Bakalářská práce se zabývá problematikou tendinopatií, konkrétně využitím tréninku opakovaných excentrických kontrakcí v terapii tendinopatie laterálního epikondylu humeru. Práce je rozdělena na několik částí. V první části jsou popsány anatomické struktury, biomechanika a kineziologie oblasti loketního kloubu a svalů souvisejících s touto diagnózou. Další část je věnována teoretickému přehledu a shrnutí aktuální literatury týkající se projevu tendinopatií obecně, možných mechanismů a patofyziologie vzniku, a nakonec konceptech využívaných při terapii tendinopatií. Následující část je už věnována přímo tendinopatii laterálního epikondylu humeru. Jsou zde shrnuty důležité informace a přehled znalostí o této diagnóze a poté zmíněné nejčastěji používané přístupy ve fyzioterapii a vědecky podložené výstupy z těchto přístupů. Praktickým cílem bakalářské práce byla výzkumná část se třemi účastníky s diagnostikovanou tendinopatií laterálního epikondylu humeru. Účastníkům byl vytvořen individuální tréninkový plán opakovaných excentrických kontrakcí a byli sledováni po dobu 12 týdnů. Hodnocení výzkumu probíhalo formou rozhovoru, vstupního a výstupního kineziologického rozboru a shromažďováním subjektivních dat, týdně zaznamenávaných do vizuální analogové škály bolesti. U všech 3 účastníku došlo po intervenci ke snížení udávané bolesti při zátěži i ke snížení palpační bolesti a bolesti u klinických testů při výstupním vyšetření. Pro potvrzení výsledků a ověření efektivity je třeba více rozsáhlejších studií. Pokud by výsledky návazných studií byly shodné, tato a další práce by mohly pomoci ke sjednocení a objektivizaci navrhovaných terapií a zároveň zjednodušení monitorování pacientů během terapie. This bachelor's thesis deals with the topic of tendinopathies, specifically the use of repeated eccentric contractions in the therapy of lateral elbow tendinopathy. The thesis is divided into a few sections. The anatomical structures, biomechanics and kinesiology of the elbow joint and the muscles associated with this diagnosis are described in the first section. The second part of the theoretical section is about current research and theoretical overview of tendinopathies as a whole. The pathophysiology and possible mechanisms of origins of tendinopathies and the concepts used within therapies. The last part of the theoretical section deals specifically with lateral elbow tendinopathy. Overview of the main knowledge concerning this diagnosis is summarized in this part and also the most prevalent methods of physiotherapy treatment and their evidence-based findings. The practical goal of this thesis involved a research group containing 3 participants with the later elbow tendinopathy diagnosis. The participants were given an individualized and structured training plan of repeated eccentric contractions for the period of 12 weeks. The methods of obtaining data were by an interview, entrance and output kinesiology analysis a by collecting subjective values recorded on the visual analogue pain scale. Each of the 3 participants recorded an improvement in all outcome measures pain during activity, pain upon palpation and pain during physical examination tests. There needs to be more extensive and follow-up studies to support these findings. If the follow-up studies show similar results, these papers could help unify and objectify the proposed therapeutic interventions and make the process of monitoring patients during rehabilitation easier.
