Prehospital urgent care for patients with acute cerebral infarction indicated for thrombolytic therapy

The topic of this bachelor thesis is prehospital emergency care for patients with acute cerebral infarction indicated for thrombolytic therapy. In the theoretical part of the thesis there are data about the actual occurrence of cerebral strokes, and there are defined terms such as medical rescue service, pre-hospital emergency care and stroke center. The following chapters describe the basic anatomy of the brain. Owing to the topic the largest part of the anatomy consists of vascular supply of the brain, which is related to the cause of cerebral vascular events. This thesis also involves the theme of cerebral vascular events, where the causes of the illness, the clinical picture and its basic distribution are described. There are also described diagnostic procedures in the following chapters. Due to the subject of the bachelor thesis, the most space is devoted to pre-hospital emergency care. Further chapters focus on how to place a patient to the appropriate healthcare facility and the follow-up treatment. The last chapters of the theoretical part mention the possible prevention of this disease. The practical part of the bachelor thesis was qualitative research. Structured managed interviews with medical rescuers of the Health Rescue Service of the South Bohemian Region (ZZS JčK) and non-medical staff involved in the admission of patients to a complex cerebrovascular center ("KCC") were used for the research. There were drawn up two different interviews. The first interview was focused on mapping the knowledge of health rescuers about providing pre-hospital emergency care to patients with stroke. This interview also involved questions for the assessment of the cooperation between ZZS JčK and KCC in the Hospital České Budějovice from the point of view of medical rescuers. The second interview was drawn up for the non-medical staff of the KCC at the České Budějovice Hospital and was about the cooperation between ZZS JčK and KCC at the České Budějovice Hospital from the point of view of the KCC staff. The interviews were rewritten in the practical part and the answers were processed in tables. The research showed that ZZS JčK health rescuers have sufficient knowledge of vascular events. Furthermore, it was found, that the cooperation between ZZS JčK and KCC is evaluated by both sides positively. Research also showed that glycemia measurement is an important point of improvement for patient examination. Both variants of the interviews showed, that this value is not always measured and therefore, it can happen that a patient with hypoglycemia is brought to the KCC instead of a cesarean stroke.
The Interconnection of Components of the Coordinated Rehabilitation to Individuals after the Cerebrovascular Accident.

Cerebrovascular accident (CVA, stroke) is a disease that affects the central nervous system. Many people, even younger in age, have suffered the stroke, and especially in developed countries. For adults it falls into the most common causes of disability. Greater attention is now focused on medical rehabilitation, but all the elements of rehabilitation are important. The status after stroke results in physical, psychological and social consequences. Therefore, early and coordinated rehabilitation is important, since it increases the possibility of the person´s integration. The diploma thesis aims to describe particular components of coordinated rehabilitation granted to the patients after the CVA in the treatment and rehabilitation centre Chvaly. More specifically, the author tries to analyse the coherence of the individual components of coordinated rehabilitation. In the theoretical part, the author focuses at first on the definition of the CVA, follows up with a general treatise about the coordinated rehabilitation, and then she specifies coordinated rehabilitation for patients after the CVA, and briefly describes the facility under examination. To achieve the objective there has been used qualitative research, the method of questioning, the technique of a semistructured interview. The research sample includes people after the CVA, and the staff of the treatment and rehabilitation centre Chvaly, applying specific elements of rehabilitation. Research indicates that in the examined centre they use therapeutic and social elements, where the treatment dominates. The treatment center puts emphasis on self-sufficiency of a person after the CVA, working with the family and a multidisciplinary team, which is also perceived by the clients after the CVA. The results of the diploma thesis can serve as a feedback for the examined treatment centre, they can be used to improve the quality of coordinated rehabilitation. This work can be used as a source of information for people after the CVA and their families.
Case Study of physioterapy treatment of a patient after hemoragic stroke with left hemiparesis

Title: Case study of physioterapy treatment of a patient after hemoragic stroke with left hemiparesis. Objectives: The aim of the thesis is to obtain academic findings about stroke concerning anatomy, pathology, therapetuic approaches and methods which can be utilised in cases of stroke. The specific part focuses on the therapeutic care of a patient after hemorragic stroke which was taken during a month practice at Rehabilitation clinic Malvazinky, Medditera s.r.o. Summary: The theory of the thesis discusses anatomy and pathology of human brain in terms of strokes. There is a brief mention of ischemic strokes and a detailed elaboration on hemorraghic strokes. Furthemore, the same part of the thesis epitomizes the risk factors and diagnosis of strokes. There is also a detailed treatise on the physioterapeutic care of stroke. The practice of the thesis introduces a case study of physioterapy treatment of a patient after hemoragic stroke with left hemiparesis. Case study of physical therapy care of patient after ischemic stroke

Title: Case study of physiotherapy treatment of a patient after the ischemic stroke Objectives: The main aim of this study was to verify my ability to work with the patient, his examination, making conclusions, proposed treatment and its subsequent implementation. Other aim was mapping of the literature on the subject. Methods: The first part of my bachelor thesis is theoretical and it is aimed on stroke. There are described anatomy of cerebral artery, knowledge about stroke, its clinical symptoms and treatment options in this part. The description of spasticity with its influence on therapy and its treatment and the techniques used in physiotherapy are also there. The Brocca aphasia is described in the last section. The second part is the case report of a patient P. V. with diagnosis: ischemic stroke with dextral symptomatology. This special part is based on two weeks' work with the patient in the hospital (Ústřední vojenská nemocnice - Vojenská fakultní nemocnice Praha). This work lasted from 9th January 2012 to 3rd February 2012. Title: Case study of physiotherapy treatment of a patient after ischemic stroke with right-sided hemiparesis Objective: The main aim of this study is to analyze the theoretical knowledge about etiology, clinical symptoms, stages of disease and the following therapeutic methods used during treatment given to patients who suffered a brain stroke. Prevention of complications in the treatment of patients after stroke

The thesis deals with prevention and appearance of complications with the patients after a stroke. These patients are endangered by many complications, connected not only with the disease itself but also with their health before hospitalization. For the purposes of my thesis, I chose a qualitative method. It is a specific method which uses given problem in details and gives a complex perspective of all examined criteria. I collected the data with the help of example studies which I examined further on and compared it. The results of my research show that the most frequent complications endanger the chosen patients. Further on I deal with interventions with which I intentionally prevented the complications. In the last part I evaluate the results of preventive arrangements about how the respondents percieved my treatment care.
