Bone metabolism, infection and ischemia in diabetic foot syndrome
Bém, Robert ; Jirkovská, Alexandra (advisor) ; Štěpán, Jan (referee) ; Broulík, Petr (referee)
The diabetic foot (DF) is one of the most frequent late complications of diabetes, which often leads to limb amputation and effects the quality of life in patients with diabetes. Ischaemia and infection play important role in pathogenesis of DF. One form of DF is Charcot neuroosteoarthropathy (CNO), defined as a noninfectious destruction of bones and joints in the foot due to diabetic neuropathy. The subject of the dissertation thesis was divided into 3 parts: bone metabolism and CNO, ischaemia and infection in DF and new methods in the treatment of DF. (...) In the third study, new quantitative scintigraphic parameters for assessment of CNO activity were defined and confirmed their satisfactory correlation with other parameters of CNO activity. In fourth and fifth study, good reproducibility was verified and efficiency of power spectral analysis of heart rate variability as a new method for diagnosis of autonomic neuropathy in patients with CNO. Sixth study refered to rare causes of atypical localization of CNO in the knee joint. Ischaemia and infection in DF were resolved in one study. In multivariate analysis of seventh study we approved, that risk factors for difficulty healing after minor amputation of the foot were low level of transcutaneous oxygen tension and high level of C-reactive protein before...
Vascular Calcification and Bone Demineralization in Patiens with Chronic Renal Failure
Mareš, Jan ; Opatrná, Sylvie (advisor) ; Topolčan, Ondřej (referee) ; Spustová, Viera (referee) ; Štěpán, Jan (referee)
Background: Patients treated with chronic dialysis show extreme mortality, particularly due to cardiovascular disease. Renal failure is associated with different risk factors of cardiovascular complications than those defined for non-renal population. One of these factors, currently considered most important, are vascular calcifications. At the same time, patients on dialysis regularly suffer from derrangements of calcium-phosphate metabolism, including skeletal demineralization. As a consequence, both incidence and prevalence of bone fractures are high. Aims: This study therefore investigates vascular calcifications and skeletal demineralization, their clinical implications and associations in patients treated with long-term dialysis. Methods: Two groups of patients were examined in a prospective manner (follow-up > 1 year); 32 patients treated with peritoneal dialysis and 72 treated with regular hemdodialysis. Treatment parameters and biochemical markers of calcium-phosphate metabolism were monitored throughout the study. Bone mineral density of lumbar spine (together with vertebral fractures) as well as coronary artery, valvular and aortic calcifcations were determined repeatedly. All radiological parameters were measured during one computed-tomography scanning session. Results: A significant...

Optimalizace cílové tloušťky při nepasečném způsobu hospodaření na ŠLP ML Křtiny
Štěpán, Jan
This thesis deals with the optimization of the target thickness in the forest managed under systems involving coupes, when converting to a permanent forest (not managed under systems involving coupes) on ŠLP ML Křtiny. The optimization was focused on Scots pine, European beech, Oak sessile and Norway spruce tree species. The final optimal target thickness was established as the intersection of biometric and economic aspects for pine 45 cm (range 39-52 cm), for beech 50 cm (range 44-56 cm) and for spruce 52 cm (range 48-58 cm). In the case of oak in the natural conditions, the culmination of the thickness growth is not occurred. That could reach dimensions of trees, they need, in addition to the appropriate site conditions ensure appropriate production measures the optimal cenotic position, respectively a growth area. The average social area is for oak 26.22 m2, for pine 29.08 m2, for beech 25.26 m2 and for spruce 22.88 m2. The average available crown space is for oak 27.83 m2, for pine 26.59 m2, for beech 23.64 m2 and for spruce 19.38 m2. The average available stem space is around 74 m2 for oak, 84 m2 for pine, 71 m2 for beech and 66 m2 for spruce. Since it is an exact determination of the culmination of the thickness growth using growth ring analysis is time consuming, is better a simpler (operational) the landscaping solutions in adult stands, trip during which evaluates production potential habitats and species on the basis of the achievements of the individual dimensions of the strongest individuals in a selected population. This information is suitable ideally complement the statistical data from the operational inventory, and it must modify their best as the upper quartile breast height diameter of the wood.
