Cíl: Cílem této práce bylo zhodnotit vliv respirační fyzioterapie s pomůckou Threshold PEP® na dechové funkce u lidí hospitalizovaných po kardiochirurgickém výkonu s přístupem přes střední sternotomii. Metodika: Během hospitalizace v Nemocnici České Budějovice a.s. byly měřeny parametry periferní saturace kyslíku (SpO2), maximálního nádechového (MIP) a výdechového tlaku (MEP) a antropometrie hrudníku. Dále byl použit dotazník, kde probandi uváděli svůj subjektivní pocit zdraví, včetně hodnocení bolesti a dušnosti dle Borgovy škály. Dotazník zohlednil přítomnost drénů, kašle a přidružených onemocnění - astma, CHOPN či COVID-19. Výsledky: Experimentu se zúčastnilo 8 probandů. Metody měření se prokázaly jako vhodné pro využití v praxi. Výsledky neukázaly u experimentální skupiny statisticky významné změny v parametrech MIP (p=0,507), MEP (p=0,339), dušnosti (p=0,266) a pružnosti v oblasti mezosternale (p=0,076). Ke statisticky významné změně došlo u pružnosti v oblasti xiphosternale (p=0,0022) a u bolesti (p=0,003). U periferní saturace kyslíku došlo ke statisticky významné změně (p=0,014), která by se však nedala považovat za klinicky významnou. Závěr: Během studie došlo ke zlepšení dechových funkcí u pacientů v experimentální skupině s Threshold PEP®, nebyla to však změna statisticky významná. U... The main objective: The aim of this study was to evaluate the effect of respiratory physiotherapy with the use of Threshold PEP® device on respiratory function among people hospitalized after cardiac surgery with access via median sternotomy. Methods: During the hospitalization in the Nemocnice České Budějovice a.s. the following measurements were taken - peripheral oxygen saturation (SpO2), maximum inspiratory pressure (MIP), expiratory pressure (MEP) and chest anthropometry. In addition, a questionnaire was used where probands reported their subjective sense of health, including pain and dyspnoe ratings according to the Borg scale. The questionnaire took into account the presence of drainage, cough and associated diseases such as asthma, COPD or COVID-19. Results: The measurement methods proved to be suitable for practical use. The results showed no statistically significant changes in the experimental group in the parameters of MIP (p=0,507), MEP (p=0,339), dyspnoe (p=0,266) and mesosternal flexibility (p=0,076). There was a statistically significant change in xiphosternal flexibility (p=0,0022) and pain (p=0,003).
