview, work at one of two departments in the České Budějovice hospital. Those departments are neurosurgical and physical therapy. These nurses often come across with speech therapists and patients who suffer from aphasia, which is why the nurses have to cooperate with the speech therapists. The age of the nurses, who became a part of the practical part of this thesis, is between 35 and 51 years old.
Application development – Aphasia dictionary 2.0

The theme of the thesis is the development of the appliacation Aphasia dictionary 2.0. Aphasic Dictionary 1.0 is an application that our project team has developed in terms of side specialization. It is a logopaedic therapeutic tool for patients with aphasia which contains entries in the dictionary with the visuals accompanied by a soundtrack. This device helps make the therapy with expert more attractive and it allows home training for the pacients. Now the team plans to develop a second version which should be comprehensive and professional manner. The aim of the thesis is to prepare the data for this project. Specifically I am going to carry out research among users analyse the changes compared to the first vision and put together a project plan.
Application development Aphasia vocabulary

The theme of this thesis is development of multimedia mobile applications. Specifically, applications in the field of rehabilitation and re-education of patients suffering from aphasia. The aim of this thesis is to determine the application parameters, which in practice could be used by people, who suffer from aphasia and all those who assist them in therapy To this end, was developed the first version of Aphasia vocabulary. By the medium of Aphasia vocabulary was then conducted research survey at pacients with aphasia. The research assesses the suitability of various multimedia applications and controls aphasic dictionary while working with a speech therapist with the patient. It is a qualitative research based on the method of interviewing and participant observation at a standard therapeutic practice. The result of the research is a set of recommendations for the development of mobile applications for people with aphasia. Conclusions are useful in the creation of rehabilitation and re-education materials.

The theoretical part has the character of a compilation and its aim is to define the concept of aphasia and the causes of its origin. Further the thesis deals with the classification and diagnostic of aphasia and the possibilities of its therapy. The thesis combines the knowledge of the field of health care with knowledge of special education. The practical part includes the detailed casuistry aphasic patient and a description of one of the possibilities motivational activities for aphasic patients.
The issue of communication and nursing care at the patient with aphasia at the neurology department

Communication is very importantfor providing high-quality nursing care.If the patient is unable to communicate there can occur a lot of misunderstandig in the nursing care process.The theoretical part is focussed on the issue of aphasia in general, on communication with the patient affected by aphasia and his or her family, on alternative means of communication and on saturation of needs connected with the speech disorder.The goal of the work was to find out what problems nurses providing aphasia patients with the nursing care met, next, whether they are well informed about the selection of aids enabling effective communication and whether they use them while caring for the patient with aphasia. We also wanted to find out more about the mutual cooperation of nurses and the speech therapist in the process of the communication development of the patient with aphasia.The last goal was to find out if the family is satisfied with the education in the sphere of using effective communication skills for the communication with their dear one.In the process of research inquiry in the empiric part there was used the quantitative method with anonymous questionnaires which were defined for nurses working at ictus centres, cerebrovascular complex and standard neurology departments. After getting information on the issue of satisfaction with the use of effective communication skills, the qualitative inquiry followed. In the research was used the method of non-standardized interview with seven family members of the hospitalized patients with aphasia.
