Souhrn Ischemická choroba dolních končetin (ICHDK) postihuje ve vyspělých zemích více jak pětinu populace nad 65 let, zhoršuje kvalitu života, může vést ke ztrátě končetiny nebo až ze smrti nemocného. Byla popsána řada rizikových faktorů, které se překrývají s rizikovými faktory ischemické choroby srdeční (ICHS) a cévní mozkové příhody (CMP). Chronické formy klasifikujeme dle Fontainovy klasifikace. Diagnostika ICHDK vychází z klinického obrazu, fyzikálního vyšetření (včetně měření rozdílu krevního tlaku) a potvrzuji jí zobrazovací metody. Léčba ICHDK spočívá v ovlivnění rizikových faktorů, léčbě symptomů a revaskularizaci metodami intervenční radiologie nebo cévní chirurgie. Při chirurgické revaskularizaci je nejčastějším výkonem provedení femoropopliteálního bypassu. Konduitem může být jak žíla, tak i protéza. Preferovaným napojením na a. poplitea je end-to-side anastomóza, která nese riziko uzávěru na podkladě trombózy, intimální hyperplázie, nebo progrese aterosklerózy. Intimální hyperplázie je fyziologickou odpovědí cévní stěny na anomálie krevního proudu - turbulentní proudění, stagnace, oscilační proudění, recirkulace. Rozvoj intimální hyperplázie lze ovlivnit tvarováním anastomózy s ohledem na její hemodynamiku - především se jedná o úhel anastomózy, poměr šíře konduitu a cílové tepny, otazné je... Peripheral arterial disease (PAD) of the lower limbs affects more than a fifth of the population over the age of 65 in the developed countries. PAD decreases the quality of life. It can lead to the loss of the limb or even death. Various risk factors that overlap with the risk factors for CHD and CMP have been identified. Chronic forms of PAD are staged according to Fontain's classification. Diagnosis of PAD is based on the clinical presentation, physical examination (including measurement of blood pressure difference) and confirmed by imaging methods. Treatment of PAD consists of influencing risk factors, treating symptoms and revascularization. Femoropopliteal bypass is the most common surgical procedure performed for revascularization. The conduit can be either a vein or a prosthesis. The preferred connection to the popliteal artery is an end-to-side anastomosis. It carries the risk of occlusion due to thrombosis, intimal hyperplasia, or progression of atherosclerosis. Intimal hyperplasia is a physiological response of the vessel wall to abnormal blood flow - turbulent flow, stagnation, oscillating flow, recirculation.
