[cze][eng] Hlavním cílem diplomové práce je mapovat management péče o dárce orgánů s prokázanou mozkovou smrtí (neboli bijícím srdcem) a o dárce po nevratné zástavě oběhu (neboli nebijícím srdcem), a rozdílu péče mezi nimi. Především to, jestli sestry při péči o dárce orgánů pracují prostřednictvím vypracovaného standardu. V teoretické části diplomové práce jsou popsány základní informace o transplantační medicíně, její historie, zahraniční i česká, legislativa dárcovství orgánů, transplantační centra, statistiky transplantací v České republice a spolupráce České republiky se zahraničními státy při transplantaci. Další kapitoly teoretické práce jsou věnovány dárcovství orgánů po nevratné smrti mozku a po zástavě oběhu. V těchto kapitolách je podrobněji popsáno stanovení diagnózy při mozkové smrti a zástavě oběhu, členění dárců a péče o tyto dárce po stanovení diagnózy. Součástí teoretické části diplomové práce jsou možné odběrové metody při transplantaci orgánů. Pro dosazení cílů bylo použito kvalitativní výzkumné šetření, které bylo rozděleno do dvou výzkumných částí, a to na sběr dat o dárcích s mozkovou smrtí a dárcích se zástavou oběhu. V první výzkumné části bylo osloveno 8 informantů z anesteziologicko-resuscitačních oddělení ve 4 různých zdravotnických institucích. Informanti museli mít bohatou zkušenost s péčí o dárce s mozkovou smrtí. V druhé výzkumné části byli dotazováni 4 informanti, kteří mají zkušenost s dárcem orgánů po nevratné zástavě oběhu. Pro sběr dat byly vytvořeny 2 anonymní polostrukturované rozhovory. K analýze dat byla použita metoda otevřeného kódování s technikou tužka papír. Z rozboru dat o dárcích se zástavou oběhu jsme získali přehled o procesu dárcovství. Zároveň jsme se ujistili, že dárcovství po nevratné zástavě oběhu je méně časté a náročné pro všechen zdravotnický personál. Po zpracování dat o dárcích s mozkovou smrtí jsme dosáhli uceleného přehledu péče o dárce v jednotlivých zdravotnických institucích. Pouze 2 ze zdravotnických zařízení zahrnuté do výzkumného šetření, pracují při péči o dárce orgánů podle vyhotoveného manuálu/standardu. Proto je výstupem práce doporučení vytvoření manuálu nebo ošetřovatelského standardu v jednotlivých institucích pro ucelení péče o dárce orgánů. The main goal of this thesis is to map the care management of organ donors after proven brain death (heart-beating donors) and donors after irreversible circulatory arrest (non-heart-beating donors), and the difference in the care between them. Above all, to find out whether nurses work according to a developed standard when taking care of organ donors. The theoretical part of the thesis provides basic information on transplantation medicine, its history, both foreign and Czech, organ donation legislation, transplantation centres, statistics of transplantations in the Czech Republic and cooperation of the Czech Republic with foreign countries in the field of transplantations. Other chapters of the theoretical part are devoted to organ donation after irreversible brain death or after circulatory arrest. Setting the diagnosis of brain death or circulatory arrest, classification of donors and the care of these donors after the diagnosis are described in more detail in these chapters. The theoretical part of the thesis includes possible methods of organ retrieval. A qualitative research survey, which was divided into two research parts,was used to achieve the goals, namely data collection on donors after brain death and donors after cardiac death. In the first research part of the thesis, 8 informants from departments of anaesthesiology and resuscitation in 4 different healthcare institutions were interviewed. The prerequisite was that the informants had extensive experience in the care of brain-dead donors. In the second research part, 4 informants were interviewed, who had experience with an organ donor after irreversible circulatory arrest. 2 anonymous semi-structured interviews were conducted for data collection. The open coding method with pencil and paper technique was used to analyse the data. The analysis of the data on cardiac death donors helped us to gain insight into the donation process. At the same time we verified that organ donation after irreversible circulatory arrest is less frequent and challenging for all medical staff. After processing the data on brain-dead donors, we achieved a comprehensive overview of donor care in individual medical institutions. Only 2 of the healthcare facilities included in the research survey work in the care of organ donors according to a prepared manual/standard. Therefore, the outcome of the thesis is the recommendation to create a manual or nursing standard in individual institutions for comprehensive care of organ donors.
