Case study of physiotherapeutic care of patient after implantatio Uni THR of left knee joint

This thesis is divided in two parts - the general and the specialized one. In the general part it follows theoretical knowledge of problematics of unilateral endoprosthesis of knee joint implementation, different types of endoprosthesis, indications and gonarthrotis. Significant part is dedicated to physiotherapeutical procedures and means of treatment of patients after unilateral knee joint replacement. I dedicated the specialized part, elaborated in form of case interpretation, to a description of essential cooperation with the patient during the time of hospitalization, I determine short and long term plan and evaluate the effect of therapy.

Physiotherapy of a displaced disc with reposition of a temporomandibular joint

The topic of my bachelor thesis is physiotherapy of a displaced disc with reposition of a temporomandibular joint. It is a functional disorder, when the mandible disc is not in a physiological position and creates an obstacle for a condyle movement. When mouth is fully opened the disc does not return to its original position. This moment is characteristic with a clicking sound. Another symptom is inclining of the chin to the disordered site when the mouth is being opened. Main cause of TMD is stress, bad occlusion and a long-term micro trauma of TMJ. It is a topical issue, because an increase of stress in daily life causes also more TMD. Almost 70 % of the population has the TMD, in most of the cases the TMJ disc is in a wrong position. The therapy of this kind of disorder should be done by physiotherapist. Unfortunately knowledge of diagnosis and healing of the TMD is not sufficient. The disorder is recognized usually by dentists, who send a patient to a specialized clinic. The goal of my work was examining of mastication and neck muscle spasms. In the work there are also described physiotherapeutic possibilities of healing including postural strategy. I have created informational booklet for patients, in which saving method for TMJ healing, exercises, ergonomic principles and correct posture both when sitting and standing are presented. The research was done by 14 case analysis, analytic-inductive method and experimental work. Patients were randomly divided into three groups. As a first step TMJ and cervical spine of each patient and their posture were examined. The examination of TMJ consisted of diagnosis, extent and symmetry of an opening movement and palpation examination of the cervical spine. Therapy was six weeks long. Each patient was introduced to TMJ saving method, which is based on limitation of a TMJ movement, eating soft food and elimination of stress. In the first group (5 patients) the therapy was focused only on mastication muscles. Patients were supposed to be exercising to loosen the muscles of mastication. Exercises were chosen after palpation examination. Third group (4 patients) did neck muscle exercises and self massage. Second group (5 patients) did the therapy of both first and third group. Results of the therapy show, that there is a possible connection between TMD and cervical spine dysfunction. There were positive (evaluated subjectively) changes in both TMJ and in all groups. The condition of TMJ is influenced by neck muscle therapy and the condition of cervical spine is influenced by mastication muscle therapy. Best results had the second group. All patients but one improved (one lady had a TMJ inflammation). There were no deterioration of TMJ and cervical spine condition either. There were some worsening in group one and three. The best therapy is a combination of exercises of both mastication and neck muscles. I would like to point out a fact that TMJ is one of the most complicated and busy joints in human body. Both TMJ are connected to mandible so dysfunction of one joint can lead to problems with the other one. Physiotherapy is needed most in cases of functional disorder.

Physiotherapy after total hip joint replacement

Pain in a hip joint can dramatically affect the chance of having active life. The recent years have seen a significant progress in the results achieved while implanting artificial hip joints. This type of operation is simultaneously becoming increasingly more often especially when we take into consideration the increasing age of population. Implanting total hip joint replacement results in improving the quality of life, which can only be achieved through interdisciplinary collaboration of doctors, complex rehabilitation treatment, and positive approach of the subject alone. The aim of this bachelor?s thesis was to process case reports of patients after total hip joint replacements and to put together rehabilitation plans for these subjects. With the established objective in mind, the author summarizes outcomes dealing with the issues relating to total hip joint replacement, physiotherapy, occupational therapy, and spa treatment following such operation. The research part of the bachelor?s thesis was accomplished by qualitative research. Data were collected by observing, interviewing and analysing documents. The data were gathered by individual in-depth interview and using case reports, which included author?s own observations, a draft of rehabilitation plan and implementation of the therapy, and subsequent outcomes from the assessment of therapy. The research corpus comprised of two patients who underwent a planned total hip joint replacement because they had been suffering from advanced primary coxarthrosis. The research was carried out at an in-patient orthopaedic department of the Hospital Pelhřimov in March 2011, when the subjects were being hospitalised. Quality and appropriately managed rehabilitation allows quicker improvement in the patient?s health following the operation. It is used as a prevention of complications, it improves engagement of functional potential in the operated joint and it consolidates appropriate motion stereotypes. The outcomes of the research confirm that such pre-operational and post-operational rehabilitation carried out during the stay in hospital leads to a decrease or complete removal of pain, an increase in joint?s mobility and muscular power, getting used to walking on flat surfaces and up and down staircases, and the level of independency in everyday activities.

Physiotherapeutic possibilities of functional involment on shoulder joint in patients with rheumatoid arthritis

This thesis Physiotherapeutic possibilities of functional involvement on shoulder joint in patients with rheumatoid arthritis contains characteristic of rheumatoid arthritis, clinical picture, epidemiology and diferential diagnosis of shoulder joint. Theoretical part includes diagnosis, examination a physiotherapy of shoulder joint. Experimental part includes statistical treatment of physiotherapetic examination. The aim of this thesis was resume the knowledge about shoulder joint questions and in experimental part complete the statistical results related range of movement, muscle strenght and the others functional disorders. Postural locomotar development in children with developmental dysplasia of the hip

The aim was to highlight the context of developmental dysplasia therapy with postural hip locomotor development for children aged 4 weeks to 12 months. General examination was carried out in 9 children with disabilities in hip joints, degrees IIa- III. without contractures of adductors. We investigated spontaneous activity, and spontaneous reactivity. We compared the clinical picture of hip joints with the ultrasound image. Investigation of spontaneous activity was done by aspection. The test of spontaneous reactivity was taken using video and photos. Experimental results of our study showed the incidence of pathology in the spontaneous activity of 88.89 % children. Spontaneous reactivity was pathology in all cases, with various tests are discussed in the work. Ultrasound findings correlated with clinical manifestation in 66,67 %, but we cannot say that the proportion of patients will generally be higher than 60 %. Postpoliomyelitis syndrome - etiology and RHB treatment options

The bachelor thesis called "Postpoliomyelitis syndrome - etiology and RHB treatment options" deals with issues of this illness, officially recognized as a separate diagnosis until less than thirty years ago. The postpoliomyelitis syndrome can develop in individuals who have experienced poliomyelitis, an infectious viral disease affecting cells of the spinal anterior horns, in their childhood or youth. The main symptoms of this syndrome are: muscle weakness, increased fatigue, muscle and joint pain. They usually appear after a period lasting at least 15 years after primary infection, during which the health state of these individuals is relatively stable. The thesis presents a wide range of pathophysiological processes responsible for the etiology of these so-called late effects of polio. It also deals with the clinical picture, diagnosis and reflects the mental condition of the affected individuals. A large part of this thesis is addressed to therapy of postpolio syndrome, especially rehabilitation, which is an important part of comprehensive care of these patients. Title: Case study of physioterapy treatment of a patient with the fracture of the distal radius Objectives: The goal of this thesis is to summerize theoretical and practical informaticion about fracture of distal radius, to peruse sources of this problems, proposing therapy and monitor its progress. Includes detailed case studies patient with a fracture of the distal radius. Methodics: The literature search and the case study of this issue was proceeded within the practical experience (from 9th January 2012 to 3rd February 2012) in Oblastní nemocnice Kladno (Regional hospital Kladno). The first part of the work is concerned with the theoretical knowledge of anatomy and functional kinesiology of antebrachium and hand. Then describes the fractures of the distal antebrachium, their treatment and physiotherapeutic methods and examinations. The second part contains the case report which refers to the patient with the fracture of the distal radius, the treatment, the therapy and the pursuance of the curative effect. The use of manipulation treatment as a method of reflex therapy in therapeutical practice.

This bachelor thesis is focused on the use of manipulation treatment and veryfying its influence in therapeutical practice. This is a treatment in which momentum of peripheral joints and spine is restored into the correct position via a manual technique. This therapy is also used in disorders of musculoskeletal function, whether it is a blockade of peripheral jonits or blockade of the spine. In the beginning of the theoretical part the thesis focuses on history of this treatment, stresses the importance of expert knowledge of anatomy and physiology of the musculoskeletal system. It deals with the causes, treatment and prevention of functional disorders of the musculoskeletal system. The practical part of the thesis validates the use and results of this therapy in therapeutical practice using questionnaires and observation. It also includes a CD containing simple instructions for some automobilization excercises in which one can tackle with some movement disorders on his own.

Title: The relationship between the occurrence of a trigger point in the soleus muscle and the functional impairment of the SI joint and the possibility of their mutual influence. Objectives/Aims: The aim is to write a summarization of facts based on a literature research on the complex problem of the trigger point phenomenon with a regard to their etiology and work out a pilot study to verify the relationship between the TrP and stiffness in soleus muscle and the functional impairment of the SI joint in terms of reduced joint play in the ipsilateral lower extremity. The experiment, based on objective results, is to answer the fundamental questions of whether patients with functional impairment of the SI joint leads to the formation of a TrP in the soleus muscle on the ipsilateral limb, whether the eventual dry needle therapy of a TrP in the soleus muscle will remove the malfunction of the ipsilateral SI joint, and vice versa, whether if by restoring joint play in the SI joint by mobilization therapy will have an effect on the muscle tone of the ipsilateral soleus muscle and remove the TRP.
