[cze][eng] Abstrakt Bakalářská práce se zabývá problematikou ošetřovatelské péče o onkologické pacienty v terminálním stadiu nemoci. Cíle bakalářské práce byly zaměřeny na zmapování specifik ošetřovatelské péče u onkologických pacientů v terminálním stadiu, zmapování specifik v komunikaci s těmito pacienty a dále na zhodnocení náročnosti péče o onkologické pacienty v terminálním stadiu. Metodika a výzkumný soubor K dosažení výsledků bylo využito metody dotazování, a to pomocí polostrukturovaných rozhovorů se sestrami pracujícími na onkologických odděleních standardního typu a také na jednotce intenzivní péče. Výzkumný soubor tvořilo jedenáct sester. Zpracování dat probíhalo pomocí otevřeného kódování, metodou tužka a papír. Analyzováním dat následně vznikly tři kategorie a osm podkategorií. Výsledky Z výsledků provedeného výzkumu vyplývá, že sestry řadí mezi hlavní specifika ošetřovatelské péče u onkologických pacientů v terminálním stadiu především adekvátní tlumení bolesti s cílem minimalizovat utrpení a co nejvíce zvýšit kvalitu života. Dále respektování pacientovy potřeby být v kontaktu s blízkými. Stran specifik komunikace bylo zjištěno, že sestry považují za stěžejní empatický přístup s nastavením lidské tváře a vyjádřením podpory v nelehkých chvílích. Dále důkladnou informovanost, kterou lze předcházet případným konfliktům či nedorozuměním. Mimo jiné bylo zjištěno, že sestry vnímají jako nejnáročnější bezmoc při umírání pacienta, který trpí. V tomto ohledu následně vyplývá, že sestry kladou důraz na oddělení pracovního a soukromého života, což lze považovat za preventivní opatření hrozícího syndromu vyhoření. Dle získaných informací sestry v paliativní péči naplňuje remise pacientova onemocnění nebo dopomoc při důstojném odchodu. Výsledky pro praxi Výsledky byly využity pro vytvoření scénáře simulační výuky pro studenty, a to v oblasti komunikace s onkologickými pacienty v terminálním stadiu nemoci. Bakalářská práce tak může poskytnout náhled na specifika péče o dané pacienty a přispět ke zvýšení povědomí o této problematice. Summary This bachelor diploma thesis deals with the issues of nursing care of oncology patients in the terminal phases of the disease. The goal of the thesis is to map specifics of nursing care of oncology patients in the terminal stage and specifics of communication with these patients. Furthermore, the goal of this thesis is to evaluate how demanding it is to nurse oncology patients in the terminal stage. Methodology and research group The results were achieved by the method of questioning via semi-structured dialogues with nurses working at standard oncology departments as well as at intensive care units. The research group consisted of eleven nurses. The data was processed via open coding by method of pen and paper. The analysis of the data subsequently created three categories and eight subcategories. Results The results of the research imply that according to the nurses the most important specific of nursing care of oncology patients in terminal stage is adequate usage of painkilling medication to minimize the suffering and improving the quality of life. The next important specific is the need of patient to be in touch with his or her close people. From the communicating point of view, the research concluded that the nurses consider empathy and friendly face as key elements. Expressing support in difficult times is also very important. The nurses perceive profound awareness as the next key element to avoid possible conflicts or misunderstanding. Among other things, the research declared that the most challenging part for the nurses is the feeling of being powerless while a suffering patient is dying. Therefore, the nurses emphasize the need to separate private and working life. This separation could be understood as a precaution against burnout syndrome that is threating. The acquired data demonstrates that the nurses working in palliative care feel fulfilled when the patient's disease is in remission or when they can assist dignified passing. Results for practice The results of the research were used to create script of simulative teaching for students in the field of communication with oncology patients in terminal stage of the disease.The hope is that this diploma thesis may provide insight into specifics of nursing care of the patients in question, and it may contribute to raise awareness of this field.
