Subjective effect of physiotherapy in patients with hereditary and diabetic neuropathies (questionnaire study)

The aim of this thesis was, through the use of the questionnaire study, to gain information on possibilities of utilization of physiotherapeutic procedures and their subjective effects on patients with inherited and diabetic neuropathies. There are also a prosthetic care and surgical solution of leg's deformities, contribution of spa systems in comparison with an ambulatory ( non-resident ) care and the life style's adjustments regarding the disease included in the survey. Vojta's reflex locomotion in hereditary polyneuropathies

CMT disease is the most frequently type of the hereditary polyneuropathy. In the Czech republic are some 2 - 4 thousand people with this disease. Clinical picture is characterized by deterioration peripheralnerves, that faces to muscular atrophy and mobility disorders. Lower limbs are affected early than upper limbs. On the lower extremitywe observe typical deformity progress - pes cavus. Fingers on the hand are in claw position. The disturbed peripheral nerves and foot deformity affect balance, movement and stability. Influencing of spasticity by means of selective dorsal rhizotomy in patients infantile paralysis

A neurosurgical method of the selective posterior rhizotomy (SPR) is used for a treatment of spasticity. The aim of this study is to evaluate an effect of the SPR on the reduction of the spasticity and on functional abilities of patients with cerebral palsy. Five patients (four males and a female) aged from 12 to 21 years with a spastic quadruplegia were tested by an Ashworth scale, modified Ashworth scale, Peacock scale and Barthel index of ADL. An initial assessment was preformed one day before the SPR. First assessment of changes was conducted one week after the SPR. An repeated follow-up assessment was done three to four years after the SPR procedure. In one week after SPR assessment a reduction of spasticity of lower and upper limb muscles and reduction of clonus were seen. A last three-year assessment detected some return of spasticity on hip adductors, plantar and dorsal flexors muscles. However, the return of the hypertonus did not reach initial values of spasticity. Objectification of disorders fine motor skills of the upper extremities in patients with hereditary neuropathies

In the first part of the thesis Objectification of disorders of the fine motor skills of the upper extremities in the patients with hereditary neuropathies there are mentioned some brief informations about characteristics, classifications, clinical symptoms, deformities and testing of upper extremities, rehabilitation and treatment of CMT neuropathy. The practical part of this thesis is concerned with testing and evaluating of strength, fine motor skills and sensation of the upper extremities in the patients with CMT. For the measurement of the hand strenght the dynamometry and the functional muscle test are used. The Jebsen-Taylor test, the Nine-Hole Peg Test and the examination of static and dynamic handgrip rating are used. For the examination of the sensation the Nottingham Sensory Assessment is used. CMT neuropathy score and Overall Neuropathy Disability Scaleare are used for the classification of disability. The aim of this thesis is the comparison of the muscle strength and the function of the dominant and non-dominant hand. Partial aim of thesis is detection of the correlations between tests. This thesis is addressed to chinese martial art Tai Chi, which it wants, among other things, to introduce on the basis of information acquired from evidence based medicine studies. This thesis presents basic pieces of knowledge about Tai Chi and its appropriate effect on patients with cardiovascular diseases, osteoarthrosis, rheumatoid arthritis, Parkinson disease and balance disorders. It also considers the usage of Tai Chi in fall prevention among the elderly, biomechanical characteristics and effectiveness of Tai Chi on mental state of human being. The Effect of Vojta's Reflex Locomotion on the Stability of Patients with Hereditary Polyneuropathy

This thesis deals with the influence of Vojta's reflex locomotion on stability of the pacients suffering from hereditary neuropathy. In the review of knowledges there is presented the part about hereditary neuropathies, it's classification, clinical symptoms and diagnostics. The second part of the review is about the stability and it's determination. The last part is dedicated to Vojta's reflex locomotion. In experimental part, there is presented studies, which evaluates the influence of Vojta's reflex locomotion on the sensitive functiones (discrimination, texture perception, vibration perception) and the postural stability (using Balance Master system) in the two groups of the pacients suffering from hereditary neuropathy. The first groupe was measured before and after Vojta's therapy and the second groupe (control one) was measured in the same way, but without therapy. Consequently, the basic statistic indicators of gained set of data were calculated. As a conclusion, there has been some changes before and after therapy in sensitive functiones (vibrations perception, discrimination) and postural function and one test of postural stability in the group of patients threated with Vojta's reflex therapy, compared to control group. This thesis "The interindividual variability in the investigation by Balance Master® System" resolves the problem of postural stability in the context of age and incidence of external factors as day-time, physical activity or stress in tested individuals. The aim of the work was to summarize knowledge about the postural stability, mechanisms of processing, holding and testing postural stability. The target of the research was to verify if there are any differences in quality of postural stability between the group of healthy young individuals and the group of healthy elderly by Balance Master® measurements, and if the quality of postural stability is affected by the day-time, fatigue and stress. It was investigated 6 tests: Weight Bearing/Squat (WBS), Modified Clinical Test of Sensory Interaction on Balance (mCTSIB), Limits of Stability (LOS), Rhytmic Weight Shift (RWS), Step Up/Over (SUO) a Forward Lunge (FL). The relation between the locomotor system and gastroesophagial reflux

The purpose of this dissertation is to explore the issue of the gastrooesophageal disease relating to the locomotive system. Special emphasis is given to the relation between the gastrooesophageal disease and the diaphragma and its viscero-somatic projection (viscero-vertebral projection). This connection will be verified on the clinical observation of a group of patients with the 81 gastrooesophageal disease. We have tried to design a physiotherapy approach to this disease in this dissertation. We have reached the view, that the diaphragm has an essential function, for it is a part of the antireflux barrier and its pressure activity affect the right function of the gastrointestinal system. We have also found, that the disorder in the area of gastrointestinal junction project to the locomotive system and constitute here the reflexive changes, which form specific visceral pattern.This pattern show side asymetry with the total dominance on the left side. The strong dysfunction of the deep stabilisation muscle system of spine, especially of diaphragm is typical for the patients with gastrooesophageal reflux.
