Problems with pelvic floor during painful intercourse

This research bachelor's thesis deals with the issue of the pelvic floor in painful sexual intercourse. At the beginning of the theoretical part, the anatomy and kinesiology of the pelvic floor are described. The physiology and pathophysiology of pain are also outlined together with the evaluation. The thesis deals with the description of female sexual dysfunctions, summarizes the cause generating changes in the pelvic floor, which can subsequently lead to painful intercourse or impossibility of intercourse due to pain. The main part of the thesis then deals with the possibility of using physiotherapeutic methods and concepts to influence dyspareunia in musculoskeletal causes. The practical part of the thesis contains a case history of patient with dyspareunia caused by a change in the pelvic floor, its kinesiological analysis, including palpation examination of soft tissues in the pelvic area and a therapeutic plan using several methods of physiotherapy.

Obligations of the employer in sickness and pension insurance Abstract This thesis deals with the individual obligations of the employer in sickness and pension insurance. Sickness and pension insurance are two of the social insurance subsystems. The state does part of its responsibilities in this area through employers, as they have the easiest access to some information. Of course, the state could obtain information from the employees themselves, but this would represent a huge administrative burden on the state and a great logistical problem in how to actually obtain information from the employees. Obligations that an employer must fulfil may be difficult for a small employer who does not want to hire external companies to fulfil its obligations to the state and it can be almost impossible to fulfil all obligations properly. The most well-known obligations of employers, of which (perhaps) all citizen-employees know, are payments into the sickness and pension insurance system in the form of social security contributions. The employer pays the social security contributions for himself and his employees. However, this is certainly not the only obligation of employers. Application of physiotherapeutic procedures in conducting physiological childbirth

This bachelor thesis explores ways physiotherapeutic procedures and concepts can be used to effect the proceedings of physiologic childbirth. The beginning of the theoretical part describes anatomical structures relevant to pregnant women, the stages of labor, pain and pain medication. The thesis then deals with ways of influencing changes in the mother's body during pregnancy. With physiotherapeutic interventions we can achieve positive impact on the pregnant woman's body and her physical condition. Through these interventions the expectant mother is better prepared for labour. The topic of the main part of the thesis is the application physiotherapeutic methods and concepts during childbirth. The selected exercises and techniques to be used at each stage of labour can reduce labour pain and shorten the whole delivery process. The last chapter discusses exercise to be done during the puerperium. The practical part of the thesis contains a case report of an expectant mother, her kinesiological analysis in pregnancy and after the labour, description of her labour, a therapeutic plan and a description of her therapy sessions. The practical part also includes a questionnaire research of 51 women after vaginal delivery.
Calf muscle abreviation. The influence of foot and ankle disorders and the possibility of treatment with statical stretching.

The calf muscle (m. gastrocnemius) is a postural muscle with a tendency to shorten what is often present in patients with foot pain. The theoretical part summarizes the basic knowledge of anatomy, biomechanics of calf muscle and manifestations of its shortening in the periphery. There are also described the possibilities of conservative solution by static stretching and possible operative solution. The aim of this work is to find out whether the pain can be reduced or eliminated by a conservative method by static stretching in order to avoid the need for an operative solution to this problem. We present a set of 18 patients sent for rehabilitation with a focus on calf stretching. In a practical part we show 3 cases for which an individual exercise plan was introduced focusing on calf stretching for 6 months. Within 6 months, these 3 patients were goniometrically examined three times by a 3-month interval to measure the ankle range of motion (ROM). The results showed that 2 patients objectively increased ROM and subjectively got rid of pain. In 1 patient, despite the objective increase in ROM, the subjective sensation of pain still persisted. Thus, it can be shown that by static stretching we can increase the ROM and consequently affect the pain.
The effect of body position on pressure in anus during relaxation and contraction of the pelvic floor muscles. Pilot manometric study

This Research thesis is focused on the influence of body position on the function of pelvic floor muscles (PFM). Anatomical structures, kinesiology and neurophysiology of the PFM are described in the theoretical part of this thesis. It also discusses the methodology used for objective evaluation of the PFM. For the practical part of this thesis, 30 healthy subjects (aged 20 - 30 years, 15 male, 15 female) where chosen via a questionnaire. The subjects were examined via the method of anorectal manometry. The goal of the examination was to measure the distribution of a) resting pressure in the anal canal in selected positions, b) pressure during voluntary maximal contraction of PFM and sphincters in selected positions, c) pressure during 20 seconds of voluntary contraction. Selected positions are: lying on the back; lying on the back with legs elevated and held in "three flexion"; kneeling on all fours with palm support; kneeling on all fours with elbow support; squatting, and standing. Results: a) The resting pressure is influenced by the body position. Highest resting pressure was observed in squatting and standing positions, lowest resting pressure was measured on subjects while kneeling on all fours with elbow support. Effect of yoga exercise on pulmonary function and ceompensation of bronchial asthma.

Bronchial asthma incidence constitutes a compelling health and socioeconomical problem. Patients of all ages suffer from asthma, and no methods of effective prevention or healing are currently available. Poorly controlled asthma causes increase of financial expenditure as well as remarkable restriction of activities of daily living; it might even lead to death. In Czech republic, the number of patients with asthma is estimated at 500 000. The aim of this study is to evaluate effects of yoga exercise as a potential effective adjunct treatment on bronchial asthma state in adults. Theoretic part of the thesis describes pathological changes present in respiratory system, breathing pattern disorders and postural functions characteristic for patients suffering asthma disease. It also represents a brief summary of up to date findings of possible mechanisms on which are Spiral yoga and pranayama based, especially on these which can have an influence on obstructive lung disorders. Experimental part deals with the methodology and it reveals the process of making randomized clinical pilot study. Physiotherapy in prevention and therapy of perineal injuries and episiotomies

This bachelor's thesis summarizes the information of pelvic floor function and perineal injuries that are caused by the delivery. In the theoretical part of this thesis are described anatomical structures in the pelvic area and their changes during the pregnancy and the puerperium. The main part of the thesis is focused on possibilities of preventing the perineal injuries. There are described problems that are caused by perineal lesions and dysfunction of the pelvic floor. Options of physiotherapeutic treatment of these problems are discussed as well. The practical part of the thesis is supplemented with a case study of a pregnant patient, who was preparing to the delivery during pregnancy with the goal of preventing perineal injury. The case study is completed with anamnestic data collection and analysis of kinesiology of the patient in pregnancy and after the delivery. A therapeutic plan is a part of the case study as well. The thesis is supplemented with the results of executed questionnaire survey of 49 patients after vaginal delivery.
Kinesiotaping

There are many factors defining the shape and configuration of the medial longitudinal arch of the foot. In the physiotherapeutical practice, we strive to alter and improve its supportive and propulsive function. Kinesiotaping, as one of the currently very popular methods, has still not been sufficiently elucidated according to the evidence based medicine approach. The practical part of the presented thesis contributes to a better understanding of the potential effect of kinesiotaping on pronated foot using quantitative and objective methods. We evaluated the parameters obtained by footprint analysis from an experimental group (n=12) in order to investigate if they improve after 3 weeks of kinesiotaping as compared to a control group (n=12). The relationship between static measurements and dynamic tests of foot postural function was also taken into account. The results may complement the study of Luque-Suareze et al. (2014) which presents short-time effect of kinesiotaping on pronated foot.
Effect of conservative therapy to deformity hallux valgus

This thesis focuses on the evaluation of a conservative approach to the hallux valgus deformity. 21 people took part in the empirical study in which the changes were tracked. I worked with a group of women who exercised regularly and a control group. The regularly exercising group participated in a 3-month therapeutic program led by a physiotherapist. Each of the participants was properly examined at the beginning of the program and after three months, when the program finished. Each examination consisted of a kinesiologic analysis, a plantography footprint, a foot X-ray, a pain evaluation (the numeric scale was used) and filling out a scoring scale questionnaire of American Orthopaedic Foot and Ankle Society (AOFAS). The changes of pain of the MTP or IP toe joint, the development of foot arch, changes of the toe valgus angle according to the X-rays and the overall score achieved in the AOFAS questionnaire were statistically evaluated. The ability to isolate toe abduction, right posture of the foot, stability of the axial joints and other changes were followed as well.
