[cze][eng] Oxygenoterapie neboli kyslíková léčba, patří mezi nejdůležitější nefarmakologické léčebné postupy a provádí se na všech typech pracovišť. Indikována je u pacientů s nedostatkem kyslíku, a to v koncentraci kyslíku vyšší než 21 %. Cílem kyslíkové léčby je zvýšit koncentraci kyslíku ve vdechované směsi, tím i v arteriální krvi, aby nebyly poškozeny důležité tkáně a orgány. Kyslík aplikuje sestra vždy na základě ordinace lékaře, kdy je stanovena dávka, způsob podání, délka podání a monitorování. Léčbu lze aplikovat při spontánní ventilaci anebo při umělé plicní ventilaci, a to za normobarických nebo hyperbarických podmínek. Základním předpokladem úspěšné léčby kyslíkem je dostatečná ventilace pacienta. Kyslík může být používán z různých zdrojů, mezi které patří tlakové láhve, kapalný kyslík a koncentrátory kyslíku. Obsahem bakalářské práce je problematika aplikace kyslíku v ošetřovatelské péči. Práce je rozdělena do dvou částí ? teoretické a praktické. Teoretická část obsahuje historii podávání kyslíku, fyziologii a patologii kyslíku v těle, indikace a kontraindikace aplikace kyslíku. Dále pak specifika ošetřovatelské péče o pacienta na kyslíkové terapii zahrnující možnosti aplikace kyslíku, monitoraci pacienta, zdroje a pomůcky pro aplikaci kyslíku. V praktické ? výzkumné části práce byla data získána metodou dotazování ? technika dotazníku, který byl anonymní a byly v něm použity otázky uzavřené a polouzavřené. Výzkumným souborem byly sestry pracující na lůžkových odděleních v Nemocnici Písek a.s. a v Nemocnici Strakonice a.s. Cílem práce bylo zmapování specifik ošetřovatelské péče o pacienta při aplikaci kyslíku a znalostí sester o způsobech aplikace kyslíku na jednotlivých odděleních. Získané informace byly porovnávány mezi sestrami na standardních odděleních a jednotkách intenzivní péče. První hypotéza vyjadřovala názor, že sestry znají specifika ošetřovatelské péče a při výzkumném šetření se potvrdila. Obsahem druhé hypotézy bylo tvrzení, že sestry znají i možné komplikace (rizika) při aplikaci kyslíku a potvrdila se. Obsahem třetí hypotézy bylo, že sestry znají způsoby aplikace kyslíku a i tato hypotéza byla potvrzena. Cíle práce byly naplněny. Výsledky výzkumného šetření byly použity k vytvoření ošetřovatelského standardu, jehož cílem je aplikovat kyslík bezpečně, správným postupem a nepoškodit pacienta. Může být zdrojem informací a vodítkem pro sestry v nemocnicích, které tento standard zatím nepoužívají.Oxygen therapy or oxygen treatment belongs among the most important non-pharmacological medical treatments and it is applied in all types of establishments. It is indicated to patients with the lack of oxygen and respectively in the oxygen concentration higher than 21%. The aim of the oxygen treatment is to increase the oxygen concentration in the breathed mixture and consequently in the arterial blood in order to prevent from damaging the important tissues and organs. The nurse applies oxygen always according to the doctor´s prescription, who states the potion, the way of application, the length of application and monitoring. The treatment can be applied at spontaneous ventilation and at artificial pulmonary ventilation namely under normobaric or hyperbaric conditions. The basic presumption for the successful oxygen treatment is sufficient ventilation of the patient. Oxygen can be used from different sources such as pressure cylinders, liquid oxygen and oxygen concentrators. The thesis deals with the issue of oxygen application in the process of nursing care. The thesis is divided into two parts ? theoretical and practical. The theoretical part involves history of oxygen application, physiology and pathology of oxygen in the body, indications and contraindications of oxygen applications. Further it contains specifications of nursing care for the patient who is undergoing the oxygen therapy including possibilities of oxygen application, patient´s monitoring, sources and aids for oxygen application. For the practical ? research part of the thesis the data were found out by means of the method of questioning. I used the technique of the questionnaire which was anonymous and the questions were both closed and semi-closed. The observed team were nurses working in the hospital wards in the Písek Hospital, a.s. and in the Strakonice Hospital, a.s. The aim of the thesis was to map out specifications of nursing care for the patient when oxygen is applied and also to find out nurses knowledge of ways of oxygen application in individual wards. The information collected from nurses working in standard wards with those working in intensive care units was compared. Hypothesis no. 1, that nurses know specifications of nursing care, was proved in the research process. Hypothesis no. 2, that nurses know possible complications (risks) when oxygen is applied, was proved. Hypothesis no. 3, that nurses know ways of oxygen application, was proved, too. The aims of the thesis were fulfilled. Results of the research work were used for making the nursing standard whose aim is safe and proper oxygen application in order to prevent from harming the patient. It can be a source of information and a manual for the hospital nurses who do not use this standard yet.
