1 Abstrakt Arteriální hypertenze je nejčastějším kardiovaskulárním onemocněním zodpovědným za řadu orgánových komplikací. Etiologicky se nejčastěji jedná o esenciální hypertenzi, u které je patogeneze multifaktoriální a v rozvoji onemocnění se uplatňuje kombinace genetické výbavy jedince s vnějšími vlivy. Její výskyt strmě narůstá s věkem. Naopak u relativně vzácných monogenně podmíněných chorob, projevujících se arteriální hypertenzí, stačí bodová mutace konkrétního genu a výsledkem může být těžké onemocnění manifestující se od dětství. Jednou z takových nemocí je Liddleův syndrom. Jde o dědičnou formu hypertenze způsobenou mutací genu pro epiteliální sodíkový kanál. Tento kanál je zodpovědný za resorpci sodných iontů v distální části nefronu. Vlivem mutace dochází k nadměrné aktivitě kanálů, zvýšené resorpci sodíku a potažmo vody, což vede k expanzi intravaskulární tekutiny rezultující v hypertenzi. Spolu s vysokým krevním tlakem jsou u pacientů s Liddleovo syndromem přítomny variabilně také odchylky vnitřního prostředí jako hypokalémie a nízká hladina aldosteronu v séru. Mohou být tak cennými vodítky při diagnostice, neboť spolu s pozitivní rodinnou anamnézou hypertenze mohou lékaře navést k úvaze o možné sekundární či dědičné formě hypertenze u pacienta. Správná diagnóza umožní zvolit cílenou terapii,... Arterial hypertension is the most prevalent cardiovascular dissease which can lead to various end organ damage. It is most commonly a primary diagnosis or so called essential hypertension with a multifactorial pathogenesis combining genetic and enviromental factors. Its prevalence is increasing with age. On the other hand there are some relatively rare monogenic forms of arterial hypertension, which are caused by a single point mutation of particular gene. These diseases can manifest in childhood and one of them is Liddle syndrome. It is a heritable form of arterial hypertension caused by mutation in gene coding of a epithelial sodium channel. This channel is responsible for sodium reabsorption in distal parts of nephrone in kidney. Mutated channel is overactive which leads to increased retention of sodium and therefore water, resulting in exapansion of intravascular volume and hypertension. Along with high blood pressure hypokalaemia and low serum aldosteron occurs is some patients as well. These disturbances can be useful leads in work-up and together with positive family history of high blood pressure may guide a physician to start thinking about more rare forms of hypertension.
