Lumbar spinal stenosis: changes in the fibrous structures of the posterior spinal column and the rule of the ligamentum flavum
Ježek, Jakub ; Krbec, Martin (advisor) ; Chaloupka, Richard (referee) ; Matějka, Jiří (referee)
The dissertation is focused on lumbar spinal stenosis (LSS), because it is a serious desease, that primary affects elderly patients and which often leads to disability. Because the pathophysiology or feasible prophylaxy is not yet fully understood, the first part of the thesis is devoted to the ligamentum flavum (LF), whose changes (hypertrophy and degeneration) are one of the main factors leading to LSS. We focused in this experimental part on changes in the LF properties that could cause its degeneration and hypertrophy - changes in vascularization (investigated as vascular density - Lv), the presence of inflammatory changes and the occurrence of chondroid metaplasia, and then to clarify changes in the LF mechanical properties (stiffness). Vascular density analysis of degenerated and healthy human LF combined with measurement of micromechanical properties was performed. It was found that Lv changes were not dependent on the degree of LSS but were age dependent, that is, LF vascular density declines up until 50 years of age and thereafter changes very little. A significant increased incidence of chondroid metaplasia and clear signs of inflammation in LSS patients were found out. LF samples from LSS patients showed significantly lower stiffness in comparison to samples of healthy patients. The...
BRDF workshop
Matějka, Jiří ; Pelikán, Josef (advisor) ; Karlík, Ondřej (referee)
This thesis develops an application for interactive experiments with BRDF functions. Arbitrary BRDF function may be specified with an analytic formula, including arbitrary number of parameters. A simple scene is displayed, using specified BRDF. The parameters can be modified which has an immediate impact to the scene's appearance. The application can utilize a programmable GPU for high speed of rendering.
Lighting workshop
Matějka, Jiří ; Pelikán, Josef (advisor) ; Horáček, Jan (referee)
The object of this work is to design and implement an application for interactive experiments with local illumination models. The user can arbitrarily define an illumination model with mathematical formulas and mark certain parameters to be changeable. While such parameters are changed, the program smoothly draws an updated 3D scene. The scene can be defined in the text format, while several scenes are bundled with the program. Two predefined well-known illumination models are also bundled with the program - Strauss' and Phong's model.
Possibilities of influencing the healing and ligamentization of the graft during anatomical reconstruction of the anterior cruciate ligament - Results of the Prospective Randomised Study
Kasl, Jan ; Matějka, Jiří (advisor) ; Paša, Libor (referee) ; Pilný, Jaroslav (referee)
Aims Prospective, randomized, non-blinded study, on the effects of use of platelet-rich fibrin (PRF) in anterior cruciate ligament reconstruction surgery. Methods Initially, 40 patients between 2012 and 2014 were enrolled in the study and randomized for the treatment with (n=20, intervention group) or without (n=20) the use of PRF (controls). The average age was 29.1 years at inclusion, with 12 female and 28 male patients. Vivostat ® PRF was obtained from patient blood and applied onto the hamstring graft. Magnetic resonance imaging (MRI) documented healing was evaluated at 6 and 12 months. Clinical outcome parameters as well as standardized evaluation of knee laxity using the GenouRob device were collected at 12 months. Results Thirty-three patients completed the full follow-up (17 intervention, 16 control). In two cases, a graft tear was documented (occurred in the control group). However, there were no differences in the remodeling progress in MRI using the log-rank test (p=0.07). There was no difference in the return to pre-injury sport levels (p=0.232), Lysholm and IKDC scores (p=0.259 and p=0.364 respectively). Knee laxity measurements showed no differences between the intervention and control groups. Discussion Similar results we found in our study have recently been published by other world...
Biomechanical Study of Force Transmission in the Area of Periprosthetic Fracture of the Distal Femur in Individual Types of Osteosynthesis with Application in Practice
Malotín, Tomáš ; Matějka, Jiří (advisor) ; Džupa, Valér (referee) ; Rozkydal, Zbyněk (referee)
Periprosthetic distal femoral fractures (PDFF) constitute an unpleasant complication in patients with a total knee replacement (TKR). The incidence reported in literature is 0,3-2,5 %. The number of periprosthetic knee fractures has been increasing due to the ageing of population, a growing number of implants, a longer life expectancy of patients, a more intensive physical activity of patients, and osteoporosis. Most of these fractures are treated surgically, non-surgical treatment is reserved solely for patients unable to undergo a surgery for general health conditions. Our retrospective study evaluated the group of patients with PDFF who were treated at our department in the period 2007-2020 and 4035 primary TKR were performed. The total number of patients with PDFF was 88. The mean age of patients with PDFF was 77 years (56- 94 years) and at the time of fracture the mean age was 71 years in men and 78 years in women. The average time from the TKR to periprosthetic fracture was 8.2 years (0-20 years). The fractures were assessed using the Su classification modified by Krbec. A primary TKR was performed in 75 cases for gonarthrosis, in 7 cases for rheumatoid arthritis and in 6 cases for secondary,post-traumatic gonarthrosis. The average incidence of periprosthetic distal femoral fractures was 6...
Radiological, Histochemical and Histological Changes M. Erector Spinae by Patients with a Fracture of Thoracolumbal Spine Treated With Open or Miniivasive Stabilisation
Matějka, Tomáš ; Matějka, Jiří (advisor) ; Džupa, Valér (referee) ; Chaloupka, Richard (referee)
Today, miniinvasive operations are part of modern medicine and in recent years have partially replaced open methods. The aim of this study was to compare radiological, histochemical and histological changes of the erector spinae muscle in patients with thoracolumbar spine fractures treated with open (OPEN) and miniinvasive (MISS) stabilization. The purpose was not to evaluate one method as better or worse, it was only a comparison and finding out the advantages and disadvantages of individual operations. During the study, we compared both the above-mentioned differences in the quality of the erector spinae muscle and laboratory changes in blood levels of muscle enzymes, we compared the duration of operations, the accuracy of insertion of transpediculars screws, blood loss, kyphotization of spine segments and exposure of radiation. It has been hypothesized that muscle damage in all factors will be lower in MISS than in OPEN. A total of 38 patients with type A and B fractures according to the Vaccar classification, 19 underwent MISS surgery and 19 at the OPEN method were included in the prospective randomized study. The average age of the whole group was 46.4 years (18 - 68), the average age for MISS procedures was 42.6 (18 - 66), for OPEN was 48.7 (19 - 68). Only monotrauma of the TL spine was...
