The issue of smoking in the medical profession

The aim of this work was to map the extent of knowledge about active and passive smoking and its effects on human organism between paramedic employees of Emergency Medical Service in the Ústí nad Labem Region, coupled together with an evaluation of their interest in further education in this area. An absolute majority of questioned paramedics says that active smoking is very detrimental to overall health, but the question of passive smoking is not considered such a big issue among them. Lectures about smoking and its effects, realized at their workplaces, are not expected nor demanded. Theoretical part of this thesis gives an account of tobacco history, describes constituents of tobacco smoke and its effects on human organism. Furthermore this work deals with possible methods for quitting the smoking, possible prevention approaches and valid legislation in Czech Republic, dealing with the regulation of smoking. The final theoretical part of this work tries to define characteristics and possible dangers of employment at Emergency Medical Service. Drug use among emergency operations centre personnel of emergency medical services

Positions in emergency medicine are endangered by increased chance of long-term stress and burnout syndrome. These risks, which are inseparable from their professions, can result in increased consumption of addictive substances. Employees of emergency operations centres are one of these risk groups, hypothesised to have increased drug consumption in comparison to general public. The goal of this thesis was investigation of drug consumption among emergency operations centres personnel and determination of addictive substances and the amounts being used. The second goal was to confirm hypothesis about connection between burn out syndrome and drug use. Data were gained through questionnaire survey which was implemented online in 11 Czech emergency medical services in the first half of the year 2019. 25 % (88 respondents) of asked employees took part in this research. Results show, that emergency operations centres personnel most frequently use tobacco and alcohol, the amount is bigger than the amount used by general public. The results show riskier way of alcohol use and higher lifetime prevalence of cannabis usage among emergency operations centre personnel than in general public. A comprehensive approach of emergency medical services providers to a transport of a patient suffering from a highly contagious disease

In the thesis "Comprehensive approach of emergency medical services provider to a highly-contagious patient transportation" we concentrated on nursing care and aspects of transportation administered by operating paramedics, all in cases that public health authorities suspect to be related to a highly - contagious disease and transportation in isolated transportation vehicle is mandated. The theoretical part of the thesis defines the key concepts of highly - contagious disease, it describes individual causal agents of contagious diseases and their ways of transmission to humans. For description of contagious diseases were chosen those with high morbidity, lethality and interpersonal transmission. We mention the danger of biological agents misuse, ways of protection against contagious diseases and types of decontamination procedures. We describe different means of personal protection used by biohazard team members and even environmental factors affecting the work of paramedics using personal protective agents. Further on, we bring to attention legal aspects and inner policies related to aforementioned issues including analysis of procedure logistics of emergency services teams dealing with emergencies with suspicion of highly - contagious disease. Research in practical part of the thesis concentrates on aims related to optimizing nursing care for patients suspected of suffering from a highly - contagious disease, when securing them in the transportation vehicle, furthermore, we examine the impact of personal protective agents used by paramedics when transporting the patient, and we also analyze environmental factors inside the ambulance vehicle and isolated personal transportation vehicle. All of these aims were researched with the help of planned experiments and the resulting findings are based on the outcomes of these experiments.
The problematics of securing airways of a patient with an acute respiratory disease

The topic of our Bachelor's thesis is "Problematics of securing airways of a patient with an acute respiratory disease". The aim of this thesis is to map the issue. The thesis contains the theoretical part and the research part. The theoretical part focuses on anatomy and physiology of the respiratory tract, monitoring of respiratory system, monitoring of consciousness, securing of the respiratory system and caring for the patient connected to the artificial pulmonary ventilation. Furthermore, this work also deals with nosocomial and professional infection and especially with acute respiratory disease. The research was conducted through qualitative research using a semi-structured interview with 10 qualified specialists and paramedics working at the department of Anaesthesiology and Resuscitation and at Emergency Medical Services. To designe researche were chosen for these research questions: 1. How do the paramedics protect themselves when securing the airways of a patient with acute respiratory disease? 2. How often do paramedics encounter the activity of securing airways of a patient with acute respiratory disease? 3. What is the attitude of paramedics in practice when it comes to securing the airways of a patient with acute respiratory disease? The purpose of this Bachelor's thesis is to find out how the paramedics proceed in securing the airways and how they react when they encounter the acute respiratory disease. This Bachelor's thesis serves as a material that points to the correct use of barrier protection. The result of the research shows that the paramedics meet with the respiratory disease only rarely. The questions were also aimed at the compliance with barrier protection. The main benefit of this Bachelor´s thesis should be the awareness of non-compliance with barrier protection and subsequent use of protection against respirátory disease.
Anaphylactick shock and the possibilities of dealing with it by a paramedic.

The aim of the thesis was to map knowledge that paramedics of South Bohemian Emergency Medical Service have during a medical treatment of Anaphylactic shock in out-of-hospital emergency care. This topic included the knowledge of terms Anaphylactic shock, describing its symptoms, the way of its medical treatment, recommended procedures and its style of training. Six employees of South Bohemian Emergency Medical Service take part in this research. Respondents received 15 questions related to this issue. The qualitative research was used to achieve results. All the interviews were semi-structured. As emerged from the research, the training concerning Anaphylactic shock is insufficient. Most of paramedical staff have not been present during any Anaphylactic shock. They have been more present during some Allergic reaction. Therefore, their motivation to gain more knowledge of this issue is not satisfactory. Also, paramedics do not have enough knowledge of a term Anaphylactic shock. They are aware of this term, however, they are not able to describe it. The biggest problem is dosage medications. The paramedics are not sure how to dose medications without consultation with a doctor. Concerning medication of adults, the knowledge is better. However, in the case of children, the paramedics confess that they do not remember the procedure. Paramedics follow the rule which takes into consideration kilograms of a patient. However, for the reason of the faster intervention, it is better to give the amount of medication with regard to the age of a child. Another problematic question is a position of a patient when transporting to a hospital. To have better orientation and knowledge of procedures during Anaphylactic shock, paramedics should have more extensive training. Except for the mandatory training from an employer, the paramedics should have an occasional initiative of gaining the knowledge by their own.
Attitude and motivation of paramedics and medical drivers of vehicles in emergency medical service to continuing professional education

The aim of the study is to determine present attitude and motivation of paramedics and ambulance drivers to the continuing education. The purpose of the research is to clarify how could be possible to recieve better motivation in this field. In the beginning of thesis are described terms as motivation, attitude, education, paramedics or ambulance drivers. Present attitude of persons in emergency medical services is measured in empirical part of study. There are discovered the reasons why employees are not motivated to the next education and is shown how to improve this statement. As research tool was used semi-structured interview. Than results were analyzed and described. Participants of research were employess of one ambulance in Vysočina. Results can help to see level of motivation these participants to the continuing education as well as employers can see that is necessary to improve situation on ambulance area. Diversity of participants´ motivation is confirmed. The most important positive motive is money, as negative (for example) many lessons without practical parts.
Management of patient with cranial trauma in prehospital care.

The bachelor thesis is entitled: "Management of patient with cranial trauma in prehospital care". The teorethical part thoroughly deals with the characteristic of craniocerebral trauma and its management in prehospital emergency care. There is an overview of anatomy and physiology of head with an emphasis on perfusion and brain vascular supply. Further ways of origin, types, symptoms and assesment of the severity of craniocerebral injuries. Within the management of prehospital care, the teorethical part deals with the individual steps in examination and stabilising the patient and thr pointing to the target medical institution for final treatment. In the practical part there were set two objetives. The first objective was to map the issue of cranial trauma and the management of the patient with cranial trauma in prehospital care, based on expert literature. The second objective was to evaluate the practical procedure for six cases and how much it actually matches with the recomended procedure. A qualitative research approach was used to process the research part. The data was collected in form od cases reports, which were subsequently categorized into nine categories. According to the results, there was found that the paramedics are partly familiar and control the procedures for patient examination, its monitoring and the criteria for its targeting to the definitive health facility. The management problems may result into unequal use of fixation tools and inconsistency in the use of pharmacotherapy and volume therapy. The output of this bachelor thesis could serve as a basis for teaching paramedics dealing with craniocerebral injury and management in prehospital care. Furthermore, the work could be used to increase or supplement the awarness of paramedics when they meet a patient with trauma brain injury.

Readiness of the emergency medical service for treating patients contaminated by radiological agens

The aim of my thesis was to find out the readiness of the South Bohemian Emergency Medical Service to deal with a potential extraordinary event of radiological nature. The content of the bachelor thesis is to analyze professional, material and technical readiness. The theoretical part deals with characteristics of integrated rescue system, exceptional events, ionising radiation and, last but not least, pre-hospital urgent care for irradiated persons. The practical part of the bachelor's thesis investigated the level of rediness of the Emergency Medical Service of the South Bohemian Region both in professional and material and technical level. There were two hypotheses in the thesis, H1: "Medical rescuers have knowledge how to treat patients contaminated with radiological agens," and H2: "The emergency medical service has material equipment for the treatment of patients contaminated with radiological agents." In order to achieve defined goals and to verify hypotheses, a questionnaire was put together and a survey was carried out. A statistical investigation was made using descriptive and mathematical statistics. The questionnaire consisted of closed questions. It contained 11 informative questions, 9 questions focused on problematics of treating patients contaminated with radiological agens, and 4 questions focused on the material and technical equipment of the medical rescue service. Respondents' questions were addressed with 3-5 possible answers, only one of which was correct. Data was collected during March 2017. Respondents were members of medical staff of the South Bohemian Emergency Medical Service. Sixty anonymous questionnaires were distributed in the South Bohemian Region, more precisely in the territorial centre České Budějovice and the regional resorts Jindřichův Hradec, Český Krumlov and Tábor. Total of 42 respondents participated in the survey. The results of the survey show that the knowledge of paramedic in this area is below average. The arithmetic mean of knowledge was 5.79 points out of 9, for material and technical equipment it was 1.79 points out of 2. The results of the survey show that the goals of the thesis were fulfilled, one (it intended hypotheses H1: Medical rescuers have knowledge how to treat patients contaminated with radiological agens) hypotheses was rejected and one (it inteded hypotheses H2: The emergency medical service has material equipment for the treatment of patients contaminated with radiological agents) accepted. The benefit of the bachelor thesis is mainly the preparation of a basic comprehensive overview of the readiness and materiál and technical support of the South Bohemian Emergency Medical Service for the treatment of patients contaminated with radiological agents.
Prague Emergency Medical Services Preparedness for Medical Intervention During Chemical Threat in Metro

The goal of this theses is to analyse the preparedness of Prague Emergency Medical Services (hereinafter Prague EMS) for chemical attack in subway (metro). The theoretical part states general information about chemical terrorism and the Integrated Rescue System's solution. The research part focuses on procedures following from Integrated Rescue System Common Type Activity during joint action - Reaction on chemical attack in subway (hereinafter STČ 13/IZS). This document was subjected to content analysis to meet the goals. The work then focuses on the evaluation of Prague EMS training Metro 2014 whose goal was to practice the procedures and joint deployment of Integrated Rescues System members and other subject according to STČ 13/IZS. For the evaluation complexity, the results were supplemented by SWOT analysis which pointed at another usable knowledge. All conclusions are clearly organized in charts and they are efficiently worked with to set new measures and recommendations which would increase the preparedness of Prague EMS for chemical attack in metro. The goal of this work to assess the level of preparedness and to submit final protocol to organisation's crisis management was fulfilled. The main benefit of this work is a recommendation for the reinforcement of crisis preparedness at Prague EMS. Furthermore the work brings new expert findings to solved problematics.
