Case Study of Physiotherapy of Patient After Rupture of the Achilles Tendon

Title: Case Study of Physiotherapy of patient after rupture of the Achilles tendon. Objectives: The aim of this bachelor's thesis is description of issues after rupture of the Achilles tendon. This is summary of theoretical knowledges, operational treatment, physiotherapy care and treatment results of the patient with the diagnosis of rupture of the Achilles tendon. Methods: The theoretical part is the summary of anatomy, biomechanics, kinesiology of the the ankle joint. Included etiology, clinical symptoms and diagnosis of the rupture of Achilles tendon as well. In the special part is specifically described the physiotherapeutic care of the pacient. The treatment of the pacient was recorded at Hospital Na Bulovce in Prague in rehabilitation department in period from 13th January to 7th February 2014 under the conduction of leadership of the department. The main aim of the special part is the input and output kinesiological analysis, therapeutic units, the effectiveness of therapy and the evaluation of physiotherapy techniques. Results: There was a big progress in range of motion, especially in the ankle joint. In most cases have been removed joint blockades and released soft tissues. The thesis includes anatomical, biomechanical and kinesiological knowledge as a foundation for treatment. I described possible operative and conservative procedures. In the special part there is formulated a casae report of one patient in detail. Ten therapeutic units are described including kinesiological analysis and final evaluation. Tendinopathy of Achilles tendon is a degenerative disease which affects mid-portion or insertional part of the tendon. The bachelor's thesis deals with the overal issue of this disease, theories of etiology, clinical manifestation, kinds of examination, diferencial diagnosis and types of therapy and focuses on conservative therapy. The thesis is supplemented with a case report of patient, who followed rules of recommended therapy on the basis on used studies, with final evaluation of patient's state of health. Title: Case study of the physiotherapy treatment of patient with Achilles tendon rupture diagnosis Aims: The aim of this work is explanation and summary of theoretical and practical knowledge about the Achilles tendon rupture and a case study of patient with such diagnosis. Summary: The thesis consists of general and special part. The general part is a research of literature about the topic. At the beginning I focus on anatomical and kinesiological knowledge about the affected area, then I follow up etiology, pathogenesis, symptomatology and treatment of the injury. Special part is a case study of patient with the Achilles tendon rupture. Viscous and elastic properties of soft tissue "in situ"

Title: Viscousand elastic properties of soft tissue"in situ" Goals and methods: The aim of this thesis is to measure the viscoelastic properties of human soleus muscle and Achilles tendonin vivo and post mortem in situ. It is a pilot study that uses myotonometry as the method of measurement. Based on the response of connective tissues on deformation made by tip of myotonometer, resp. its viscoelastic properties, curves in graphsare created. Three main described parameters of thecurveare steepness, deflection and its surface area. Main goal of the experiment is to compare properties of different types of tissue and their potential differences while denervated or innervated. Results of this study may help with better understanding of the soft tissues behavior in response to manual therapeutic contact. The study also describes the differences between in vivo and post mortem tissue properties that may be help further studies which use post mortem tissues to predict in vivo behavior. Results: Soft tissues in vivo have higher viscosity. In comparison, post mortem tissues have significantly higher stiffness and energy dissipation than in vivo. Tendinopathy of the Achilles' heel as effect of functional disorder of thelocomotive system

This diploma thesis deals with the Achilles tendinopathy, and particularly with its etiopatogenetics. The theoretical part includes information about the Achilles tendon, tendinopathy of Achilles tendon and a summary of dysfunctions of the locomotor system. We assumed that the dysfunction of the locomotor system is the main etiopatogenetic factor of the Achilles tendinopathy, and that it results in changes of its position and loading. In the practical part we tried to reveal these dysfunctions by examination of seven patients, and to prove its main etiopatogenetic influence by a successfull therapy. Physiotherapy Achilles tendo rupture incurred during sport

The Bachelor thesis deals with the theme of "Physiotherapy of rupture of the Achilles tendon due to sport". The rupture of the Achilles tendon is most common by young men who are dedicated to professional or solely amateur sporting activity. Pathologies of the Achilles tendon are becoming more common due to an increase in sports facilities and better access to leisure sports. In the case of a tendon injury, the patient is forced to undergo treatment. Currently there are three ways of treatment: conservative, optative, and miniinvasive treatments by percutaneous technique. The latter two techniques are a popular choice among active people and athletes. The theoretical part summarizes the knowledge of the Achilles tendon, the anatomy, physiology and biomechanics. This part also deals with the emergence of rupture (its causes), clinical contexts and tests. Finally, there is a description of the various types of therapy of rupture, physiotherapy and possible contraindications during treatment. In the theoretical part, I also included a set of preventive measures which make it possible to prevent injuries. The importance of these principles is in the fact that their compliance is disappearing in the field of professional sports. The practical part was processed by qualitative research and includes targets and research questions, methods of work, the course of therapy and a description of the processed results in the form of case reports of patients. The task of set target is suggestion and realization of individual physiotherapy in patients with rupture of Achilles tendon. The research question focuses on monitoring changes in the investigated parameters after physiotherapy. The methodology section includes a characteristic of the study sample which consists of two patients diagnosed with total rupture of the Achilles tendon, both were treated at the Department of Rehabilitation of the Hospital in České Budějovice, JSC. Patients suffered an injury during sport activities (basketball, football) and subsequently they underwent operations in the form of an open operation. In this part of thesis a description of the input and output tests will be found. First, data was collected from anamnestic of patiens. Each examination of kinesiology analysis were then evaluated. The techniques of data collection were interviews and observations. The process of physiotherapy treatments recorded several local problems, which were necessary to focus while therapy was in occuring: swelling of the surrounding tissues, limitation of movement, change of soft tissue in the ankle and instep, weakened musculature leg and thigh, and rigidity of Achilles tendon. It was also necessary to conduct reeducation of walk and proprioception training with patients. After application provided physiotherapy treatment was observed in two patients improvement of values of somatometric measurements (circumference thigh, calf muscle, ankle, head of metatarsals), momentum in the ankle joint in dorsiflexion and plantarflexion and improve of muscle strength especially in the m. quadriceps femoris and m. triceps surae. Tasks of the thesis were fulfilles and the research question is answered. Results will be made public for professional physiotherapy public and the results should be useful for clinical practice of physiotherapists and for students in modern physiotherapy techniques in the diagnosis.
The pain of Achilles tendon in endurance runners aged 15 to 30 years

This bachelor thesis focuses on long-distance runners suffering from pain of Achilles tendon. It presents basic findings about reasons of the inconvenience, prevention and treatment. The first objective of the thesis was to describe potential causes of pain of Achilles tendon suffered by long-distance runners; the second objective was to propose prevention measures and the third objective was oriented at compilation of a suitable therapeutic procedure. The research questions asked whether the proposed therapeutic plan would be effective and whether it will operate also as a prevention of an injury. The theoretical part contains basic information about the leg anatomy, brief description of long-distance running and its techniques and an overview of potential causes of injury or pain of Achilles tendon. It also contains a lists of methods that prevent the injury and examination methods, including instructions about pathology identification, and it is concluded with a list of treatment methods. The practical part used a method of qualitative research, including interview and observation (input and output kinesiological analyses), special examination methods, processing of case reports and data analysis. The research group consisted of 2 male and one female track and field athletes specializing in long-distance running from the Sokol Opava sports club. Each of them underwent a therapy that lasted approximately two months and the results have been provided in case reports. Each case report includes the case history, initial kinesiological analysis, short-term and long-term rehabilitation plan and the course of the therapy. Each case report is concluded with a kinesiological analysis and evaluation of treatment effects. Results of the research have been affected by the approach of the individuals and severity of their problems. The pain has completely disappeared in one person and it has persisted in the other two. The two latter cases were affected by the following factors: in the first case the quiet mode was not complied with and the rehabilitation was conducted at full load; in the second case the significant factor was the severity of the problem as the person had suffered Achilles tendon rupture in the past. The bachelor thesis has been written to present the acquired information and experience in order to inform the general public interested in this issue and, particularly, coaches, runners and physiotherapists.
