Pain as indicating and limiting factor in physiotherapy
VONEŠOVÁ, Jana
This bachelor thesis focuses on the issue of pain. Pain is part of most of the diseases not only of the musculoskeletal system. This unpleasant sensation is a warning sign for the affected person, that there is some pathological process in his body, or there is going to be a pathological proces. The pain should not be underestimated, because its function is primarily protective. The pain is a subsequent process, during which is happening biochemical and neurofyziological interactions at different levels of irritation. It includes a wide range of predominantly negative perceptions of varying intensity and character. Painful mechanisms are handled by the nociceptors, thus aferent neurons with thin and thick myelinize fibres. It is conduct by the primary aferentn fibres mixed or sensitive peripheral nerves, with the way of back root into the spinal cord and ends in the substantia gelatinosa of the back corner of the spinal cord. But its intensity is very subjective and people with the same disease can experience totally different intensity. This thesis deals with the importance and possibilities of physiotherapy in the control of pain of the musculoskeletal system. The main objective of my work is to provide basic theoretical information about the pain, its distribution, examination, and treatment. The research part of my thesis will be map the possibilities of physiotherapy in some diseases. And in other part the reasons that bring the patients to the physiotherapy ordination. The first part of the theoretical work includes the definition of pain, its way via the human body and its basic division. The other two chapters of the theoretical part include the examination of pain and its treatment not only pharmacological, but also physiotherapy with a component of physical therapy, manual therapy, kineziotherapy and other methods. The practical part consists of two researches. The main, qualitative research, and in the second place, then research quantitative. The file for the quantitative research, consist of four patients. Two patients are after surgery of the lumbal spine for prolaps of the disk and their problems have been of a structural base. Another two patients don't have diagnosed structural changes, so their pain problems are of the basis of the functional changes. Every patient was sampling anamnesis, checkup by the sight and palpation examination and four times during the period of therapy was filled in a questionnaires. And there are also described the methods used in the therapies. He research consist of 152 people. In the chapter "Results" in the practical part you can find the advances and changes for all probands, the final examinations and their subjective evaluation and also the results of the qualitative work. My bachelor thesis can be used to increase knowing about physiotherapy in the case of treatment the pain, also can be instrumental towards educational material for professional and general public.
Physiotherapy of pelvic floor and its effect on menstrual problems in women
ŠTĚPÁNOVÁ, Kristina
The topic of my bachelor's work deals with issue of physiotherapy of pelvic floor and it's influence on menstrual difficulties among women. The main goal of this work is to describe possibilities of physiotherapeutical methods of exercising for pelvic floor among women with dysmenorhea. Partial goal is to create an educational leaflet for needs of these women. In order to make the probands feel better a periodic exercising was implemented. It contained special exercises focused on the right activation of the muscles of the pelvic floor. The theoretical part of the bachelor's work is focused on the anatomical relations and the function of the area of pelvis and pelvic floor. Furthermore it defines dysfuncions of pelvic floor. After that comes the definition of menstural cycle and it's hormonal indicators and list of malfunctions of menstrual cycle. In the last chapter of the theoretical part is covered a part which presents selected physiotherapeutical paradigms and supportive medical methods used for dysmenorhea. For preparation of the practical part was used the method of qualitative research.Three probands suffering from dysmenorhea and various symptoms of premenstruall syndorme were studied. The research group was between 24 - 30 years of age. The anamnesis of probands were made, the initial kineziological analyses were made and after the end of the therapy the final kineziological analyses were made. The founded data and the results of research have been worked into. For each of the probands was made an individual plan of rehabilitation by their diagnostics. Therapy took time from 20 weeks sice the first of January 2015 till May 2015. At the beginning the therapy was made once a week and in next day it was once in 14 days in duration of 45 minutes per exercise. 15 therapies should be made with each of probands. The research can be found helpful for probands, because the positive effects of the therapy were seen at each of them. The problems of dysmenorhea have became better, also the pain of locomotive system have been reduced, the patients condition have improved and the psychological problems have been reduced. Last but not least almost all the probands have stopped using analgesics in the context of dysmenorhea. This bachelor's work can be used for physiotherapeutical purposes not only for women suffering from dysmenorhea but even for women suffering from premenstrual syndrome. It can also be useful for students of physiotherapy and physiotherapists in clinical practice for comprehensive orientation in specified area and it also can be used as an information booklet for specialists and laymen.
Access physiotherapy in disorders of postural stability in patients with multiple sclerosis
KRÁLOVÁ, Lucie
The bachelor thesis summarizes the current knowledge of multiple sclerosis. The object of my thesis was to choose the appropriate physiotherapy procedures to influence postural stability in patients with multiple sclerosis. In the theoretical part of it I have dealt with the current situation of the problem, then with ethiopathogenesis, symptoms, pharmacological and rehabilitative treatment. Then I dealt with the functions of the deep stabilization system and methods which can be used to work with. These methods were chosen with regard to the diagnosis MS. The research section includes casuistry of three patients. The kinesiology examination is supplemented by testing in posturograph which patients undertaken at the beginning of the three-month therapy and after its completion. This section contains actual status of the patients, their medical history, designs short-term rehabilitation plan, a description of rehabilitation and long-term rehabilitation plan and overall assessment of therapy and research. The whole research took place over a period of three months. Two patients attending for treatment in a rehabilitation clinic in J.Hradci, therapy third patient was at home through briefing. After three months of therapy, I evaluated the effectiveness of exercise based on the input and output tests, balance tests and examinations on posturograph. I also took into account the patients' subjective feelings. Physiotherapy had an impact primarily on the breathing stereotype and better postural stability when standing. Furthermore, the psychological state of patients also improved. Physiotherapy may interfere with some clinical symptoms of MS. It helps to improve the quality of life, improve self-sufficiency and engage individuals in active life. However, we have to expect short-term effect and that it is necessary to rehabilitate all the time. Also, we must continue to adapt to the development of the disease and the current condition of the patient.
Comparison of physiotherapy techniques after conventional and mini-invasive surgical myocardial revascularization
JÍRA, Pavel
This thesis is focused on surgical therapy of coronary artery disease and its following physiotherapy in acute state. The main task is to find the right form of physiotherapy and check its effectiveness during the early postoperative period. I compare two different surgical intervention the mini-invasive one and the conventional technique method of surgical myocardial revascularization. The thesis is divided into theoretical and practical part. In theoretical part is described the origin and the etiology of coronary artery disease, its therapy - particularly the surgical therapy and its following physiotherapy. This part is created by research of sources enumerated at the end of the thesis. In practical part are described the possibilities of physiotherapy and the progress of patients after the operations. It was created the special list of exercises. Exercises was aimed at breathing techniques and vascular gymnastics, improving of physical condition by light special exercises, practising of verticalizing and self-reliance. The exercises had to solve the muscles disbalances without interrupting the process of recovering the postoperative scar. Special exercises was the part of the healing process. For the first time patients were examined, then they were well educated about helping techniques and special precaution. Patients were examined again before leaving the cardiothoracic ward and the acquired values were compared in all observed aspects. Physical condition of all patient were improved, it has a huge influence on disbalances in shoulder girdle and muscles of thorax and scapula. The outcome of the research is the finding that the proper activity helps with process of recovering after the coronary revascularization.
Possibilities of physiotherapy in patients with hypermobility
BÜRGEROVÁ, Gabriela
This bachelor´s thesis deals with the issue of joint hypermobility. Hypermobility does not constitute a disease in the true sense, but a certain level of ligament quality. It is manifested by increased range of movements in joints over the physiological limit. Above all, it should be noted that especially the ligament fulfills the strengthening and fixing function of the musculoskeletal system. In cases of hypermobility there is increased laxity of ligaments and looseness of joint capsules, moreover, it is associated with low muscle tone. As a consequence of the insufficiency of ligament, hypermobility is accompanied with joint instability, which leads to the development of functional disorders. Therefore, physiotherapy constitutes a crucial role in the compensation of the hypermobility signs in order to avoid that functional disorders grow into structural ones. The aim of this work was mainly to explain the problems of hypermobility and to propose an appropriate physiotherapeutical treatment of hypermobile individuals. In the theoretical part of this work, I define the musculoskeletal system focused on the description of ligament and joints. It is followed by the explanation of functional disorders and their relationship with structural diseases. Then, I continue with a characteristics of hypermobility and the associated joint hypermobility syndrome, which I subsequently elaborate with the description of epidemiology, etiopathogenesis and clinical signs. Later on, I present the classification of hypermobility and diagnostic tests with a broader description of the most commonly used ones in the Czech Republic and abroad. In the final part, I come to the differential diagnosis and the characteristics of the treatment of hypermobile individuals. In the practical part of my thesis, I did a qualitative research. The subjects of my research were two adult men with confirmed hypermobility. I took the anamnesis of the two probands and performed an initital check-up including the examination of hypermobility according to Janda and also using goniometry. Based on the examination, I proposed a therapy under my leadership for the period of 2-3 months with 60 minutes sessions once a week. Probands were recommended to do exercise for about 15-20 minutes every day. For one of the probands we planned a total of 10 sessions. After the completion of the therapy, we conducted an output assessment and made a conclusion that the overall physical condition had improved, but due to the length of the therapy we failed to influence the hypermobility itself because it was impossible to influence the quality of the ligament in such a short time. Compensation of hypermobility is a question of a very long-term exercise. Unfortunately, we had to finish the therapy of the second proband before our seventh session because of sudden health complications. That was the reason why we did not reach any positive results. However, this case would also require a long period of time to compensate the hypermobility.

Possibilities and importance of physiotherapy in diabetic polyneuropathy
ŠTÍCHOVÁ, Kateřina
This bachelor thesis focuses on issues in diabetic polyneuropathy. Diabetic polyneuropathy is one of the most frequent chronical complications in internal disease diabetes mellitus. The point is that the functions of peripheral nerves are being destroyed sensitive, motoric, even the autonomous ones. The destruction of these nerves is based on longstanding hyperglycaemia. Due to the variability of levels and localizations of afflictions, polyneuropathy is characterized by a huge amount of clinical symptoms pain, paraesthesia, amyosthenia, paresis etc. This disease is always very uncomfortable for a patient and it can be the cause of invalidity in the future. The main objective of the thesis is to bring fundamental theoretic information about diabetes mellitus and diabetic polyneuropathy and notify about possibilities of physiotherapy in therapy and prevention. To reach the research objective, I get some of physiotherapeutic methods to practice and then I evaluate the effectivity of therapy. First two chapters of the theoretical part describe diabetes mellitus and diabetic polyneuropathy in general. They contain a brief anatomy and basic information of these diseases. Chapter three comprises investigations that were used in diagnostics of the diabetic polyneuropathy. Fourth part informs about overall therapy and at the end the fifth is about possibilities of physiotherapy within a frame of polyneuropathy. In the experimental section of thesis, a qualitative form of research was chosen. It was performed by six patients, came from the Internal Practice of MUDr. Jitka Brádlerová, Aurora Spa, Třeboň. By the five of them, the diabetic polyneuropathy was actually diagnosed and by the last one it is in a phase of suspicion. Each of the patients' case report brings detailed anamnesis, kinesiology analyse, basic neurological examinations and two versions of the rehabilitation plan short and long-term one. There is subjective and objective evaluation of therapy's effectivity in the chapter seven, that is also annotated in discussion and conclusion of thesis. All the patients acknowledged an improvement in their status and that proclaims the therapy was selected properly. Bachelor theses can be useful as a comprehensive summary of information about diabetes mellitus and diabetic polyneuropathy, physiotherapeutic methods and their importance in the therapy of these diseases for physiotherapy students, physiotherapists, other medical staff even the laic public.
The influence of sensorimotor stimulation induced by specialized tools and pshysioterapeutical techniques on the function of the foot of middle childhood aged children
VOTAVOVÁ, Eva
This bachelor thesis addresses the function issues of children's foot and the possible ways of influencing it by using sensomotoric stimulation. The foot is an important organ, which has multiple functions. Locomotion is one of the primary functions. The ability to adapt to different types of surfaces is one of the other functions. The gait on different terrains is natural for the foot. Alas, under the influence of the modern age, we the deviate from nature and lock our feet into misshapen footwear. Last but not least it is the sensitive component, which allows the motor system to respond appropriately to stimuli. The name of this phenomenon is sensorimotor coordination. Its base principle is the processing of data sent from the proprioceptors embedded in the body structures. The central nervous system processes the information and responds in an organism-specific way. It is an essential requirement for physiological gait. Basic information allowing the understanding of the given topic is stated in the theoretical part, for example the anatomy and structure of the locomotion system in the lower extremity area, biomechanics of gait included. A great deal of attention is paid to sensomotoric stimulation and its possible usage. The practical part was made in the form of a qualitative research. The research poll consists of four younger school-aged subjects, who took part in a 3-month therapy. The results were obtained by comparing the pre-therapy examination and post-therapy examination. The aim of the bachelor thesis was to conduct a survey about sensomotoric stimulation and to elaborate how it can impact functions of the foot. The results show, that it can have a positive impact on subjects with signs of flat feet. The muscles of flat feet are flaccid and therefore the sensomotoric stimulation is a suitable therapy for this condition. It activates the main leg joints stabilizers and the short muscles of the arches. The results are not as convincing in the case of a high arch, nevertheless even this condition has shown improvement. The muscles of a foot with a high arch have higher tension and in many cases, structural changes are present. These changes can be removed only by operation, e.g. pes cavus. Nonetheless if the therapy is undertaken at an early age and with a proper intensity, a positive effect can be achieved. Sensomotoric stimulation should be a part of natural development for the human body. It ensures the correct and efficient integrating of muscle units during locomotion.
