Emergency readiness of the South-Bohemian Region inpatient healthcare facilities and their medical staff

In the theoretical section of my master thesis I focused especially on the role of the health services within the crisis management. In particular chapters I tried to write down a simple and comprehensive summary of the role of the healthcare within the integrated rescue system, I dealt with the function of the Ministry of Health, the hierarchy of the healthcare in solving the emergencies, the valid law and the crisis preparedness in the healthcare. In one part of my master thesis I also mentioned the importance of testing the professional and practical readiness of particular units of the integrated rescue system by emergency exercises and the characterization of their individual types. Of course I could not omit the crisis planning and a brief description of some chosen types of plans that are used during emergencies.The goal of my thesis was to map the preparedness of the South-Bohemian Region inpatient healthcare facilities for possible emergencies in the respective facility or out of it and the medical staff´s awareness of possible risks and of possible solving the emergencies in the respective facility.In the practical section of my master thesis I used the methods of quantitative research. The technique of this research is based on the questionnaire survey with bounded, half-bounded and open questions. The questionnaire designated for medical staff consists of 23 questions. The first three questions are focused on the characterization of the respondents; the remaining twenty questions are to find out the stock of knowledge related to the crisis preparedness. The respondent gets one point for each correct answer to a knowledge question. This part of the questionnaire is composed as a multiple-choice test and one can get 43 points at most. The questions were created using the crisis plans that are valid in the České Budějovice Hospital. After our mutual agreement I took over the questionnaire designated for healthcare facilities from Mgr. Jolana Němečková, who analysed the crisis preparedness of the South-Moravian Region healthcare facilities in her master thesis. I did not change the method of questionnaire analysis either to enable a subsequent benchmarking of the results in the South-Moravian Region and the South-Bohemian Region. The questionnaire consists of 28 questions. One can get one point for a dichotomous question and two points for a closed-ended question. The highest possible score was 22 points. The results of the questionnaire survey for medical staff show that the preparedness of the České Budějovice Hospital´s medical staff is at a very good level. The average score was 37 out of 43 points and that complies with the classification 2. When analysing the relation of the preparedness with the highest achieved education level and the preparedness with the length of the practice, the positive correlation was confirmed. That means that the higher achieved education and the longer practice, the more was the knowledge of the respective area. The results of the questionnaire survey for the inpatient healthcare facilities management are not so positive. The average score was 12.3 and that complies almost with the classification 4, which means a bad preparedness. In comparison with the South-Moravian Region healthcare facilities, the preparedness in the South-Bohemian Region is one level worse. For the purposes of my master thesis I set four hypotheses that were proved or falsified during a follow-up statistical assessment. Hypotheses No. 1 and No. 2 cannot be accepted. Hypotheses No. 3 and No. 4 were accepted.
The knowledge of the Emergency medical service paramedics of Ustecky region in the field of crisis preparedness

In the diploma work, we familiarised the reader with the basic theoretical starting points concerning emergency medical services and preparedness for emergencies. First of all, in this part, we elaborated a basic overview of the organisational structure of the emergency medical services of the Ústecký kraj region, and of its subdivisions. Next, we outlined the educational system of the paramedical staff of the emergency medical services of the Ústecký kraj region, focused on the area of crisis management. The crisis management competence of paramedical staff involved in emergency medical services was the subject of a separate sub-chapter. We linked these chapters with the issue of the formation of the department of disaster medicine. Here, we defined the tasks and aims of disaster medicine. To create a better understanding of the issue, a part of the chapter consists of the basic terminology used in disaster medicine. The main aim of the diploma work is the analysis of the knowledge of the paramedical staff of the emergency medical services of the Ústecký kraj region in terms of preparedness for emergencies. The sub-objectives of the thesis are the analysis of the knowledge of the medical rescue workers of the emergency medical services of the Ústecký kraj region in terms of preparedness for emergencies, and the analysis of general nurses' knowledge of, and qualifications in, the specialist anaesthesiological-resuscitative care of the emergency medical services of the Ústecký kraj region in terms of preparedness for emergencies. The methodology of the work lay in the quantitative analysis of statistical data that we gathered from questionnaires that were filled in by randomly selected respondents among the paramedical staff of the emergency medical services of the Ústecký kraj region. We prepared the content of the questionnaire in the context of the issues studied. The respondents chose from a number of pre-defined answers to a total of 20 questions. The data obtained underwent statistical analysis with the use of statistical research methods, scaling, measurement in descriptive statistics, elementary statistical processing, non-parametric testing, estimation theory, and parametric testing. From the results of the practical part of the diploma work, it is evident that the knowledge of the medical rescue workers of the emergency medical services in terms of the preparedness for emergencies of the Ústecký kraj region is close to the normal frequency distribution N (mi and sigma). Also the general nurses' knowledge of, and qualification in, the specialist anaesthesiological-resuscitative care of the emergency medical services of the Ústecký kraj region, in terms of preparedness for emergencies, is close to the normal frequency distribution N (mi and sigma).
Analysis of emergency preparedness organizations in the field of education

The content of this thesis is an analysis of crisis preparedness organizations in the field of education. It consists of a theoretical - methodological part and a practical part. The theoretical part discusses the specifics and peculiarities of crisis management, and analyzes the objectives and nature of crisis preparedness organizations of this type. The practical part presents the current research shows the strengths and weaknesses, as well as the threats and opportunities, which significantly limits the status and level of crisis preparedness of schools surveyed. The penultimate section focuses on the design methodology to ensure crisis preparedness in schools. Conclusion then summarizes and evaluates the investigation. For the sake of better illustration is the work of the Annex attached to the issue more closely.
Emergency preparedness of inpatient medical facilities of a non-university type in the South Moravian Region

Society today is increasingly exposed to the effects of emergencies. These situations not only affect the normal functioning of the society but also the functioning of medical facilities. The preparedness of medical facilities is a very important factor influencing the success in dealing with emergencies and crisis situations. The preparedness of the health care system as a whole consists of all medical facilities in the Czech Republic. Therefore, it is desirable that they are prepared to address them. The objective of the thesis, in its theoretical part, was to provide a comprehensive overview of the current state of crisis preparedness in medical facilities. The practical part was intended to introduce in more detail the territory of the South Moravian Region and its issues and to map out emergency preparedness in the specified research set. The research set included medical facilities in the region that are not university hospitals but that are part of the region traumatology plan. The data on this issue was collected using a questionnaire. The questionnaire was in a format of selected work indicators of emergency preparedness and it was uniform for all the respondents. The work indicators were as follows: theoretical preparedness, organizational preparedness and practical preparedness. Based on the questionnaire survey, analysis of the results and determining the average level of preparedness of all the participating medical facilities, it was concluded that the preparedness of the medical facilities of a non-university type in the South Moravian Region is very diverse: from hospitals whose preparedness is excellent to those who do not reach the required basis. In the overall assessment, 40.0 % of the respondents did not reach the determined average. Overall, the level of preparedness of the research set can be seen as slightly below average. To increase the level of preparedness of non-university medical facilities, it is suggested, for example, to create unified procedures for processing various types of plans, to assign each medical facility the obligation to employ a professional emergency manager, and to establish cooperation among individual medical facilities.
Evaluation and confrontation performance of the tasks of municipal corporation on the section fire protection and protection population in the Czech Republic.

In my diploma thesis, I focused on the issues related to preparedness in the field of fire protection and the protection of the population in newly established municipalities with extended powers. Within the survey, I sent out 205 questionnaires to the municipalities with extended powers throughout the Czech Republic. In correspondence with the representatives of municipalities with extended powers, I noticed in their responses more than apparent differences, especially in open questions.

The thesis deals with analysis of current security system in the Czech Republic and, in particular, with the legal framework for the training of state authorities and local governments to manage non-military events. The study and analysis of legislation and the subsequent synthesis of the findings enabled to arrange the chapters into a clearly structured whole. The rights and obligations of the emergency management authorities at the emergency response training and management, the system of security councils and crisis headquarters, activities and the scope of competencies of individual entities providing the emergency response training and management, focusing on the scope and powers of the regional and local administrative bodies, are described. The level of financial security spent by individual institutions on preparation for emergencies management is clearly surveyed. For municipal authorities, a model of a call for the provision of material support and resources at the time of an emergency situation has been suggested, since the large majority of mayors perform this activity voluntarily and without a refund of their salaries (besides their jobs) and they are not fully familiar with the issue. The current security system in the Czech Republic is functioning, and its settings and functionality have been verified in recent years in various emergency situations, without finding significant deficiencies. Yet it is still necessary to optimize the scope, methodology and actions of its components at different levels and in different areas and to consolidate the relevant legislation
Role of municipalities and cities in crisis management

The thesis rounds up fundamental information about municipalities and cities concerning crisis management. Further analyzed are responsibilities and duties of municipal and city authorities in resolving major emergency incidents and crisis situations, and status of municipalities in the security system of Czech republic. The thesis further describes the ensuring of crisis preparedness at the level of municipalities and cities. The aim of the research part of the thesis was to analyze, describe and evaluate present state of crisis preparedness of municipalities and cities in South Bohemian region and, at the same time, to find suggestions for systemic solution to the current situation. And this for a reason that the status of municipalities in the security system of Czech republic is irreplaceable because a municipality, most importantly represented by a mayor, is closest to a citizen, and therefore a help in resolving major emergency incidents and crisis situations must primarily be ensured from the level of a municipality. For this purpose the hypothesis was set that municipalities, as opposed to municipalities with extended powers, are professionally, personally and materially not sufficiently prepared for resolving emergency incidents and crisis situations. Furthermore, the thesis deals with methodical tool ? procedure for solving emergency incidents and crisis situations by municipality bodies that could directly serve mayors of municipalities. And also, a proposal for tasks and measures is dealt with which could improve crisis preparedness of municipalities and cities.
The view on the Krnov Hospital X Ray Department's Preparedness to Handle an Increased Number of Casualties at Emergency Management

Exercising their profession, nursing staff in health facilities may commit a fatal mistake in tense moments in terms of time. An illustrious example may be the admission of a higher number of casualties after an emergency event. So that to prevent these negative consequences, emphasis is put on the readiness of the entire health system from the headquarters to individual care providers. Special attention is also paid to trauma planning, because especially in complex cases greater demands are developed on the functioning and rapid succession of individual operations of medical care, along with observing all approved medical principles, of course. To verify the readiness and to reveal any shortcomings and reserves tactical exercises are held. The Krnov hospital also participated in that kind of training with activating the trauma plan. Another workplace involved was also the radio-diagnostic department. A simulated bus accident was to reveal whether the individual workplaces could provide adequate examinations and treatment. The thesis is divided into several parts. In the introductory section it is explained in brief how the health system is organized. To understand the text, it was necessary to define the necessary terminology and to mention the binding, legal legislation for emergency management authorities at all levels that affect the operations and carry out supervision on medical institutions. Gradually, the theoretical part presents trauma plans, sorting out casualties in the place of an emergency event, and specific exercises in the Krnov hospital that were announced and carried out in the early afternoon, when the hospital was fully filled with staff members. In the research part the analysis of a real accident that occurred in a late evening about a year later is made. Time demandingness of individual examinations is evaluated. Factual results are compared with the data obtained from simulated exercises. In the final part I tried to assess the quality and topicality of the trauma plan elaborated by our facility as well as the internal trauma plan of the RDG department and to propose possible changes and improvements.

Population evacuation plan is one of the plans of specific operations, which are covered within the region's emergency plan according to Decree No. 328/2001 Coll.Evacuation belongs among the basic methods of population protection, being a summary of principles and organisation measures to ensure moving the persons, animals and material assets from the area endangered by an extraordinary event to other area. In the case of an extraordinary event endangering larger territorial unit, not only the populated agglomerations and industrial facilities may be included in the area evacuation, but also the inpatient healthcare facilities, where the situation difficulty is amplified by the patients depending on care by medical personnel and provision of healthcare services. Presently, there is no clear legal standard imposing an obligation on organisations to elaborate a complete evacuation plan. This issue is covered only partially in the sphere of legislation concerning fire protection. Nevertheless within the region's emergency plan elaborated for solution of extraordinary events requiring declaration of the third level (in the case of endangering more than 100 and less than 1,000 persons) and special level of alarm (when more than 1,000, a part of a municipality or enterprise grounds are endangered), all organisations with more than 100 persons should have the evacuation plans prepared. In the case of inpatient healthcare facilities, elaboration of evacuation plans is absolutely inevitable. The theoretical part of the dissertation describes the general principles and methods of performing the population evacuation. Attention was paid also to professional coverage, evacuation authorities and organisations and their structure according to the scope of the taken measures. In the chapter called "Application of the evacuation plan to inpatient healthcare facilities", the general principles and procedures concerning evacuation were applied to the inpatient healthcare facilities. In the practical part of the dissertation, an analysis of preparedness of the inpatient healthcare facilities for possible area evacuation to all-nation extent was made and the hitherto experience of the personnel of the inpatient healthcare facilities with any type of evacuation and their preparedness for possible complete evacuation were examined. Such examinations were made by questionnaire surveys. The collected data have been processed by statistical methods. The evaluation of results is presented in the "Results" chapter. On the basis of the acquired knowledge, a proposal of evacuation plan was drafted for the inpatient healthcare facilities. At the end of the dissertation, the procedures are suggested that may help manage perfectly the potential complete evacuation. Expected utilisation of the dissertation results was directed to definition of the optimum procedures and measures leading to successful management of evacuation of an inpatient health care facility. The dissertation conclusions may be used not only as the document for creating evacuation plans for other health care facilities, but they may contribute to considerable extent to elaboration of uniform evacuation preparation methodology within the emergency preparedness of the healthcare services.
Creation of pandemic plan for medical institutions

The aim of this B.A. level is to analyse the risk of creation of a pandemic caused by a new influenza virus. This academic paper particularly discusses the fact whether hospitals are prepared for emergency situations when the number of hospitalised persons is rising enormously. This thesis introduces and analyses the Czech National Pandemic Preparedness Plan, as well as the plan relating to the South Bohemia Region with respect to the fact that every state is obliged to have its pandemic preparedness plans. Furthermore, the thesis attempts to ascertain whether these plans are in compliance with proposals and measures introduced by the World Health Organisation (WHO) and the European Union. WHO has been monitoring the influenza virus since the end of World War II. For this monitoring, WHO has been using the influenza preparedness programmes. Due to these programmes we may be able to foresee the creation of pandemic to certain extent. However, despite the facts mentioned above, the risks still remain on a rather high level since for example in South-East Asia, contamination of humans via the bird flu is quite common. Some sources believe that if a new influenza pandemic occurs in the world, it will break out exactly in South-East Asia, and therefore it is necessary to monitor this location carefully. Furthermore, this thesis focuses on describing the influenza virus, what structure it has, and, in particular, what difficulties it causes to humans. In the Czech Republic, seasonal influenza is quite common in relation to which basic differences in the symptoms are also discussed in this paper. The thesis subsequently focuses on the description of the treatment for both seasonal influenza, as well as bird flu since they differ significantly. Nowadays, also the issue of influenza vaccination is being discussed quite often. It is necessary to state that vaccination is not suitable for all citizens and that it may not protect us from a pandemic caused by new influenza virus. In this thesis, the expression ``influenza pandemic`` has been explained and plans of substantial importance playing the role of measures preventing the creation of pandemic have been described. Furthermore, an analysis of the Czech National Pandemic Preparedness Plan, with special focus on the South Bohemia Region, has been carried out. It is a well-known fact that the first occurrence and the most afflicted area of the bird flu virus in 2006 (the infected swan) was in the České Budějovice area.
