Tato disertační práce se zabývá rolí endoteliálního glykokalyxu (EG) při transplantacích ledvin, s důrazem na jeho význam pro udržování mikrocirkulační homeostázy a integritu endoteliální bariéry. Narušení EG, obzvláště při ischemicko-reperfuzním poškození během transplantace, vede k mikrovaskulárnímu poškození. Cílem práce je prozkoumat možnosti ochrany EG a identifikovat potenciální biomarkery, jako je syndekan-1, které by mohly sloužit k monitorování funkce štěpů a predikci komplikací po transplantaci. Práce zahrnuje tři studie: První studie prokázala korelaci mezi hladinami syndekanu-1 u zemřelých dárců ledvin a sérovým kreatininem, a také vyšší hladiny syndekanu-1 u dárců s akutním poškozením ledvin. Druhá studie zkoumala efekt sulodexidu a albuminu na ochranu EG po transplantaci ledvin, přičemž výsledky naznačily potenciální ochranný účinek těchto látek na prasečím modelu. Třetí studie analyzovala hladiny syndekanu-1 a dalších markerů u příjemců ledvin po transplantaci, s výsledky ukazujícími na mírnou korelaci mezi syndekanem- 1 a hladinami kreatininu, a významné rozdíly v hladinách syndekanu-1 u pacientů s opožděným nástupem funkce ledviny. Práce přispívá k hlubšímu porozumění úlohy EG v kontextu transplantace ledvin. This dissertation focuses on the role of the endothelial glycocalyx (EG) in kidney transplantation, emphasizing its importance in maintaining microcirculatory homeostasis and the integrity of the endothelial barrier. Disruption of the EG, particularly during ischemia- reperfusion injury associated with transplantation, leads to microvascular damage. The aim of the work is to explore potential strategies for protecting the EG and to identify biomarkers, such as syndecan-1, that could be used to monitor graft function and predict post-transplant complications. The dissertation includes three studies: The first study demonstrated a correlation between syndecan-1 levels in deceased kidney donors and serum creatinine, as well as higher syndecan- 1 levels in donors with acute kidney injury. The second study investigated the effects of sulodexide and albumin on EG protection after kidney transplantation, with results suggesting a potential protective effect of these substances in a pig model. The third study analyzed syndecan-1 and other markers in kidney transplant recipients, with findings showing a mild correlation between syndecan-1 and creatinine levels, and significant differences in syndecan- 1 levels in patients with delayed graft function.
