Míšní léze mohou zasáhnout motorické, senzitivní i autonomní funkce. Největší skupinu míšních lézí tvoří krční míšní léze. U těchto pacientů je patrné zhoršení dechového stereotypu a hygieny dýchacích cest z důvodu paralýzy respiračních svalů. Z tohoto důvodu jsou nepřetržitě ohroženi respiračními komplikacemi, které jsou též nejčastější příčinou úmrtí u pacientů s krční míšní lézí v chronické fázi. V teoretické části jsou uvedeny obecné poznatky o míšních lézích, dále zejména respirační komplikace, varianty respirační fyzioterapie a možnosti využití odporových dechových trenažerů a mechanické insuflace-exsuflace při terapii. Praktická část se zabývá zmapováním četnosti respiračních infektů a způsoby ovlivnění respiračních funkcí prostřednictvím dechových trenažerů (Threshold PEP, POWERbreathe) a přístroje CoughAssist. Součástí vyšetření je měření vybraných antropometrických a spirometrických parametrů, využití standardizovaných dotazníků (Pulmonary function basic data set - ISCOS) a sledování subjektivního vnímání stavu probandů. Tato práce si klade za cíl zaznamenat četnost infektů a možnosti ovlivnění plicních funkcí u chronických pacientů s krční míšní lézí. Rádi bychom také prokázali, že využití CoughAssistu u chronických pacientů má větší efekt než standardní terapie pomocí odporových... Spinal cord injuries can affect motor, sensory, and autonomic functions. The largest group of spinal cord injuries are cervical spinal cord injuries. Patients with these injuries can suffer from deterioration in breathing patterns and airway clearance techniques due to the paralysis of respiratory muscles. For this reason, they are continuously at risk of respiratory complications, which are also the most common cause of death in those with chronic cervical spinal cord injuries. The theoretical section presents general knowledge about spinal cord injuries, with a particular focus on respiratory complications, variants of respiratory physiotherapy, and the possibilities of using resistance breathing exercisers and mechanical insufflation-exsufflation in therapy. The research section focuses on mapping the frequency of respiratory infections and methods of influencing respiratory functions through the use of breathing exercisers (Threshold PEP, POWERbreathe) and the CoughAssist device. The examination includes the measurement of selected anthropometric and spirometric parameters, the use of standardized questionnaires (Pulmonary Function Basic Data Set - ISCOS), and the monitoring of the participants' subjective perception of their condition.
