Communication problems of nurses in pacients after stroke
PECKOVÁ, Jana
The bachelor thesis "Problems in nurses´ communication with patients after cerebral vascular accident? deals with the knowledge of nurses in communication with patients with aphasia after an ictus. Right communication belongs to key points of the subsequent treatment of this disease. A cerebral vascular accident ? a stroke - can negatively affect the life of a person affected by the disease. Other negatives symptoms are provoked by acquired disorders of cognitive function. Phatic failure is of a concern in the nurse-patient communication. Patients are not usually able to express themselves properly or understand others. During their hospitalization the role of a nurse is therefore very important. Nurses should be sufficiently educated in communication and to know the specifics of this disease. Compensation aids to facilitate and improve communication with patients after a stroke should also be available and used by nurses. The objective of this thesis was to determine whether nurses follow the correct procedures in communication with patients after a stroke, and to map the most frequent communication problems between nurses and patients after a stroke. The empirical part of this study consisted of quantitative research, using an anonymous questionnaire that was designed for nurses in neurological and internal wards of the hospitals in Příbram, Písek and České Budějovice. The questionnaire contained a total of 24 questions. 107 questionnaires were distributed and the research was processed with 90 questionnaires, i.e. 84% of the original number. Three hypotheses were stated. The first hypothesis assumed that nurses know principles of good communication with patients after a stroke. This hypothesis has been confirmed. The second hypothesis assumed that nurses use aids to improve communication with patients after a stroke. This hypothesis has been confirmed. The third hypothesis assumed that nurses are interested in further upgrading skills in communicating with patients after a stroke. This hypothesis has also been confirmed. The objectives have been met on the basis of confirmed hypotheses. This bachelor thesis can serve as educational material for nurses working with patients with aphasia after a stroke.
Nurse´s view on speech disorders of patiens cured from stroke
TALÁROVÁ, Libuše
This bachelor thesis deals with cerebrovascular accidents (CVA)and their serious effects on the pacient. Special attention has been paid in this thesis to speech disturbances that are the second most frequent effect of CVA. The objective of the survey was to find out whether nursing staff are informed about the speech disturbance issue, are willing to work with the client according to clinical speech therapis´ts recommendations, and whether the nursing staff have adequate conditions for work with the client. All of the objectives mentioned above have been achieved. Quantitative methods have been used for the survey, namely the analysis of the speech disturbance number to the CVA number from July 2008 to February 2009, and a questionare for the nursing staff. The survey was carried out in internal diseases and neurology departments of the Blansko Hospital. The results of investigation showed that nurses do not have information about disorders of speech and that each deparment should be designated at least one worker who was trained by clinical speech therapist and in cooperation with him, he devoted himself to the care of patients.
Demands put on the nurse during the communication with the aphasic patient
VIDENSKÁ, Magdalena
Aphasia is defined as a systemic speech disorder which is caused by organic brain damage and on different levels it affects the organisation of speech. It is caused bz the damage of the dominant hemisphere associated with tumours, injuries, brain stroke, haemorrhage, meningitis and the like. Aphasia can affect people of all ages. The communication with an aphasic patient is difficult and the demands required from nurses are high. In my work I suggest alternative way of communication which would facilitate and improve the communication of an aphasic patient with other people. My work is based on the methodological handbook ``Pictograms{\crqq} written by Mgr. Kubová. This book deals with using communication picture system {--} pictograms {--} for education and developement of social communicative skills of children and adults with either mental or multiple handicaps, and it is used in many western countries. I decided to provide nurses and aphasic patients with these pictograms. The aim was to help aphasic patients to express their feelings and needs. By using the pictograms I tried to cover the basic human needs {--} the need of bowel movement, the feeling of thirst, pain, cold and so on. After about a three month use of pictograms I asked the nurses to complete a questionnaire and express their opinion on using the pictograms and on the pictograms benefit to the aphasic patients. The research was quantitative. For the data collection I chose the method of a questionnaire. The data obtained are processed in the form of tables and charts. The research group were the nurses who, when working with aphasic patients, were using the pictograms. The research was carried out in the Hospital of České Budějovice a.s., at the Nerve ward and the Follow-up care ward. The Charity Nursing Service in České Budějovice also used the pictograms in home nursing. The best of results with the pictograms were definitely reached in home nursing. 87,5 % of the nurses recommend using pictograms. When filling in the questionnaire the nurses included not only their own experience of using pictograms but also the experience of the family members taking all-day care of their aphasic relatives. The family members were able to say how well the pictograms served their purpose. The pictograms are recommended to be used by 73 % of the nurses of the Nerve ward and 62,5 % of the nurses of the Follow-up care ward. The results say that the pictograms were best used in home nursing aphasic patients. I believe that pictograms will become an important part of the communication with an aphasic patient and that they will be used effectively even though their use is time and patience demanding. The nurse{\crq}s reward is better quality nursing care and happier patients.
