[cze][eng] Současný stav. Kolorektální karcinom je jednou z nejčastějších onkologických diagnóz a jeho incidence setrvale narůstá. Podle nejnovějších dat z roku 2018 zaujímá česká populace mužů v Evropě 14. místo a ženy zaujímají 19. nejvyšší pozici. Ročně je v České republice nově diagnostikováno kolem 7700 pacientů a přibližně 3400 pacientů na něj umírá. Závažným problémem je, že stále velké procento nádorů je primárně zachyceno v pokročilém stádiu. Cíle práce. Hlavními cíli bylo zmapovat informovanost dospělé populace starší 50 let v Libereckém kraji v oblasti prevence kolorektálního karcinomu, identifikovat faktory ovlivňující úspěšnost screeningu kolorektálního karcinomu a zjistit roli sester v oblasti prevence kolorektálního karcinomu v ordinaci praktického lékaře. Metodika. Výzkumná část byla zpracována kombinovanou kvantitativně-kvalitativní formou. Ke sběru dat byl použit nestandardizovaný dotazník o 29 otázkách. Výzkumný soubor byl záměrný a zahrnoval muže i ženy Libereckého kraje, ve věkové kategorii nad 50 let, kteří neabsolvovali koloskopii. Celkem se kvantitativního výzkumného šetření účastnilo 160 respondentů. Kvalitativní výzkumné šetření probíhalo na základě předem domluvených polostrukturovaných rozhovorů s 10 sestrami pracujícími v ordinaci praktického lékaře. Podkladem bylo 7 otázek, kterými jsme zjišťovaly roli sester v oblasti prevence kolorektálního karcinomu v ordinaci praktického lékaře. Výsledky a závěr. Z celkových výsledků můžeme soudit, že informovanost dotazované dospělé populace v Libereckém kraji v prevenci kolorektálního karcinomu je celkem na dobré úrovni. Ukázalo se, že míra informovanosti u dotazované dospělé populace v oblasti kolorektálního karcinomu statisticky významně závisí na vzdělání, ale není statisticky závislá ani na pohlaví, věku a ani na výskytu nádorového či zánětlivého onemocnění v rodině. Hlavním zdrojem informací jsou rodina, přátelé a internet. Mezi faktory ovlivňující úspěšnost screeningu kolorektálního karcinomu jsou úroveň vzdělání, informovanost o kolorektálním karcinomu a o rizicích jeho vzniku, informovanost o screeningu, a negativní postoje jako jsou rozpaky, nezájem, strach z rakoviny nebo screeningu, nedostatek času a pocit zdraví. Dalším zjištěním bylo, že většina sester je aktivní v prevenci kolorektálního karcinomu. Pacienty si na preventivní prohlídky samy zvou a informují je, jaká vyšetření jsou v konkrétním věku doporučená. Všechny sestry poskytují svým pacientům test na okultní krvácení ve stolici, jakmile dosáhnou příslušného věku pro screening, s vysokou návratností. Dále se ukázalo, že mnohé sestry používají kombinaci ústního poučení a poskytování informačních letáků k použití testu na okultní krvácení ve stolici. Bohužel téměř není pacientům nabídnuta volba mezi testem na okultní krvácení ve stolici a koloskopií, a v čekárně u praktického lékaře nemají k dispozici žádné informační letáky o prevenci kolorektálního karcinomu. Všechny sestry pozorují u pacientů kladný postoj k prevenci kolorektálního karcinomu. Current state. Colorectal cancer is one of the most common cancer diagnoses and its incidence has been steadily increasing. According to the most recent data from 2018, the Czech the male population in Europe ranks 14th and women rank 19th highest. Annually, there are about 7700 patients newly diagnosed and around 3400 patients are dying from colorectal cancer in the Czech Republic. Unfortunately, there is still a large percentage of this tumours diagnosed in an advanced stage. Aims. The main objectives were to map the awareness of the adult population over 50 years of age in the Liberec Region regarding colorectal cancer prevention, to identify the factors influencing the success rate of colorectal cancer screening and to determine the role of nurses in the field of colorectal cancer prevention in a general practitioner's office. Methods. The research part was elaborated by a combined quantitative-qualitative method. A non-standardized questionnaire comprising 29 questions was used to collect data. The exploratory cohort of participants included both men and women from the Liberec Region in the age category over 50 years who have not undergone a colonoscopy. A total of 160 respondents participated in the survey. The qualitative research was carried out using pre-arranged semi-structured interviews with 10 nurses working in a general practitioner's office. This part was based on 7 questions that were used to assess the role of nurses in the field of colorectal cancer prevention in the general practitioner's office. Results and conclusion. From the overall results we can conclude that the awareness of the surveyed adult population in the Liberec Region in the prevention of colorectal cancer is quite at a decent level. It has been shown that the level of awareness of the surveyed adult population in the field of colorectal cancer is statistically significantly dependent on the level of education, but it is not statistically dependent on gender, age or the occurrence of cancer or inflammatory bowel disease in the family. The main sources of information identified were family, friends and the Internet. Factors influencing the success of colorectal cancer screening included level of education, awareness of colorectal cancer risks, awareness of screening and negative attitudes such as embarrassment, lack of interest, fear of cancer or cancer screening, and lack of time or feeling healthy. Another finding was that most nurses are active in the prevention of colorectal cancer. They invite patients for preventive check-ups and inform them about what examinations are recommended at a given age. Once patients reach the appropriate age, nurses will provide them with a faecal occult blood test with a high return rate. In addition, it has been shown that many nurses use a combination of oral instruction and the provision of information leaflets to instruct them on the use of the faecal occult blood test. Unfortunately, patients are hardly offered a choice between a faecal occult blood test and a colonoscopy, and there are no information leaflets on colorectal cancer prevention available in the General Practitioners waiting room. All nurses observe a positive attitude towards the prevention of colorectal cancer in patients.
