Utilisaation of Physiotherapeutical Procedures and Methods in Patients with the Multiple Sclerosis

The title of thesis: Utilisaation of physiotherapeutical procedures and methods in patients with the multiple sclerosis. Abstract of thesis: This work is divided into theoretical and practical part. The content of the firt part is a study of theoretical works related to multiple sclerosis. The main chapters are devoted to history of multiple sclerosis, the definition of why and how the disease arises, the types of disease, clinical presentation, diagnosis, therapy, desce incidence and complications that can occur to people with this disease. Finally, I looked at various physiotherapeutic procedurs and methods related to this disease. In the practical part of the thesis , a qualitatice research metod - cause study - was used. The research was conduted at the Center for demyelination disorders (MS center) at the Charles Square. The test set consisted of two probands with a diagnosis od cerebro-spinal multiple sclerosis, specifically of the relapsing - remitting form of disease. These diagnoses were determined by the attending physician of the probands. During the physiotherapy I used elements of the Bobath koncept. The aim of the thesis " A Physiotherapist's View of Overactive Bladder function in Multiple Sclerosis Patients" and the influence of physiotherapy on the symptoms of the overactive bladder. This thesis deals with exert influence on symptoms of an overactive bladder by physiotherapy. Therapy is based on the fact that guarding reflexes that maintains continence at the level of spine and subcortical (micturition centre in pons) are activated by the contraction of the pelvic floor muscles. Physiotherapy impact on the stability of standing and selected clinical parameters in patients with multiple sclerosis

Diploma thesis "Impact of physiotherapy on stability in stance and selected clinical parameters in Multiple Sclerosis patients" is aimed to study problems of disturbed postural stability in connection with this disorder. Research was made to evaluate the impact of two-month's physiotherapy, based on sensomotor learning, on the balance in stance, gait parameters and related functions. The sample of 6 patients with disability EDSS = 5, age 43 and more than five years course of MS on average, were undergoing a battery of clinical and paraclinical tests. Checking of clinical functions in patients with multiple cerebrospinal sclerosis - changes after the aerobic training

The aim of the this dissertation was to introduce a set of examinations of clinical functions in patients with multiple sclerosis (MS), which would be optimum for utilisation in physiotherapeutic practice, and also to compare changes to the individual examinations of this set after the patients underwent therapy consisting of aerobics training. We also monitored the persistence of the therapy's effect a month after its termination. The research was conducted on a set of 6 probands in the case of whom MS had been definitively confirmed, motor handicaps predominated, and the EDSS ranking was 3 to 5. Aerobic training took place twice a week for eight weeks. The intensity and length délka zátěže were stipulated individually according to the zátěžového vyšetření. After the therapy there were significant improvements (a p-value less than 0.05) only in the examination of cognitive functions, though we recorded a relatively significant improvement (a p-value of almost 0.05) in the case of investigations of balance, the functions of the upper limbs, and trembling in the upper limbs. On the other hand there was a relatively significant deterioration in the investigation of dysmetry in the lower limbs. Case study of physiotherapy treatment of a patient with the diagnosis of Multiple Sclerosis

The thema of this bachelor thesis is "Case study of physiotherapy treatment of a patient with the diagnosis of Multiple Sclerosis". The severity of multiple sclerosis incrementaly increases as the incidency of this disease - globaly and in the Czech Republic as well - rapidly grows year by year. Multiple sclerosis -as an autoaggressive chronic disease - is still incurable, however, plenty of changes in the diagnosis and the therapy have emerged during last years. This illness afflicts mostly persons during the productive period of their lifes. The patient is often without clinical symptoms during the first few years, which, until recently, misled to an idea that there is no urgent need to apply a long-term medical treatment. Today it is evident that only early executed diagnosis and early started therapy afterwards can prevent from the permanent neurologic impairment of the patient. Besides the pharmacological treatment, there is also physiotherapy which helps patients to live an active and valuable life. The point of this paper is to illustrate a complex view of this problem. The first, theoretical part deals with multiple sclerosis as a whole, with the goal to summarize the latest knowledge and findings about this disease. It describes the potential causes of the origin of the disease, its occurrence, particular symptoms, development of the disease, different types of the disease and the possible diagnosis together with the differential diagnosis. The ending of the theoretical part informs about the therapy comprising the pharmacological treatment and the complications it may bear, and the rehabilitation treatment summarizing therapy of all symptoms, which can appear. Nowadays, it is possible to find different approaches in the rehabilitation of multiple sclerosis. But according to many physiotherapy studies those methodologies, which are based on the neurophysiological fundamentals, were already very positively aplied on the patients. It is an objective of the practical part of my thesis to introduce one of these methodologies. The impact of an aplication of the individual physiotherapy, which utilizes the neurophysiological findings, can be evaluated by comparing the results from the entering and exit. This work is conceived as a case study in the form of casuistry. Qualitative research based on the data collection was chosen to verify the efficiency of the aplied physiotherapy. Data was obtained by interviews, observation, analyses of medical records and kinesiology testing. An entering and exit examination, comprised from the set of tests, were performed after the six months therapy to assess changes in the clinical conditions of the patient. These tests allows the therapist to comprehensively evaluate the effect of the rehabilitation on the patients with the central disabilities, particularly on the patients with multiple sclerosis. Tests were selected regarding the clinical signs of the disease. There were examinations done to test the balance, dysmetria, dysdiadochokinesia, scales of postural reactions, walking, muscle strength and spasticity. The test of grabbing ability was done to examinate the light motor skills and goniometry was aplied to determine the joint's range of mobility. The results of the research proven, that the individual physiotherapy employing findings in neurophysiology together with aerobic therapy and other techniques appropriately selected with the respect to the actual complications of the patient has a positive effect on the overall condition of the patient. This bachelor thesis can serve as a theoretical basis for the teaching needs or as an informational source for all, who are interested in the multiple sclerosis disease issue. It can also be an inspiration for physiotherapists on how to proceed in the rehabilitation of patients with multiple sclerosis.
Social services for people with multiple sclerosis in the South Region

The work deals with the supply of social services for people with multiple sclerosis in the South Region. The theoretical part contains a general introduction to the problems of multiple sclerosis, information about its causes, signs and types, describes multiple sclerosis in a social context, dealing with social security and social services for people with multiple sclerosis. The practical part contains the results of a survey conducted among persons diagnosed with multiple sclerosis and the results of the analysis of data from the register of social services. The aim was to determine the status of social services for people with multiple sclerosis in the South Region and the awareness of people with multiple sclerosis to offer social services for people with disabilities in the South Region.
The influence of physiotherapy on improving general state of patients suffering from Multiple sclerosis

The bachelor thesis summarizes present knowledge of the disease Multiple sclerosis and its ways of treatment. The aim of the thesis, is to build up a suitable complex physiotherapeutical treatment, which would have a positive effect on a general state of patients suffering from MS. The thesis consists of two parts. The first part is devoted to etiopathogenesis, symptoms of the illness, diagnostics, pharmacological therapy and the importace of rehabilitation treatment. The second experiential part contains a case study of two patients suffering from different forms of the illness. It describes their present health state, the course of the rehabilitation treatment and its evaluation. Furthermore performed examinations, medical history and a suggestion of a longterm rehabilitation treatment are also presented in this part. The research took place in the Multiple Sclerosis centre in Prague. The effect of the physical therapy was evaluated by measurement of physical condition and a kinesiological analysis. The thesis also involves a questionnaire in which the patients evaluate influence of physiotehrapy on their general state from their point of view. Rehabilitation has an effect on the state of patients particularly in correction of posture, breath stereotype and affecting balance. Physiotherapy helps the patients to influence some of the symptoms of the disease and therefore improve everyday-life activities. However the effect is only shortlasting. Consequently it is necessary to exercise on a long term basis and to adapt the therapy according to the progressive character of the disease.
Epidemiology of multiple sclerosis

There are approximately 2.5 million cases of multiple sclerosis [MS] in the world. Each year, around 10,000 new cases of MS are diagnosed. There are approximately 0.1%. in the Czech Republic and that means that there are approximately 15,000 people with this disease. Multiple sclerosis is a progressive neurological inflammatory disease of the central nervous system in pathogenesis. The auto-immune mechanisms are appliedinvolves; both the affect the myelin and damaged axons. This damage is responsible for the permanent disability of MS. The main objective of this thesis was to use quantitative research to describe the occurrence of MS in the Regions of South Bohemia and Usti, and within objective to estimate the true prevalence of multiple sclerosis in the South and the Usti Regions. MUDr. Príkaszký and I used the method of descriptive epidemiological studies on the technique of data collection, as well as the analysis and the comparison of two sources using the double-capture method. As a research area, we used a set of patients with multiple sclerosis. The data were gathered in two regions of the Czech Republic: Usti nad Labem Region and South Bohemia. We investigated the number of patients from neurologists and MS centers in both regions.There is also the data on health insurance included with the Ústí Region in this research. According to the data, we estimated the prevalence of neurologists in South Bohemia to be 79.20 per 100,000 people. The MS Center at the hospital in Ceske Budejovice has registered 512 patients: 388 women and 124 men. The values of the MS Centre in Ceske Budejovice received from the neurologists are a total of 882 patients, which is a prevalence of 138.6 patients per 100,000 of the population. Regarding the Usti Region, the prevalence reported by neurologists is 208.2 per 100 000 people. The MS Center at the hospital in Teplice, has registered 1139 sick 730 women and 409 men. According to health insurance data in their database, of 1187 people listed with a diagnosis of G.35, 333 are men and 854 are women. This represents an estimate of the prevalence 305/100000 of the people who are registered in the NGA. We calculated the prevalence of illnesses from those sources in both of these regions is higher than the general estimates. When comparing the data source of health insurance and the model file from the MS center of Ústí Region, we calculated the overall sensitivity of 91.7% within a health insurance group. The calculation of the estimated prevalence has been reached by using the method of double-capture of those sources that provided an estimate of 2,681 patients in the Usti Region. The models of this data are mainly demonstrated as a possible approach to the estimate of the prevalence of this chronic disease in the case of multiple sclerosis. The information has been obtained from the collection of data. The results of this study can be used by other workers who have the same objective as this study.
