Title: Measurement of lower extremities spasticity in patients with cerebral palsy Objectives: The aim of the theoretical part of this thesis is to evaluate possibilities of lower extremities spasticity measurement in adult patients with cerebral palsy. The main focus was given to the concept of French professor J.-M. Gracies: Five- step clinical assessment in spastic paresis. This unique concept presents differentiation of three main factors of motor impairment that emerge as a result of a lesion to central motor pathways: stretch sensitive paresis, soft tissue contracture and muscle overactivity. Ability to distinguish these factors is crucial for specific treatment indication. Finding a correlation between the Five-step clinical assessment in spastic paresis and muscle activity in gait measured with polyEMG was the main objective in the practical part of the thesis. Methodology: This thesis has a theoretical-empirical character. The theoretical part is in a form of a research on the topic of spasticity diagnosis, focused on cerebral palsy patients. OF BACHELOR THESIS Title of bachelor thesis: Using progressive splints in cases of spastic paresis of upper extremity The bachelor thesis is focused on usage of progressive splints in cases of spastic paresis of upper extremity. It is quite a new method in curing of spastic paresis, which try to prevent shortening muscles and others structures. The text is divided into two parts: theoretical and practical. The theoretical part is to clarify the terminology regarding spastic paresis, its basic characteristics and types, clinical symptoms and brief survey of cure of the symptoms. The theoretical part follows with differences between splints and orthosis and more detailed description of progressive splints, which are used in cases of spastic paresis for prolonged static stretching. This part contains types of progressive splints, their usage, and how to work with them. In practical part the comparison of usage of this kind of splints and therapy of upper extremity. The aim of practical part was to summarize information about progressive splints and their using. For this purpose an instruction book was put together and it is as an appendix of the thesis. Orthotic options of paretic limbs in patients after a cerebrovascular stroke

Title: Orthotic options of paretic limbs in patients after a cerebrovascular stroke Objectives: The main goal of the thesis is to introduce a medical reader to the orthotic options of paretic limbs in patients after a cerebrovascular stroke in the context of a comprehensive multidisciplinary approach. Methods: In my thesis, I have used the methods of systematic research of accessible books, online sources, published studies, and proceedings of Czech or even foreign conferences. Further materials used in the thesis comprise presentations and notes from certified trainings for physiotherapists, professional internships in rehabilitation or orthotics departments, and from personal consultations with experts from fields of orthotics, neurophysiotherapy, robot-assisted rehabilitation and multidisciplinary team management. Results: The thesis offers a comprehensive overview of rehabilitation care in patients after stroke together with an overview of orthotic options of paretic limbs in patients with permanent effects, e. g. spastic paresis, post stroke shoulder pain or joint instability. Title: The effect of progressive static stretching on treatment of spastic paraparesis in patients with cerebral palsy Objectives: The goal of this dissertation is to evaluate the effect of progressive static stretching on treatment of spastic paraparesis in patients with paraparetic form of cerebral palsy (CP). Elements chosen for the treatment were those of the therapeutic concept Guided Self-rehabilitation Contract by French professor Jean-Michel Gracies, which is presented in detail in the theoretical part of the dissertation. Methodology: This dissertation has a theoretical-empirical character. First theoretical part of the dissertation is in a form of a research on the topic of spasticity and diagnosis and treatment of spastic paresis according to the concept of J.-M. Gracies. Use of the Modified Frenchay scale in stroke patients with spastic paresis

The objective of this thesis is to describe the Modified Frenchay scale and its use in stroke patients with spastic paresis. The thesis is divided into two parts - theoretical and practical. In the theoretical part, the issue of spastic paresis in stroke patients is specified; and in addition to this the methods of examination and assessment of spasticity are presented and described in detail. A large part of the thesis deals with the history of Frenchay arm test and its modification - Modified Frenchay scale. The theoretical part also contains a description of the phases and types of grasp, some of which can be observed during the assessment with Modified Frenchay scale. The practical part consists of a questionnaire mapping the situation in the institutions in the Czech republic, that use the Modified Frenchay scale in their practice, and of two case reports of patients after stroke. Examinations and evaluations of spasticity, including Modified Frenchay scale, which are described in the theoretical part, are applied in these case reports. The practical part also contains an evaluation of inter-rater reliability of the Modified Frenchay scale. The Use of Physiotherapy in Spasticity Management

Title: The Use of Physiotherapy in Spasticity Management Objectives: The purpose of this thesis is to provide a summary of present findings on spasticity, with particular regard to its pathophysiological mechanisms and clinical manifestations, and to present an overview of medical and physiotherapeutic approaches applied in its treatment. Furthermore, it aims at making a research in physiotherapeutic methods and techniques used for reduction of spasticity. The thesis shall present relevant studies, provide their comparison and critically evaluate the effect of methods and techniques examined in them. Methods: This thesis is a descriptive analysis based on a literature review. Results: Based on the research findings, it may be concluded that physiotherapy has proven to be effective in reducing spasticity. However, it is not possible to determine whether the techniques to reduce spasticity are more effective than techniques without this primary purpose. The examined studies have shown that the choice of physiotherapeutic practices to reduce spasticity was not influenced by disease etiology. Moreover, the same techniques and methods were applied in pediatric and adult patients, regardless of diagnosis.
