Objectivization of a postural mechanism in patients with a spinal cord injury

It is important to maintain a posture via equilibrial and protective reactions in all activities of daily living. There is normal neurological finding in upper extremities in patients with a spinal cord injuries in the area from Th1 to Th7, muscles of the thorax are paralysed under the level of the lesion and when there is a complete2 damage of the upper pectoral spinal cord, abdominal muscles are always paralysed. There is an important part in a therapy of patients with a spinal cord lesion, which consists of an activation of the postural system by means of sensomotory stimulation exercise according to Janda called sitting on a cylinder. The aim of this study is to objectively evaluate the impact of the exercise on a cylinder on postural system of patients with lesion of postural muscles. Surface electromyography was chosen as the most suitable objective method . Special set of testing movements on a deckchair or on a cylinder was created. Selected patients were tested by exercises on a deckchair or on a cylinder before a stay in the centre. After a ten-day stay the patients are tested by the same set of exercises and evaluation of their muscular activities is performed. Rehabilitation of the carpal tunnel syndrome

In the theoretical part the thesis gathers basic information on carpal tunnel syndrome (CTS) - its etiology, pathophysiology and clinical picture. The thesis further describes the anatomy and function of hand following the CTS, the diagnosis of disease and treatment options. The thesis also presents possibilities of prevention when working with computers. In the practical part the thesis describes a case of a patient with the CTS disease and suggests using an appropriate treatment methods to postpone a surgery. For treatment application a female patient 36 years old diagnosed by a neurologist with severe stage of CTS has been selected. The patient completed 8 individual targeted therapies during four weeks with thorough investigations and various methods of treatment applied, particularly the mobilization, electrotherapy, thermotherapy including an auto-therapy brie-fing. The results showed that the therapy brought subjective and objecti-ve improvement and surgical treatment could be postponed. The impro-vement of subjective symptoms continues passing time interval after the physiotherapy. The practical part of this thesis has been limited by possible number of therapies as well as by the therapy duration.
Back school for fitness treinees

In introductory chapters the work deals with theoretical starting points of exercising in fitness centres. It focuses on anatomical and physiological basis of exercises and neuromuscular imbalances which may be caused by inadequate exertion. In further chapters the work enlightens about the rudiments of sport training, training of strength and model form of a training unit. In final chapters suitable techniques of exercises of individual muscular systems are discussed. Common mistakes done by clients are also described.
Physiotherapy and It's Role in Impingement Syndrome

Subacromial impingement syndrome is among the most frequent cases of shoulder pain and dysfunction in adults. It's reffered to as shoulder pain is third only to headache and back pain in freqency seen in the practitioner's office. It's known, SIS is a heterogenous entity, which can be caused by a lot of ethiological factors. These factors can have bad influence of architecture of the subacromial space. The tendons of the rotator cuff are passing trough this space and so that structures are most often affected by degenerative disorders. Clinical history and physical examination play a key role in diagnostics. Provocative manoeuvres (Neer's sign, Hawkin's test, Yocum manoeuvre) seem to be useful for identifying impingement syndrome. If there is a reasonable doubt in diagnostics we can use imaging methods. We should start with the minimally invasive and the most economic method. Commonly used course of action include x-ray, ultrasound and MRI. Well-timed and the most accurate diagnostics enable us to start suitable treatment. The treatment commonly starts with pain-relief drugs (NSAID, subacromial injection of mixture of corticosteroid and local anestethic). Impact of the "Spacecurl" excercise therapy on a hip extension movement stereotype, objectified by PEMG

The subject of my Bachelor thesis is impact of the "Spacecurl" excercise therapy on a hip extension movement stereotype, objectified by PEMG. In the theoretical part is intended to give a review of anatomy, biomechanics and kinesiology of the hip. There are also summarized information about the movement stereotypes according to Janda - mainly extension of the hip, the method PEMG, Sensory Motor Stimulation and the "Spacecurl". The second part is a practical and contains kinesiological analysis, screening method PEMG and exercise unit, the respondents conducted on the unit 9x ( 20 min, for 3 weeks).
Hydrokinesiotherapy

In this work I tried to summarize all the possible ways how we could take advantage of hydrokineziotherapy and implement it into different levels of prophylactic procedures in health care. It is based upon the knowledge of locomotion itself and its control mechanisms on one hand and the physical characteristics of water on the other. Very interesting is the comparison of movement on shore with the movement in water in relation to control mechanisms of movement. To help to compare those two adequately there is also an explanation of how the body and mind gets used to being in water enviroment. This is connected to the movement automatisms that the brain has got and uses in everyday life. These automatisms in order to master the movement in water precisely have to be rearranged and adjusted. In the chapter that deals with the different types of exercise I concentrated on the most common types of gymnastic hydrokinesiotherapeutic exercises with the slight accent on swimming and pasive relaxation which creates bases for the next chapter. This chapter which is also the last explains how different methodics and programs can bu put into practice. Physiotherapeutic procedures in paraplegics after spinal cord lesion

Rehabilitation of patients with spinal cord injury begins immediately after the accident. Before begining a particular therapy, a specific neurological examination according to ASIA (American Spinal Injury Association) protocol is required, which allows to determine the level and severity of neurological damage. Specific rehabilitation procedures and methods are used to minimalize the neurological and functional deficits that occur after spinal cord injury. Procedures that are used include procedures from rehabilitation nursing, pasive and active movements, methods based on neurophysiology, techniques from breathing therapy, manipulative therapy, exercises with apparatus (MotoMed, Lokomat), FES, IMF-therapy, verticalisation, relaxation techniques, physical therapy and other. The goal of therapeutic rehabilitation in acute, subacute and chronic phase of spinal cord injury is maximal restoration of function, best use of remnant muscle potential and the formation of alternative mechanisms to reach the highest level of self-sufficiency and quality of life.
