PEDI - Pediatric Evaluation of Disability Inventory: Practical Application in Occupational Therapy

Eliška Haškovcová Title: PEDI - Pediatric Evaluation of Disability Inventory: Practical Application in Occupational Therapy The Pediatric Evaluation of Disability Inventory (PEDI) is a standardized assessment developed in the USA. The PEDI samples key functional skills, caregiver assistance and modification in three content domains: self-care, mobility and social function. This thesis is aimed at the characterization of the PEDI and its practical application in occupational therapy. The theoretical part contains introduction of the PEDI, description of cooperation between occupational therapist and parents and brief description of functional skills development to the age of 7. Occupational therapists and parents information value of the PEDI is described in the practical part. The advantages and disadvantages for occupational therapist are also summarized. 20 assessments were used to achieve the aims of this theses. Feedback from the respondents was recieved using the anonymous online questionnaire. The detailed case study and graphs demonstrate the results of the research. Analysis of Painful Shoulder Therapy for Stroke Patients in Acute and Subacute Phase

Title: Analysis of Painful Shoulder Therapy for Stroke Patients in Acute and Subacute Phase Abstract: The aim of this thesis is to describe the possibilities of occupational therapy interventions aimed at preventing the development of painful shoulder syndrome or eliminating its manifestations. The theoretical part contains a summary and explanation of the basic concepts and description of individual facilitatory methods that occupational therapists can use when working with this syndrome. In the practical part there are four case reports of patients who have suffered a stroke and already been diagnosed with painful shoulder snydrome, or been suspected of developing this syndrome. The best methods for these patients were chosen and applied to affect the painful shoulder syndrome and the deficiencies that have arisen. According to the results, these patients have achieved a reduction in pain, increased self-sufficiency and quality of ADL and last but not least, an increase in range of motion, which also affects the quality of life. Creating a draft translation version of "The Chedoke Arm and Hand Activity Inventory (CAHAI)" evaluating manual

This bachelor thesis deals with a translation of the manual for the evaluation tool "The Chedoke Arm and Hand Activity Inventory (CAHAI)". Creating a translation draft of the CAHAI manual is this work's main goal. The theoretical part summarizes knowledge about cerebrovascular accident (CVA), and its consequences. Next, the theoretical part deals with a function of the upper limb and the testing of the upper limb in occupational therapy. A part is also dedicated to a summary of the evaluation manual of the CAHAI. The process of translating technical and academical texts is also discussed. In the practical part, the author describes the realization and the method of translating. The secondary goal of this work is to perform a pilot study with 3 patients after cerebrovascular accident using the Czech translation of CAHAI-13. For that reason, the practical part also contains case studies. The thesis contains case studies with a diagnostical part i.e., case studies do not have a plan and process of therapies. The result of this thesis is the Czech translation of the CAHAI and an evaluation of CAHAI's usage in research. Title: Pilot Testing of Usability "Action Research Arm Test (ARAT)" in Persons after Acquired Brain Injury Abstract: The bachelor thesis is focused on use of the Action Research Arm Test (ARAT) tool in person after acquired brain injury. The often problem in this population may be decreased or impaired function of upper limbs, which is what the ARAT is dealing with. The aim of the thesis is assessment of the tool and its characteristics. Theoretical part describes different types of brain injury, primarily stroke and their consequences. A chapter about grips and their pathologies follows. Grips are the dominant part of the ARAT testing. The second half of the theoretical part describes testing in occupational therapy and importace of standardised tool. The ARAT is further described in detail. Other tools for testing upper limb function, are briefly described in the conclusion. The practical part deals with pilot testing of the ARAT with the 8 patients. The effect of chronic cold on Ca2+-ATPase (SERCA2) in rat heart

Acute cold exposure is a significant stressor activating heat production by shivering after the prolonged exposure cellular oxidative stress increases. Chronic exposure to cold lasting at least 2 weeks leads to the development of cold acclimatization. The main thermogenic role is taken over by non-shivering thermogenesis taking place in brown adipose tissue, which significantly increases its weight due to cold. Cardiac hypertrophy, hypertension and impaired renal function are frequently observed pathologies of acclimatization at 4-5 řC. Our laboratory recently introduced a model of mild chronic cold acclimatization at 8 řC, during which no damage to the heart or kidneys occurs and has proven cardioprotective effect on reducing infarct size. Hence, the influence of this cold acclimatization model on the other cellular and molecular processes needs to be investigated. The cardioprotective effect of cold acclimatization includes changes in β-AR signaling, activation of anti-apoptotic pathways or augmentation of the antioxidant system. Title: Occupational Therapy Assessment of People with Guillain-Barré Syndrome in Productive Age Focusing on Possibilities of their Employment Abstract: Guillain-Barré syndrome is an acute inflammatory neuropathy affecting the peripheral nerves. Its most common symptoms are symmetrical weakness, myalgia, paraesthesia or dysesthesia, areflexia, and temporary quadruplegia. The syndrome starts affecting lower limbs. It progresses in the cranial direction which may lead to ventilation insufficiency and autonomic dysfunctions. It may even affect ability to work of people in productive age. Occupational therapists may specifically target the employment opportunities of people with Guillain-Barré syndrome and support them in returning to work as part of occupational therapy interventions conducted in health and social care institutions or prevocational assessment centers. The role of beta1 adrenergic receptors in heart of cold acclimated rat

During cold acclimation the heat production is shifted from shivering to non-shivering thermogenesis, which is mediated by adrenergic signaling. It has also been observed, that cold acclimation may increase the organismal resistence to pathological stimuli and may affect functional parameters of cardiovascular system. However, acute exposure to sever cold is often associated with detrimental effects on the body. We have recently shown that chronic exposure to cold increases the heart's resistance to ischemia-reperfusion injury without negative side effects when mild temperatures are used, however the mechanism of protection is not yet known. The aim of this work was to determine whether: i) if the sensitivity of the heart to ischemia changes already after the first day of cold exposure and does not show any negative effects, ii) if β1-adrenergic signaling plays a role in chronic regimen of cold-induced cardioprotection. The results of this work showed that i) one day of exposure to mild cold did not change the sensitivity of the heart to ischemia and ii) metoprolol treatment reduced the infarct size in the control group, but did not affect the heart of cold-adapted rats. Clinical Utility of Occupational Therapy Standardisee Test "Action Research Arm Test (ARAT)"

The Action Research Arm Test (ARAT) is a standardized tool, which is used for fine and gross motor skills evaluation of hemiparetic patients after stroke. It is widely used abroad thanks to its excellent psychometric properties and easy administration. It is also used in many foreign studies to evaluate the function of upper extremity, especially for neurological patients. Unfortunately, so far it has been used only sporadically in the Czech Republic. In the theoretical part of this thesis the ARAT and the Modified Frenchay Scale (referred as "MFS") is described. In the practical part, the MFS is compared with the ARAT. The main aim of this diploma thesis is a pilot comparison of sensitivity of upper limb motor skills evaluation done by ARAT and MFS. Thirty patients after stroke were tested in the Rehabilitation Center Kladruby in three weeks. They were further divided into three intervention groups according to the severity of the paresis (light, medium, heavy). The analysis of the results shows that the sensitivity of both diagnostic tools is statistically comparable. According to the Spearman correlation coefficient, the ARAT and the MFS correlate very strongly in range of 0.96-0.98.
