Úvod: Trombofilní stavy mohou být provázeny zvýšenou generací trombinu, zvláště pak v těhotenství, kdy nárůst generace trombinu vyplývá z řady změn v hemostatickém systému. Při podávání antikoagulační léčby je žádoucí nárůst generace trombinu snížit. Cíle studie: Jedním z cílů naší studie bylo porovnat generaci trombinu u jedinců s prokázanou mutací faktoru V Leiden a jedinců se zvýšenou hladinou faktoru VIII. Dalším úkolem bylo porovnat pacienty s VTE s prokázanou mutací faktoru V Leiden (nebo bez ní) a užívající rozdílnou antikoagulační léčbu. Posledním cílem bylo posoudit generaci trombinu u těhotných žen a jejich odpovědi na podávání LMWH při závažném trombofilním stavu. Soubor pacientů a metody: Do kontrolní skupiny bylo zařazeno 170 zdravých dárců krve, do skupiny vrozené a získané trombofilie bylo zahrnuto 44 jedinců s mutací faktoru V Leiden a 38 jedinců s hladinou faktoru VIII >150 % bez další prokázané trombofilie. Do skupiny s antikoagulační léčbou bylo zařazeno 347 jedinců s VTE užívajících přímé inhibitory koagulace. Do skupiny těhotných žen bylo zařazeno 60 žen s profylaktickou dávkou LMWH. Pro sledování generace trombinu jsme použili kit Technothrombin® TGA RC Low a RC High a analyzátor Ceveron® Alpha. Výsledky: Zvýšená generace trombinu byla nalezena u skupiny s vysokým faktorem... Introduction: Thrombophilic conditions may be accompanied by increased thrombin generation (TG), especially during pregnancy when the increase in thrombin generation results from a series of changes in the haemostatic system. During administration of anticoagulant treatment, it is desirable to reduce the increase in thrombin generation. Aims of the study: The first aim of our study was to compare TG in individuals with a proven factor V Leiden mutation and individuals with elevated levels of factor VIII. Another aim was to compare patients with VTE and also with or without proven FV Leiden mutation that are taking different anticoagulation treatments. The final aim was to assess thrombin generation in pregnant women and their response to LMWH administration during severe thrombophilic conditions. Group of patients and methods: There were included 170 healthy blood donors in the control group, the congenital and acquired thrombophilia group included 44 individuals with FV Leiden mutation and 38 individuals with factor VIII level >150 % without other proven thrombophilia. 347 subjects with VTE taking direct coagulation inhibitors were included in the anticoagulation group.
