Case study of physiotherapy treatment of a patient with lumboischiadic syndrome with radicular irritation S1

Title: Casuistry of physiotherapy care for patients diagnosed with lumboischiadic syndrome with S1 radicular irritation. Objective: Acquisition of theoretical knowledge of kinesiology, functional and structural failures of lumbar spine and physiotherapy approaches used in this issue. In a special section has been selected to develop a case study patient during work placements. Method: The general part of bachelors thesis contains a theoretical knowledge of kinesiology lumbar spine, as well as disorders of the axial system and diagnostic and therapeutic approaches to the back pain issue. The practical part includes the case of a patient diagnosed with the syndrome lumboischiadic S1 irritation. Results: Alleviate pain, improve overall fitness and mental condition. Conclusion: Indications of physiotherapy in patients with lumboischiadic syndrome proved beneficial in terms of overall improvement of health of the patient. Prevention of back pain in adults using a computer

Computers in modern times have become part of our lives, but apart from benefits they bring many medical complications as well. This paper provides a brief overview of existing knowledge and studies, dealing with back pain in the adult population working with computers. In the theoretical part, the emphasis is on prevention and treatment of back pain, which is very intertwined. The practical part presents the results of research, which had the form of a checklist answered by 50 men and 73 women. The results show high prevalence of back pain of computer users, many differences are to be found in the characteristics of pain of men and women, ages and dependence of pain intensity on movement habits of respondents. The work points to the significance of the problem, which is important for the society. It stresses the need for an active approach of computer users and their employers. Periradicular therapy in the Hospital Český Krumlov a.s.

This bachelor thesis focuses on the issue of backache and its treatment. Backache is one of the most common diagnosis. The most affected are usually people in their productive age and therefore the economic and social consequences are very significant. There are many techniques used in diagnosis of backache, easier and cheaper techniques are used in the first place. The diagnosis consists of examination of the patient, medical history and clinical examination. The treatment depends on type and progress of the illness. The basic division is acute (4 to 6 weeks) and chronic case (more than 3 months). Another division is common backache, nervous radicular pain and serious illness of spine. I will mainly focus on nervous radicular pain. The basis of the therapy should be active treatment accompanied by physiotherapy. In addition to this therapy, pharmacological treatment is also used. The pharmacological treatment consists of application of nonsteroidal anti-inflammatory drugs, analgesic and also antidepressants. If the conservative therapy is not effective, it is possible to use the least invasive techniques. The most popular one is the periradicular therapy under CT guidance, which is also the subject of this thesis. During the periradicular therapy under CT guidance the needle is being inserted to intervertebral foramen in the area of radicular affected nerve where lytic mixture is then being applied. The lytic mixture is sometimes replaced by ozone with the same effect. When the PRT was first used, it was mainly applied in the area of lumbar spine. Nowadays, it is usually used in treatment of lesion in the area of cervical spine. The objective of this thesis is to describe anatomy of the examined area, physiological and pathological state, the main indications for the periradicular therapy and methods of implementation including the used tools and pharmaceutical. Following part is focused on development of application of this method in the Hospital of Český Krumlov, a.s. the radiodiagnostics department, and to analyse the effectiveness of the treatment. I have conducted analysis and processed data of performed periradicular therapies in the Hospital of Český Krumlov, a.s. from 2010, when the method was introduced, up to 2015. I have made a questionnaire survey of 40 people selected by chance and then I processed the data in order to create a general overview of obtained results. The result of the thesis is that the number of performed periradicular therapies is steadily increasing. One-third increase was recorded from 2010 to 2015. The number of women patients prevails, 53% of patients in the given period were women. The most common age category is 50 to 59 years, this category constitutes 37% of all patients. It was found out that most of the patients were treated just for one or two times. Only 7% of the patients were treated for more than two times. When processing the data from my survey, I have found out that most of the patients are satisfied with the effectiveness of the therapy, which corresponds with the results quoted in the bibliography.
The possibilities of using ointments containing industrial hemp to treat skin diseases

Medicinal hemp can influence symptoms of many diseases. It seldom causes total recovery; however the refilling of a healing effect of hemp can reduce occurring of some diseases and prevent many others. The key to successful hemp usage as a medicine is in systematic using and also in frequency of administration. The following diseases were chosen because hemp was already used there and also was proved its effectiveness towards symptomatic relief. In the first part of my baccalaureate thesis is introduced a historical and scientific survey of hemp and its usability for medical purposes. In the practical part of my work is mentioned a qualitative research dealing with its potential influence. The aim of my work is to propose already mentioned facts in a form which is available to the general public as well as contributing other interested persons to further exploring.
Influence of compensatory exercises in hockey players focused on pain conditions of back

The theory is aimed to familiarize the given issues. This part describes the basic anatomy of the spine, including the biomechanics of walking. Great attention is given to the sensorimotor stimulation and the options of its usage. The practical part was processed by means of qualitative research. The research part includes 5 respondents who play ice hockey and who suffer from back pain. In the practical part the anamnesis was removed and a functional diagnosis was executed. Standing was examined by complexion and a dynamic test in the standing position was carried to verify the possible infringement of the musculoskeletal system. Another two parts were testing of the shortened and weakened muscle groups and the stereotype of activation of specific muscle groups. The probands were educated on the issue by means of pictures and text; others additionally underwent palpation examination including the application of soft techniques and the use of PIR. The therapy included five visits over some time. Some probands were educated on the possible activation of HSS in the future therapies; a practical demonstration including exercises was demonstrated to some of the probands. At the end of each visit (15) a visual analogue scale (VAS) was collected to evaluate the pain perception. Some probands were examined, rehabilitated and assessed in groups, some as individuals. In terms of overall assessment of the possible improvements in removing the issue an output test of functional diagnosis was carried out and VAS was collected after the final fifth visit. The data were obtained by collecting VAS during the individual visits and through these data an overall pain perception in the context of possible removal was evaluated. The aim of this bachelor's thesis is to survey and process how back pain can be affected. The collected data shows that all the individuals who underwent group or individual therapy achieved a positive result. Back pain is typical when having poor posture and the increasing tension in the muscle tissue and the formation of so-called trigger points (TrPs) occurs.
The influence of the sensormotoric stimulation by using special tools and physiotherapeutic practices on the feet of patients with the recurrent vertebrogenic syndrome

Sensorimotor stimulation is closely linked to vertebral algic syndrome and is used for the treatment of functional disorders of the musculoskeletal system. The methodology consists of many exercises from which the most important are the elements in the vertical. Due to sensorimotor stimulation it comes to the incensement of afferents from the skin exteroreceptors and proprioreceptors, which are based on the muscles and joints. Therefore we can say that thanks to the sensorimotor stimulation, which is based at the bottom of the foot, we can prevent the formation of functional disorders of the musculoskeletal system, thus we can positively influence the recurrent vertebral algic syndrome. The purpose of the theoretical part is to provide information on the effect of the sensorimotor stimulation on the vertebral algic syndrome, provide general information about the anatomy of the spine and legs, and at last provide information about the origin of the disease, methodics and physiotherapeutic procedures while treating the vertebral algic syndrome on the basis of sensorimotor stimulation. The specialized part consists of a qualitative research, which compiles by case studies of three people. An input and output kinesiology examination was performed to the patients. Also, there was taken medical history of the patients. The therapy lasted for four months. I met the patients two to three times a month individually, depending on their time. The therapy took place at the Rehabilitation and regeneration center in Borovany. The aim of the specialized part was to outline whether the sensorimotor stimulation and other physiotherapy practices have a positive effect on the pain in the spine and other vertebral algic syndrome. The supplements include photo documentations of patients at the beginning and at the end of the therapy and also examination of the bottom of the feet of patients at the beginning and at the end of the therapy as well. Vertebral algic syndrome is a very extensive topic and certainly could be described in more detail and would expand in other professional work. This topic is very interesting and wide for further research and therapy.
The issue of prevention and early detection for patients with vertebrogenic syndrome in the cervical spine

This bachelor thesis deals with preventive measures and early detection of the vertebrogenic algic syndrome in the cervical spine. Vertebrogenic algic syndrome is a pain in different parts of the spine, which is one of the most common reasons for doctor visits. An alarming fact is that up to 80% of adults face these difficulties up and age of patients is constantly decreasing. Most common cause is poor posture, cervical spine injuries, mental health, degenerative changes, tumors and more. Part of this work is devoted to the prevention of vertebral problems. In these modern times, when the use of computers, laptops, tablets, became the ordinary matter, it is necessary to raise awareness of the basic ergonomic principles when using these devices, modifying work and home environment. The theoretical part is supposed to inform about the causes, mechanisms and physiotherapy practice in the treatment of vertebrogenic algic syndrome. It includes basic knowledge of anatomy, biomechanics of the cervical spine and preventive measures concerning the ergonomics of the working environment, sleeping position and appropriate sports activities. For the practical part of the thesis has been used qualitative research method. Qualitative research was done in the form of case studies of three persons. Each was subjected to an input and output kinesiology examination and history-taking in the form of a structured interview. The probands were interviewed regarding home and work ergonomics. The therapy lasted five weeks at the Health centre Medipont in České Budějovice. The task of the practical part was to use physiotherapy techniques to alleviate pain in a particular segments of the cervical spine and report about basic preventive measures. The result is the production of an educational booklet that contains exercises to stretch and release the cervical spine, basic ergonomic positions accompanied by photographs. This brochure will be available at the front desk at the Health centre Medipont in České Budějovice, in waiting rooms of general practitioners in Jemnice as a practical and synoptical helper in the fight against the problems in the cervical spine. This bachelor thesis could increase an awareness among general public about preventive measures and how to prevent pain in the cervical spine. Furthermore, it can be used by clinical practice physiotherapists in the treatment of vertebrogenic algic syndrome.
The Utilisation of the Feldenkrais Method in patients with backache without radicular irritation

I chose the topic The Utilisation of the Feldenkrais Method in patients with backache without radicular irritation for my bachelor thesis. The method bears the name of its author Moshé Feldenkrais, who started to develop his conception after his own injury during the Second World War. Feldenkrais Method of somatic education is an approach employing the process of learning by means of movement. It is based on presumption that a human being has four basic constituents: feelings, thinking, sensual perception and movement, which are present to each human action, and which interact with one another, which results in the fact that influencing of one constituent causes changes in the others. The method has two basic approaches: group lessons Awareness Through Movement and Functional Integration. Backaches are now classified among so called ?civilisation diseases?, which is mostly given by today?s life style ? sedentary jobs, stress, lack of movement, which lead to numerous improper stereotypes leading later to pain occurrence. The spine and back in general, which are the most loaded parts in terms of posture maintenance against gravity are usually the first to be affected by chronic pain. Back pains are said to be the most frequent cause of inability to work. The thesis is structured in two parts. The theoretical part mainly deals with the Feldenkrais Method. At the beginning I introduce the author by his brief biography explaining the process of the conception development. Then I describe the principles and basic elements of the method like self-awareness, somatognosia, organic learning. Chapters containing indications and contraindications, possibilities and course of education in the field of Feldenkrais Method follow. The next part informs on two basic teaching methods, which are a group form ?Awareness Through Movement? and individual ?Functional Integration?, where I compare the first one with conventional exercises and the latter with classic manual techniques. In the last part dedicated to the Feldenkrais Method I am trying to outline the possibilities of applying Feldenkrais Method to chronic pain treatment. The following chapter of the theoretical part deals with the phenomenon of pain. In the last part I summarize the most frequent causes and consequences of backache and their schematic categorisation. The practical part contains the goals of the thesis, methodology description, research sample characteristic and the research results processed in the form of case studies. Quantitative research method was chosen for data collection and processing. Two patients with pains in cervical, respective lumbar spine were chosen. Anamnesis was taken from the patients and basic physiotherapeutic examination, which is recorded in the input and output kinesiological analyses, was performed. The research results aren´t so significant because of small testing group, but in my opinion the Feldenkrais Method might become a part of therapy for chronic pain treatment, as both the patients subjectively perceived improvement after the exercises, which is also obvious from objective examination recorded in the output kinesiological analysis. The fact that the research was based on just a small sample of probands however does not enable us to generalize the results. Remarkable improvement of perception and awareness of own body and work with it occurred in both the women, which was obvious on improved breath stereotype and mastering of localized breathing, which had been a big problem for both the patients before. Another improvement was in higher spine movement and particularly reduction of backache in both the patients. Improvement of psychical condition is surely another indicator of the method effectiveness.
Kinesiotherapy in patiens with osteoporosis

Abstract The theme of this bachelor's thesis is kinesiotherapy in patients with osteoporosis. Osteoporosis is the disease of metabolism which causes the density of bone mass, the bone becomes less firm and tough. This disease can be asymptomatic in the long term, but it causes many physical and mental problems later, which markedly get worse the quality of life. The first symptoms of osteoporosis are chronic backaches and fractures caused by minimal strain. These pains often raise feelings of depression, anxiety and fear. The preconditions of origin of osteoporosis are genetic but they are formed also from the early childhood, that's why it is very important to follow healthy lifestyle which includes sufficient calcium and vitamine D intake, activity, proper diet and avoiding toxic influences from the birth. Kinesiotherapy is one of the most important ways of preventive and medical care of patients who are endangered by osteoporosis or suffer by this disease. It is aimed at the improvement of motoric stereotypes, muscular strenght, mobility, coordination and activation of deep stabilizing system. It teaches proper breathing, lying, sitting, standing, lifting and carrying loads, forward bend and relaxation. World Health Organization considers this disease as one of the biggest health problems of mankind. Therefore I think it is important to pay attention to these issues. The first part of this bachelor's thesis is aimed at theoretic knowledge, divided into general and special part. The general part includes knowledge about structure and function of a bone, defines osteoporosis, deals with its classification, its clinical symptoms, its diagnosing and risk factors. Next there are described the possibilities of therapy, preventive measures and the impact of osteoporosis. In the special part there is introduced the definition of kinesiotherapy, its medical effect and general principles which is important to follow during kinesiotherapy procedure. It deals with possibilities of kinesiotherapy according to clinical phases and then describes particular kineziotherapy methods suitable for patients with osteoporosis. The aim of a theoretical part was to outline the possibilities of kinesiotherapy of patient with osteoporosis. The second part of this thesis is my research. For this part was chosen the method of qualitative research. It was done as case history including anamnestic data, initial and final kinesiological analysis, the progress of therapy, conclusion and long-term rehabilitation plan. The data were collected during the interviews with patients, by observing of them, sampling of anamnesis, kinesiological screening and the analysis of data from medical documentation. The research set was formed by two patients diagnosed with osteoporosis. The research was carried out in rehabilitation ward Poliklinika Jih in České Budějovice during four weeks. Exercising took place twice a week for 20-30 minutes. Soft tissue techniques, mobilisations, post isometric relaxation, stretching, fitness, breathing and relaxation exercising, elements training of the back, exercising with a big ball, proprioceptive neuromuscular facilitation, sensomotoric stimulation and the activation of deep stabilizing system. The aim of the research was to compile and realize the most effective kinesiotherapy procedures for chosen patients diagnosed with osteoporosis. The results show that the chosen kinesiotherapy methods influenced positively the condition of both patients. It is evident that their backache and posture are much better. They evaluate the therapy as positive and useful for their health. From the kinesiologic point of view it is evident the improvement in breathing stereotype, posture and reduction of some asymmetry and muscular unbalance which negatively influenced physical and mental aspects of patients.
