Radioterapie maligních onemocnění je i přes značný technický pokrok stále doprovázena nežádoucími účinky. Adenokarcinom prostaty patří mezi nádory dávkově závislé, pro které platí, že čím větší je počet nádorových buněk, tím vyšší dávka musí být aplikována na nádorové ložisko k jeho destrukci. Dodaná dávka na lůžko maligního tumoru prostaty musí dosáhnout alespoň 70 Gy, další navyšování dávky při užití konvenční techniky radioterapie je limitováno okolními kritickými orgány. Kritickými orgány malé pánve je střevo a močové ústrojí. Tomu odpovídají také nejčastější akutní nežádoucí účinky: proktitida, průjmy, tenesmy, dysurie a obtížné močení. Větší četnost nežádoucích účinků nastává při kombinaci chirurgické léčby a radioterapie (adjuvantní radioterapie). Tvrzní z odborné literatury jsem ověřila vlastními výsledky, které jsem získala od 192 nemocných léčených na Onkologickém oddělení Nemocnice České Budějovice a.s. v letech 2003 ? 2006. Despite significant technological progress, radiotherapy of malignant diseases has been accompanied with undesirable effects. Prostatic adenocarcinoma falls among the dose - dependent tumours for which it applies that the larger is the local finding the larger dose has to be applied to destruct the tumour centre. The dose applied to the prostate tumour bed should achieve at least 70 Gy, additional increase of the dose in the use of conventional radiotherapy technology is limited by the surrounding critical organs. Critical organs of the pelvis minor are the intestine and urinary tract. The most frequent adverse effect correspond to this: proctitis connected with diarrhoea, tenesmus and cystitis with dysuria and difficult urinating. Higher frequency of undesirable effects occurs in the combination of surgical treatment and radiotherapy (adjuvant radiotherapy). In my thesis, I assessed 192 patients with prostate gland tumours who were radiated by conventional technology in the years 2003 ? 2006 at the oncological ward of the Nemocnice České Budějovice a.s. I compared my own results with the data in the professional literature.
