Regulation Fees in the Czech Health Care System

A large part of the work has been devoted to regulation fees as one of the health care systems regulating mechanisms. An essential place in the work is taken by elaboration of the topic of increase of costs for the health care systems functioning in the current world and the possibilities of their solution. The work also deals with an extensive and frequently discussed topic of justice in the physician-patient relationship. This topic has been processed in connection with historical circumstances and a philosophical dimension of perception of justice in health care. Further to this, ideas and ideological foundations of justice, which project into political ideologies, have been dealt with. Health care systems, the system of health insurance in the Czech Republic and its legal regulation have been briefly characterized. The aim of the work was to map an opinion of some ordinary citizens regarding perception of justice in the Czech health care and introduction of the regulation fees. Partial goals were to find out in what way political orientation reflects in the opinion on justice of the collected regulation fees and furthermore to map, in what way an amount of income of respondents influences an opinion that regulation fees represent an income for a health care facility. Two hypotheses were determined. Hypothesis no. 1: University graduates understand better the system of regulation fees. Hypothesis no. 2: More than 80% or respondents consider it unjust that regulation fees represent an income of a physician, health care facility, or pharmacy. A method of questioning was used for the processing, data collection was carried out by using a questionnaire technique. Statistical methods suitable for an analysis of categorical data were used for evaluation of information. 300 questionnaires were handed out, the return rate being 74%, the final research population consisted in 182 respondents from the city of Votice. Another employed processing method was the secondary data analysis. Results of the questionnaire research brought a number of intriguing facts relating to attitudes of the respondents as to the issue of justice and state of the Czech health care. Further to this, it has been confirmed that university educated respondents understand better the system of regulation fees. In more than 65%, respondents consider it unjust that the proceeds from the regulation fees collection represent income of a health care facility. One of the hypothesis was confirmed and the other was disproved. The acquired information can be used as a guideline for public discussion.

In May 2012, CVVM included to the survey a question about satisfaction with some areas in the Czech Republic. Citizens also responded to the question whether the evaluated areas need the reforms, and CVVM investigated to what extent the Czechs know planned reforms.

December 2011 survey of Public Opinion Research Centre included a block of questions concerning health and health care. Within the frame of this block there were investigated views of Czech citizens on different types of existing fees and on other cost-saving measures being considered in healthcare. The block was supplemented by questions asking the number of visits to GPs and medical specialists during the last year.

We were interested in satisfaction of citizens with health care and change of quality of medical care during the last year. Czechs were asked a question about current status of their health and should answer, whether doctors and other medical staff give the clients enough information about medication.

We were interested in satisfaction of citizens with health care system in the Czech Republic and public opinion on issues in specific areas of the health care system. With health care is satisfied 34 % citizens, unsatisfied 35 %.
The effects of population aging on the health care financing

This thesis is concerned with the issue of health care financing due to the upcoming changes in the demographic structure of the Czech Republic. This work documents the obvions trend in population ageing (given by the demographic development). It brings about the fast growing demand on health care and simultaneously slowly rising helth care system income. These data are used here for the health care system analysis and I used the data to analyse and estimate the future financial demand on the health care system. I concluded my thesis by evaluating possible suggestions and changes which are supported by a survey conducted in the Czech Republic. I have also included other problematic areas concerning health care, the improvement of which could result in financial savings.
Financial aspects of health care reform in the Czech Republic

Healthcare in the Czech Republic is currently undergoing reform changes. The aim of the thesis is to evaluate the upcoming changes in the reimbursement of health care in hospitals. To achieve the goal is used as the literature, as well as proposed legislation and the case law. The practical part of the thesis focuses on the hospitals. Emphasis is placed on the analysis of mechanisms fixed costs reimbursement of health care and reimbursement of health care by the DRG method. The potential impact of reform measures is presented on the example of an extremely costly medical care (orphan drugs). Based on the information and analysis are in the final part of the thesis describes the effects of health reform on financing health care in hospitals. At the same time also outlined a possible solution to save the cost of medical equipment in the field of medicines.
Analysis of health care system in the United Kingdom of Great Britain and Northern Ireland

The thesis examines health care system in the United Kingdom of Great Britain and Northern Ireland. In the beginning there is characterised the United Kingdom and its present structure of health care system including assessment of individual functions of system's parts. Then there are analysed sources of finance and their development between 1999 and 2009. The thesis continues with analysis of expenditures where is traced their development between 1999 and 2009 including international comparison. Next there are introduced methods of financial resource allocation into segments of health care system. In the last chapter there are pointed pros and cons of British health care system out and chosen certain components for application in Czech Republic.
Effectiveness of public health system and impact of introduction of regulatory payments

This diploma thesis aims to describe Czech health care system participants` motivations and impact and meaningfulness of setting up regulatory payments on supply of health care. It also attempts to suggest other meanings to rising effectiveness of whole system. This work is based on theoretical concepts of various health care systems and endeavours to expose their positive and also negative features. The theory of health care markets comes out from works of Stiglitz (1997) and Arrow (1963). They focused rather on US, but this thesis aims to use this knowledge also in conditions of Czech Republic. Thesis sum up expenditures on health care during the time in Czech Republic, compare with expenditures abroad and estimate their trend in future. The central point of this paper is the analysis of the introduction of regulatory payments and its influence on concrete types of health services and on the consumption of drugs. It includes also a short survey.
