Prognostický význam molekulárních alterací u dětských gliomů centrálního nervového systému Abstrakt Tato disertační práce se zabývá komplexní analýzou kohorty dětských pacientů léčených pro gliomy nízkého stupně malignity (pLGG) v letech 1999 - 2021 na Klinice dětské hematologie a onkologie 2. lékařské fakulty Univerzity Karlovy a Fakultní nemocnice v Motole. Cílem práce bylo popsat molekulárně genetické pozadí vybrané kohorty pacientů a korelovat je s klinickým obrazem a průběhem onemocnění. Práce zdůrazňuje význam diagnózy jako syntézy vhledu jednotlivých oborů a podtrhuje důležitost integrace molekulárně genetického poznání do klinické praxe. Byla provedena detailní analýza genetických alterací a jejich distribuce v závislosti na anatomických lokalizacích. Výsledky ukázaly, že většina nádorů nesla fúzi KIAA1549::BRAF nebo mutaci BRAF V600E, přičemž byly identifikovány i jiné genetické změny včetně alterací v genech FGFR, NTRK, RAF, ROS a dalších. Výzkum přinesl důležité poznatky o molekulárně genetickém pozadí pLGG a jeho vlivu na klinický průběh a léčbu. Ukázal, že integrace molekulárně genetického vyšetření do diagnostického procesu je klíčová pro optimalizaci léčebných strategií. The prognostic significance of molecular alterations in pediatric gliomas of central nervous system Abstract The purpose of this dissertation is to delve into the molecular and genetic underpinnings of low-grade gliomas in children, focusing on a cohort treated over the past two decades. It aims to correlate these findings with the clinical course and outcomes of the disease. The study was conducted through a collaborative effort between the Clinic of Pediatric Hematology and Oncology and the Laboratory of Molecular Pathology at the Charles University, highlighting the significance of a multidisciplinary approach in diagnosing. This work highlights the importance of diagnosis as a synthesis of insights from various disciplines, underscoring the significance of molecular-genetic factors in the prognosis and treatment of low-grade gliomas in children. Through a detailed study involving methods such as PCR, Sanger sequencing, RNA panel sequencing, MLPA, and DNA methylation profiling, key genetic alterations were identified, including frequent BRAF fusions and mutations, changes in FGFR genes, and alterations in MYB/MYBL genes, among others.
