Usage of NIC and NOC clasifications on patients with complete artificial hip joint.

Abstract The theoretical part of the thesis entitled The Use of NIC and NOC Classifications at Patients with the Total Hip Arthroplasty deals with the problem of the total hip replacement, educating patients before surgery and the regime which must be observed by the patient during the hospitalization and when being discharged from the hospital to home nursing. Further, the nurse has been described as a provider of rehabilitation nursing cooperating with the physiotherapist. The thesis is concerned with the nursing process, it preoccupies with the terminology in the health care, the issue of NANDA, NIC and NOC classifications and the Alliance 3N. Three goals of the thesis were defined: 1. to find out what types of NIC activities are typically used by nurses within the care of patients with total hip replacement. 2. to verify the application of NIC an NOC classifications in the care of patients with total hip replacement. 3. to investigate what sorts of the NOC indicators are typically used by the nurses in the care of patients with total hip replacement. The combination of the quantitative and qualitative research is applied. The four hypotheses were determined in the frame of the quantitative survey: 1. Nursing diagnoses are assessed as not being beneficial by the nurses. 2. Ensuring of tools facilitating the movement is a commonly preferred kind of an intervention by the nurses. 3. The knowledge of NIC and NOC terms is influenced by the previous education. 4. The satisfaction of the nurses with the nursing documentation is influenced by the length of practice. Data was collected by means of the questionnaires. One research query was established for the qualitative research: 1. What is the standpoint of the nurses on the NIC and NOC classifications in the care of the patient with total hip replacement? Data was collected by means of the formed nursing documentation based on the NIC and NOC classifications. The semi-structured interviews were used, as well. H1 remained unconfirmed. H2, H3 and H4 were not statistically validated. The NIC and NOC classifications are generally classified as not being beneficial for the nursing practice by the nurses.

Electron microscopy of nanoparticles for lead-free soldering prepared by wet chemical synthesis

The nanoparticles of pure metals and alloys exhibit the depression of the melting point compared to bulk material, hence they are able to aggregate and to from firm interlayer joints at low temperatures. Exploiting this effect in soldering industry can save energy, work and materials. Using emulsions with nanopowders might be the solution of demanding task of replacing classical Sn-Pb solders by their lead-free substitutes. In this work, Ag- and Sn-based nanopowders were prepared as potential low-toxic constituents of novel solders by a chemical wet synthesis from chemicals of high purity. Various ways of preparation and further storage of the product were examined. Resulting nanoparticles (their size distribution, morphology and tendency for clustering) were characterization by means of scanning and transmission electron microscopy.

The effect of kinesiotape application on rheological properties of the knee joint with chondromalacia patellae

Petra Mertová Title: The effect of kinesiotape application on rheological properties of the knee joint with chondromalacia patellae Problem definition: Chondromalacia patellae is a very frequent diagnosis related to knee injuries. Diagnosing chondromalacia is very difficult. Knee arthroscopy is the best method for evaluating the cartilage condition, however this technique is invasive. Kinesiotaping is a therapeutic method extensively used to affect knee pain. Patellar taping is an effective method in improving symptoms resulting from patellofemoral joint problems, nevertheless the evaluation of its effect has not yet been objectively investigated. Aim of the thesis: The aim of this study is firstly to discover the possibility to noninvasively detect chondromalacia patellae using the method of biorheometry and secondly to discover the impact of kinesiotaping on rheological properties of the knee joint with chondromalacia patellae. Method: The designed experiment was performed as a pilot case study using the method of biorheometry. Title Physiotherapy after arthroplasty procedures for rheumatic patients. Defining the problem The main problem this thesis solves is to find and compare findings on the practice of physiotherapy in rheumatic patients, patients with rheumatoid arthritis (RA) who have undergone surgery, joint replacement of lower limb arthroplasty, with a focus on the ankle joint. It will be a comparison of the approaches used in the workplace catchment for the Czech Republic, Institute of Rheumatology (RU) in Prague and the world. Aim The aim is to create a comprehensive overview and comparison of physiotherapy procedures used in rheumatic patients who underwent joint replacement legs, zjm. ankle joint, in the CR is in the hands, and in the world and see whether these practices vary significantly, or are identical. The method of solution The work is prepared by comparing the findings relating to the care of patients with available literature. Rheumatoid arthritis (RA) and osteoarthritis (OA) represent the most common forms of musculosceletal disorders that affect diarthrodial joints, lead to joint damage and disability. Extra-articular manifestations accompanied the joint disease only in RA. Diagnosis of both conditions most commonly bears on the conventional radiography. Mostly in OA, radiographic changes often occur late in the disease and are largely irreversible. Molecular markers could reflect joint damage, inflammation, or immune response. Current investigation revealed potential uses of molecular markers, ranging from understanding pathogenesis of the diseases to predicting and monitoring the outcome of the treatment. The aim of the thesis was to analyze several biochemical markers in serum, synovial fluid and synovial tissue samples from patients with RA and OA. and to evaluate their diagnostic and predictive values as well as their contribution to the pathogenesis of the diseases. We found increased serum pentosidine concentrations in OA patients that were ol a predictive value of the joint space narrowing in OA of the knee joint and correlation between pentosidine and cartilage oligomeric matrix protein (COMP) in synovial fluid that make pentosidine one of the new potential biomarkers of the OA. It adverts to connections following from kinesiology of joints of lower extremity, bipedal locomotion, total hip joint replacement and use of supporting - locomotive aids with loading of lower extremities. It describes the function and use of force platform - one of the possibilities, with which we can measure the loading of lower extremities. A part of this work is evaluation of the results of values of reaction force, vertically influencing the lower extremities, which demonstrate a real loading. The patients after TEP of the hip joint burden the operated lower extremity with this loading, when they walk the stairs.

The work is divided into a theoretical part and special part. The theoretical part deals with theoretical knowledge in terms of anatomy, kinesiology and injury mechanisms. It also includes the possibility of treatment in the matter, the possibility of investigative techniques and physiotherapy practices in the postoperative period. Related special section describes a case report of a patient with multiple disabilities posttraumatic soft tissue knee followed by rigidity, behavior therapy and evaluate the effect of therapy. Results: Status of patient is different at the beginning and end of therapy. Mainly to improve range of motion in L knee, increase muscle strength and improve walking stereotype. Case study: Physiotherapy treatment of a patient with the diagnosis of paresis n. peronei

Title: Case study of physiotherapy treatment of patient with the paresis n. peroneus . Goals: Gain of theoretical and practical knowledge about peripheral paresis, especially peripheral paresis n. peroneus . Subsequent case study formulation of patient with selected diagnosis made during coherent scholarly practice. Methods: The theoretical part of this bachelor thesis contains theoretical knowledge about anatomy of peripheral nervous system of the lower extremities, the clinical image of the peripheral paresis ane the treatment with sequential therapy. Results: Decrease of muscle shortening. Increase of muscle strenght in weakened muscels, improvement of movement range, elimination of reflective changes and restoration of joint play. Deformation and stress analysis of the parts of the total replacement of the trapeziometacarpal joint

The presented diploma thesis builds on author’s bachelor’s thesis of the similar name which was focused, in particular, on rhizarthrosis. Because of this joint disease, implantations of the total joint replacement are performed. The bachelor’s thesis also served as a familiarization with the finite element method and contact analyses issues, giving a theoretical framework for this diploma thesis. As opposed to that, the diploma thesis aims entirely at deformation and stress analysis of the total replacement. The main objective is to carry out analyses of different variants of geometric models. In addition, there is a tendency to analyze contact pressures of advanced model concerning all the total replacement, bone tissues, ligaments and tendons guaranteeing the static equilibrium of components. The bachelor’s thesis has been developed in ANSYS Workbench, whereas the diploma thesis in ANSYS APDL. Work in APDL allows user to control better over the entire computational process, however, requires more skills and theoretic knowledge. Contact analyses bring a lot of problems associated with convergence of procedures and accuracy of results. Therefore, explanations of important terms and set of recommendations are included for the purpose of easier solving of the contact problems.

Diffusion of carbon in welding joints of low alloys and high alloy steels.

Content of a document result measurements and analysis carbon´s diffusion at welding joint created from hight alloyed austenitic steel CSN 417242 and not alloyed steel CSN 412 050. Samples have been put throught isothermal anneal in a temperature range 500 °C up to 1100°C. Aim one´s effort to metalographyic quantification carburized and uncarburized zone. The next part of the document content measuring x-ray energy through microanalysis, where has been record course of a carbon disintegration and selected replacements elements at whole length of the welding joint . At the end of experimental part has been gauged course microhardness HV 0,05 at whole lenght of welding joint, from an acquired data has been chose the most optimal temperature of isothermal anneal, to produce the smallest structural notch.
