[cze][eng] Termínem náhlé příhody břišní se označují onemocnění postihující orgány dutiny břišní, které ve většině případů vyžadují chirurgické řešení. Nástup onemocnění bývá rychlý, většinou postihuje pacienta ve fázi plného zdraví a často ohrožuje pacienta na životě. Přesto příznaky onemocnění bývají pacienty podceňovány a ti odbornou pomoc vyhledávají často až v těžkém stavu. Do rukou zdravotníků a lékařů se tak dostává pacient, který potřebuje akutně řešit své onemocnění. Cílem této práce bylo specifikovat odlišnosti v ošetřovatelské péči o pacienty s náhlou příhodou břišní. Dalším kladeným cílem bylo poukázat, jaké rozdíly jsou v komunikaci s pacienty s náhlou příhodou břišní, která jsou zástupcem akutních onemocnění, a u pacientů s plánovaným chirurgickým výkonem. Pro zjištění bylo osloveno třináct respondentů. Jednalo se o sestry, které pracují s pacienty s onemocněním chirurgické povahy. V souvislosti s cíli byly vysloveny výzkumné otázky zabývající se problematikou ošetřování pacientů s náhlou příhodou břišní. První výzkumnou otázkou zabývající se specifiky ošetřovatelské péče byly zjištěny činnosti, které dohromady vytvářejí komplexní obraz péče o pacienty s náhlou příhodou břišní. Druhá výzkumná otázka se zabývala rozdíly v předoperační přípravě u pacientů s náhlou příhodou břišní a u pacientů s plánovaným výkonem. Na základě výpovědi a pozorování respondentů je nejvíce vnímán rozdíl v čase, který je na přípravu umožněn. Třetí výzkumná otázka se zabývala rozdílem v komunikaci u pacientů s náhlými příhodami břišními a u pacientů s plánovaným chirurgickým výkonem. Dle zjištěného, komunikaci s pacienty ovlivňuje řada faktorů, zejména však nespolupráce pacienta, čas a také nedůvěra pacienta. Čtvrtá výzkumná otázka se zabývala kvalitou poskytované ošetřovatelské péče z pohledu sester v souvislosti s akutním stavem pacienta. Dle shledaných údajů, akutní stav pacienta, rychlost při provádění ošetřovatelských intervencí v rámci příjmu pacienta a předoperační přípravy, částečně ovlivňují kvalitu poskytované péče, která je pak prováděna více automaticky, rutinně. Na základě zjištěných informací by tato práce měla osvětlit sestrám, jako ošetřujícímu personálu, problematiku léčby a specifik péče o pacienty s náhlou příhodou břišní. Dále by měla pomoci pochopit problematiku komunikace s pacienty s náhlou příhodou břišní.The term acute abdominal emergencies denotes diseases affecting abdominal organs that in most cases require surgical intervention. The onset of the disease is usually quick, it mostly attacks a patient in full health phase and often threatens patient?s life. Nevertheless the disease symptoms are often underestimated by patients, who often seek for medical help late, when they are in bed condition. The aim of the Thesis was to specify distinctive features of nursing care for patients with abdominal emergency. Another aim was to point out the differences in communication with patients with abdominal emergency and those with planned surgical operation. Thirteen respondents were addressed to get the answer. They were nurses working with patients suffering from diseases of surgical nature. Research questions dealing with the issue of caring for patients with acute abdominal emergency were raised in relation to the aims. The first research question dealing with the specifics of nursing care asked about the activities forming together the complex image of the care for patients with abdominal emergency. The second research question dealt with the differences in pre-operation preparation of patients with abdominal emergency and those with planned operation. Upon the answers and upon monitoring the respondents the difference is particularly perceived in relation to the time spent on the preparation. The third research question dealt with the difference in communication with patients with abdominal emergency and those with planned surgical operation. The results show that communication is influenced by a number of factors, mainly lack of cooperation on patient?s side, time and patient?s distrust. The fourth research question dealt with quality of the nursing care from the point of view of a nurse in relation to patient?s acute state. According to the collected data the patient?s acute state, the speed of performing nursing intervention during patient reception and pre-operation preparation partly influence the quality of the care, which is then performed more routinely. Upon the obtained information this work should explain to nurses, as caring personnel, the problems of the treatment and the specifics of the care for patients with acute abdominal emergency. It should also help explain the problems of communication with patients with abdominal emergency.
