Duktální adenokarcinom pankreatu (PDAC) je vysoce agresivní zhoubné onemocnění s vysokou mortalitou. Jeho pětiletá míra přežití je jedna z nejnižších mezi maligními chorobami zejména kvůli diagnóze v pozdních stádiích onemocnění a omezené účinnosti dostupné léčby. Nádorové mikroprostředí PDAC, tvořené nádorovými, stromálními a imunitními buňkami a jimi secernovanými molekulami, má vysoce imunosupresivní charakter a zprostředkovává mechanismy rezistence nádoru vůči chemoterapii a imunoterapii. Mezi dostupné léčebné modality PDAC patří převážně chirurgická resekce a systémová chemoterapie, a to buď ve formě kombinovaného režimu FOLFIRINOX, kombinace gemcitabinu s nab-paklitaxelem, nebo jako monoterapie gemcitabinem. Chemoterapie však nevede k výrazné odezvě a pouze mírně prodlužuje život pacientů trpících PDAC. Protinádorová imunoterapie v posledních letech zaznamenala významný pokrok a stala se konvenční léčbou řady malignit. V kontextu léčby PDAC se však imunoterapie potkává s nízkou odpovídavostí a omezenou účinností. V současnosti probíhá intenzivní vývoj inovativních terapeutických postupů, které si kladou za cíl obejít mechanismy rezistence PDAC k chemoterapii a imunoterapii a zvýšit jejich léčebný potenciál. Klíčová slova: pankreatický karcinom, chemoterapie, imunoterapie, nádorové... Pancreatic ductal adenocarcinoma (PDAC) is a highly aggressive malignant disease with a high mortality rate. Its five-year survival rate is among the lowest of all malignancies, primarily due to diagnosis at advanced stages of the disease and the limited efficacy of available treatments. The tumor microenvironment of PDAC, composed of tumor, stromal, and immune cells along with the molecules they secrete, has a highly immunosuppressive nature and mediates mechanisms of resistance to both chemotherapy and immunotherapy. The main therapeutic modalities available for PDAC include surgical resection and systemic chemotherapy, administered either as the combined FOLFIRINOX regimen, a combination of gemcitabine and nab-paclitaxel, or as gemcitabine monotherapy. However, chemotherapy generally does not elicit a significant response and only modestly prolongs the survival of patients with PDAC. Cancer immunotherapy has made substantial progress in recent years and has become a standard treatment for several malignancies. In the context of PDAC, however, immunotherapy is associated with low responsiveness and limited efficacy.
