Respiratory physiotherapy - part of a comprehensive treatment of Duchenne muscular dystrophy patients

Duchenne muscular dystrophy is a progressive muscular disease. This serious disease attacks not only the musculoskeletal system, but also the respiratory system. Respiratory system is one of the most influenced systems. The risks of respiratory problems increase with deteriorating condition of the patient due to muscle weakness. This work is focused on treatment of respiratory system. The aim of treatment is early prevention and solving of complications. These complications include not only scoliosis and cardial problems, but mainly inefficient cough, night hypoventilation, irregular night breathing and failure of respiratory system even during the day. The aim of the respiratory physiotherapy is to keep the right functionality of lungs for the longest time and to choose the correct method of the rehabilitation following the patient's condition deteriorating. Interventions suitable for the patients with the DMD are techniques that leads to increasing lung volume, manual and mechanical techniques of assisted coughing, and noninvasive ventilation. NIV is one of the preferred methods of treating nowadays. Another method is continual invasive ventilation and ventilation with tracheostomy. Annotation This thesis deals with the influence respiratory function in children with spinal muscular atrophy (SMA). The theoretical part provides an overview of respiratory physiotherapy techniques that can be used in the care of the airways in people with SMA. The practical part deals with the question whether it is possible using the selected techniques of respiratory physiotherapy after six weeks of training to affect ventilatory parameters in children with SMA. The group of six probands with SMA I. - III. type in the range of the age from 3.5 to 12 years participated in this study. To objectively assess changes was performed spirometry efore the beginning of the therapy and after its conclusion. The main therapy was daily training with inspiratory breath simulator CliniFlo. Importance of physiotherapy for patients after surgery for colon cancer

Physiotherapy is an important component of modern perioperative care strategies that aim to promote postoperative recovery via early enforced mobilisation. Current recommendations for perioperative rehabilitation are provided by ERAS (Enhanced Recovery After Surgery) programmes, highlighting importance of preoperative patient education and training, structured mobilisation plan and maximal support of patient's motivation. To survey patient's experience of provided physiotherapy we created a questionnaire for 3rd Department of Surgery, 1st Faculty of Medicine, Charles University in Prague and Motol University Hospital inpatients undergoing elective abdominal surgery for colorectal cancer. Results suggest regular provision of professional physiotherapy in early postoperative care, but also common preoperative absence of patient physiotherapy education and possible insufficiency in patient's understanding of the rehabilitation process. Regarding the small number of respondents the results the problem needs further investigation.

The possibilities of physical therapy in patients with Pierre Robin sequence

Orofacial clefts are among the most common craniofacial deformities and could be associated with serious anomalies. The rare condition - Pierre Robin sequence is classified into these deformities, due to presence of isolated cleft palate. Pierre Robin sequence is defined as a triad of symptoms: micrognathia, glossoptosis and airway obstruction. These symptoms were described in 1923 by French stomatologist Pierre Robin. In 70s the original name" Pierre Robin syndrome" was changed in "Pierre Robin sequence" because of the sequence of defects development. Today, many authors use modified definition of the sequence. They add the "U" or "V" shaped cleft palate and feeding disorders to original triad of symptoms. Pierre Robin sequence is usually classified into 3 groups: isolated form, syndromatic form and form additional to other malformations without knowing syndrome diagnosis. The treatment therapy in patients with Pierre Robin sequence is divided into conservative or invasive approach. Choosing the approach needs cooperation of many specialists, who should be part of multidisciplinary team. Although it is not usual or frequent, the physical therapist should be involved. BACHELOR THESIS ABSTRACT The title of the work: Importance of Postural Activation of the Diaphragm in Patients with Chronic Obstructive Pulmonary Disease Bachelor Thesis Abstract: This bachelor thesis on the topic of the postural activation of the diaphragm in patients with chronic obstructive pulmonary disease consists of a theoretical and a practical part. The theoretical part includes a description of the physiology of breathing - breathing is considered a motoric function here. The theoretical part occupies itself with a description of kinesiology of breathing and the breathing pattern. A separate chapter is devoted to the diaphragm, its anatomy, and its postural breathing function. In this vein, the thesis presents the notion of the deep stabilizing system of the spine and the possibilities of affecting it therapeutically. Further, it defines the chronic obstructive pulmonary disease, characterizes its effects, especially with regard to physiotherapy, and presents the possibilities for a complex treatment. Adenoid hypertrophy and adenodectomy from the viewpoint of the respirotory physiotherapy

The aim of the thesis is the examination of respiratory and postural changes on examined group of children after adenoidectomy and evaluation, whether it is recommended in post- operative phase to indicate respiratory physiotherapy for the examined children. The theoretical part of the thesis deals with the anatomy, etiology and symptomatology, which accompanies adenoid hypertrophy. Furthermore, there is a description of the kinesiology examination and complete methodology of respiratory physiotherapy and more. In the practical part of the thesis there is a statistic analysis of the examination of 20 children after adenoidectomy and control group investigation. Examination content is the thoracic respiratory excursion (breathing amplitude) and orientational examination of lungs function (PEF, FEV 0,5, FEV 0,75 and FEV1) using digital Peak Flow Meter. The part of the thesis i also kinesiology analysis and questionnaire filled up before adenoidectomy and 3 weeks after operation.
Respiratory physiotherapy in neurology

Pneumonia is the leading cause of death in a large proportion of neurological diseases. Respiratory physiotherapy serves as a non-pharmacological option for supporting airway hygiene. However, current awareness of respiratory physiotherapy in neurological patients is not satisfactory. The aim of this thesis is to verify the effectiveness of expiratory muscle strength training (EMST) in three neurological diseases: multiple sclerosis (MS), Parkinson's disease (PD) and multiple system atrophy (MSA). The results of this thesis show: 1) EMST led to a significant increase in expiratory muscle strength and voluntary peak cough flow in patients with MS. Therefore, we have shown that EMST is an effective method of respiratory physiotherapy in promoting airway hygiene in patients with MS. 2) We found in a demographic study that the PD healthcare model in Czechia is suboptimal. The utilization of a respiratory physiotherapy is low, and therefore its reorganization is needed. 3) We developed and patented a mobile application SpiroGym which provides visual feedback during training and sends the training results via a secure server directly to the therapist for telemedicine needs.
