[cze][eng] Tato bakalářská práce se zabývá četností výskytu jednotlivých bakteriálních ESBL (Extent-Spectrum-Beta-Lactamases) kmenů ve vzorcích moči, které byly vyšetřovány v laboratoři Unilabs na adrese Antonína Barcala 404, 370 01 České Budějovice (dále jen laboratoř Unilabs) za rok 2023. ESBL jsou enzymy produkované bakteriemi díky kterým mohou rozkládat široké spektrum beta-laktamových antibiotik. Práce se zaměřuje především na infekce močových cest a na jednotlivé původce těchto onemocněních, zabývá se teoretickým základem tedy etiologií, příznaky, průběhem i léčbou. Dále se v teoretické částí práce zabývá podstatou antibiotické rezistence včetně výskytu ESBL kmenů. Tyto poznatky jsou důležité pro monitorování růstu antibiotické rezistence, která je za poslední léta stále na vzestupu. Cíle této studie byly konstruovány jak na sběr dat tak i na laboratorní praxi zaměřenou na průkaz neznámých bakteriálních kmenů ve vzorcích moči. Data pro statistické vyhodnocení byla anonymizována a získávána ručním přepisem z laboratorního informačního systému Envis. Za důležitá data pro cíle a hypotézy této bakalářské práce byla považována následující > věková skupina, pohlaví, typ zdravotnického zařízení odkud byl vzorek poslán, typ odběru moči, typ diagnózy, porovnání zda se jedná o monoinfekci či koinfekci a o bakteriální etiologii koinfekce. Výsledky byly zpracovány do četnostních grafů a byl využit chi-kvadrát test. Pro tuto práci byly stanoveny dvě hypotézy > Nejčastěji zastoupeným patogenem produkující ESBL bude Escherichia coli. Escherichia coli, Klebsiella pneumoniae, Proteus Mirabilis, Citrobacter spp. budou nejvíce spojeny s diagnózou N30.0 tedy akutní cystitidou. Z výsledků této práce vyplývá, že nejčastějším ESBL produkujícím patogenem byla Escherichia coli, infekce vyvolaná touto bakterií nejvíce postihovala ženy a osoby starší 61 let. Všechny zkoumané ESBL kmeny byly vázány na různé diagnózy a nebyla potvrzena druhá hypotéza tzn. že akutní cystitida nebyla nejčastější diagnózou pro každý jednotlivý patogen. Většina vzorků byla zaslána z ordinacích praktického lékaře a nejvíce volený způsob odběru byl odběr středního proudu moči. Poznatky získané v této bakalářské práci lze použití jako zdroj informací pro další výzkumné účely. This bachelor thesis deals with the frequency of occurrence of individual bacterial ESBL (Extent-Spectrum-Beta-Lactamases) strains in urine samples that were examined in the Unilabs laboratory at Antonína Barcala 404, 370 01 České Budějovice (hereinafter referred to as Unilabs laboratory) for the year 2023. ESBLs are enzymes produced by bacteria that can degrade a wide range of beta-lactam antibiotics. The thesis focuses mainly on urinary tract infections and the individual causative agents of these diseases and deals with the theoretical basis, i.e. etiology, symptoms, course and treatment. Furthermore, the theoretical part of the thesis deals with the nature of antibiotic resistance including the occurrence of ESBL strains. These findings are important for monitoring the growth of antibiotic resistance, which has been on the rise in recent years. The objectives of this study were constructed for both data collection and laboratory practice aimed at the detection of unknown bacterial strains in urine samples. Data for statistical evaluation were anonymized and obtained by manual transcription of data from a laboratory information systém Envis. The following data were considered important for the objectives and hypotheses of this bachelor thesis > age, group, gender, type of health facility from where the specimen was sent, type of urine collection, type of diagnosis, comparison of whether the specimen was monoinfection or coinfection, and the bacterial etiology of the coinfection. The results were processed into frequency plots and chi-square test was used. Two hypotheses were established for this work > Escherichia coli will be the most commonly represented ESBL producing pathogen. Escherichia coli, Klebsiella pneumoniae, Proteus Mirabilis, Citrobacter spp. will be most associated with the diagnosis of N30.0 i.e. acute cystitis. The results of this study show that the most common ESBL-producing pathogen was Escherichia coli, infection caused by this bacterium mostly affected women and people over 61 years of age. All ESBL strains studied were associated with different diagnoses and the second hypothesis, i.e. that acute cystitis was not the most common diagnosis for each individual pathogen, was not confirmed. The majority of samples were sent from GP surgeries and the most chosen method of collection was midstream urine collection. The knowledge gained in this undergraduate thesis can be used to inform further research.
