The influence and possibilities of physiotherapy in patiens after oncological-surgical interventions through the thoracic way - its specifics in the preoperative, acute postoperative and outpatient period

Diplomová práce Vliv a možnosti fyzioterapie u pacientů po onkologicko-chirurgických zákrocích hrudníku - její specifika v předoperačním, akutním pooperačním a ambulantním období Abstract Lung tumor diseases are the most frequent tumors in the world. In men it is the most common tumoral disease in incidence, in women it occupies the third place. Bronchogenic carcinoma is the most frequent one, which includes tumors of trachea, bronchus and lung parenchyma. In most cases, the treatment requires surgical removal associated with hospitalization and subsequent rehabilitation. The theoretical part addresses oncologic surgical issues of lung tumors. A major part of the theory is dedicated to the options of physiotherapy and its specifics in particular parts of rehabilitation care. The practical part observes two groups of patiens and comparatively evaluates the effect of the therapy in particular parts by objective and subjective tests.
Comparison of performance of pediatric patients with diagnosis of cystic fibrosis before and after a lung transplantation

Title: Comparison of performance of pediatric patients with diagnosis of cystic fibrosis before and after a lung transplantation. Objectives: To compare selected parameters of pediatric patients as an index of physical condition in time before and 1 year after transplantation. To compare a group of patients that undergone longtherm intensive physiotherapeutic treatment before transplantation with group of patients that did not using spirometric parameters FVC, FEV1 and BMI. To compare subjective signs of dyspnea between these groups using selected questionnaires. Methods: This theses was attended by 15 probands (7 boys and 8 girls) in age between 12 and 18 years. The data were collected by using selected dyspnea questionares and by using hospital documentation files taking place at the Motol University Hospital. The data were statistically analysed using Jamovi programme, mostly using correlations and paired T- test, and graphically analysed by using MS Excel programme. Results: We found changes of spirometric parameters in the level of significance of p < 0,001 (FEV1) and p = 0,003 (FVC) after transplantation in comparison with time before transplantation. Martina Havlová Title of bachelor thesis: Physiotherapy Focused on Physical Activity Intoleration in Patients with Chronic Obstructive Pulmonary Disease Abstract: The thesis is divided into theoretical and practical part. The theoretical part is written in the form of literary research. It begins with a brief introduction to COPD, etiology, pathogenesis, epidemiology, and classification. Substantial part is devoted to clinical manifestations of the disease. Furthermore, it deals with physical activity in patients with COPD, which is the central topic of the work. Last but not least, lung rehabilitation and treatment options are mentioned. The practical part of this work is focused on influencing the intolerance of physical activities of a patient with COPD. I set up a therapeutic unit to increase physical exercise tolerance. I verify the effectiveness of the therapy in the form of a case report, which contains the input and output examinations. The result is not only a positive effect of this therapy on physical exercise tolerance, but also on other symptoms that are often associated with this disease. Rehabilitation in lung transplant candidates

Lung transplantation is the method of treatment for patients with end-stage lung disease. The main theoretical part of this bachelor's thesis is focused on the possibilities of rehabilitation in lung transplant candidates. This rehabilitation concept includes patient's education, individually designed exercise program, techniques of respiratory physiotherapy, nutrition therapy and psychological support. The practical section of this bachelor's thesis incorporates a case report of a lung transplant candidate who underwent 4 months of preoperative rehabilitation program. Tha main difference after completing preoperative rehabilitation was found in maximal inspiratory muscles strength, which increased of 17,71 cmH2O.
Dynamics of thorax in patients with COPD, its influence through POWERbreathe

This diploma thesis deals with chronic obstructive pulmonary disease (COPD) and its influence on the musculoskeletal system, lung transplantation and pulmonary rehabilitation. It discusses the role of POWERbreathe, a breathing training device, which is primarily used to strengthen the inspiratory muscles. The practical part was performed as a randomized control pilot study to assess the effect of POWERbreathe on the strength of the inspiratory muscles, the thorax dynamics and selected pulmonary function. The aim of the study is to determine the benefits of using the POWERbreathe for COPD patients during the pre-transplant period. In total, 18 patients with COPD IV. grade participated in the study (6 women and 12 men) with an average age of 59.8 ± 5.53 years. Probands were divided into two groups, only one of which used a POWERbreathe. Pulmonary functions (FEV1, VCin, FVC, PIF), muscle strength (SIndex), and chest dynamics (chest disturbances in the mezosternal and xiphosternal region with maximum inspiration and exhalation and respiratory amplitudes in these areas) were evaluated. The results did not show a significant inter-group difference in the strength of the inspiratory muscles. Importance of physiotherapy for patients after surgery for colon cancer

Physiotherapy is an important component of modern perioperative care strategies that aim to promote postoperative recovery via early enforced mobilisation. Current recommendations for perioperative rehabilitation are provided by ERAS (Enhanced Recovery After Surgery) programmes, highlighting importance of preoperative patient education and training, structured mobilisation plan and maximal support of patient's motivation. To survey patient's experience of provided physiotherapy we created a questionnaire for 3rd Department of Surgery, 1st Faculty of Medicine, Charles University in Prague and Motol University Hospital inpatients undergoing elective abdominal surgery for colorectal cancer. Results suggest regular provision of professional physiotherapy in early postoperative care, but also common preoperative absence of patient physiotherapy education and possible insufficiency in patient's understanding of the rehabilitation process. Regarding the small number of respondents the results the problem needs further investigation.
The significance of physiotherapy after the surgeries of tumoral processes through the thoracic way

Tumoral diseases of the lungs are one of the most frequent causes of hospitalization and necessary surgical interventions in the chest area. The most common is bronchogenic carcinoma, which is mentioned as the most common malignancy among men in incidence and mortality. In women it occupies the third place. Worldwide, it results in 20% of all deaths from oncological causes. The main theoretical part of this bachelor's thesis is focused on the possibilities of physiotherapy with patients after chest surgery emphasizing respiratory physiotherapy. Part of the practical section of this bachelor's thesis reviews questionnaires distributed to the patients with the aim of analyzing patient participation, the quality of and satisfaction with their preoperative and early postoperative rehabilitation and interest in outpatient rehabilitation. A case report of a patient who underwent chest surgery due to a lung cancer is added.

Title of thesis: The possibilities of physiotherapy in patients with Myasthenia gravis. Diagnostics and therapy. Abstract: This bachelor thesis discusses the possibilities of physiotherapy in patients with the neurological disease, Myasthenia Gravis. A physical therapy can be an important part of the treatment in myasthenic patients. The theoretical part of the thesis includes all the important information about Myasthenia Gravis - clinical symptoms, pathogenesis, diagnostics, treatment and selected physiotherapeutic methods. The practical part contains the case study of two selected pacients. The goal of the practical part was to find out what physiotherapeutical methods are suitable for patients with Myasthenia gravis and if a regular physical activity contributes to good health. The therapy was divided into two parts where one part took two weeks. The standardized scores were used to compare the results before and after the therapy. These scores are described in the theoretical part of the thesis. The output examination confirmed an improvement in both cases. BACHELOR THESIS ABSTRACT The title of the work: Importance of Postural Activation of the Diaphragm in Patients with Chronic Obstructive Pulmonary Disease Bachelor Thesis Abstract: This bachelor thesis on the topic of the postural activation of the diaphragm in patients with chronic obstructive pulmonary disease consists of a theoretical and a practical part. The theoretical part includes a description of the physiology of breathing - breathing is considered a motoric function here. The theoretical part occupies itself with a description of kinesiology of breathing and the breathing pattern. A separate chapter is devoted to the diaphragm, its anatomy, and its postural breathing function. In this vein, the thesis presents the notion of the deep stabilizing system of the spine and the possibilities of affecting it therapeutically. Further, it defines the chronic obstructive pulmonary disease, characterizes its effects, especially with regard to physiotherapy, and presents the possibilities for a complex treatment. As regards the complex treatment, the thesis focuses on the non-pharmacologic alternative, i.e. lung rehabilitation.
