GLUKÓZOVÁ HOMEOSTÁZA V PERINATÁLNÍM OBDOBÍ A JEJÍ VLIV NA ČASNÝ PSYCHOMOTORICKÝ VÝVOJ DÍTĚTE DIABETICKÉ MATKY. Abstrakt Disertační práce ukazuje novou možnost screeningu novorozenecké hypoglykémie pomocí kontinuální měření glukózy (CGM) v intersticiu u nejrizikovějších novorozenců, dětí matek s diabetem. Novorozenecká hypoglykémie je jednou z nejčastějších novorozeneckých patologií, která může akutně výrazně zkomplikovat celkový stav pacienta a může mít negativní dopad na psychomotorický vývoj dítěte. Hlavním nástrojem pro její prevenci je screening intermitentními odběry glykémie. Odběry krve jsou invazivní, narušují komfort novorozence a dyády matka - dítě, mají lokální rizika, a mohou minout poklesy glykémie mezi jednotlivými odběry. CGM nabízí při menší invazivitě detailní vhled do adaptace glukózové homeostázy. Hlavními cíli práce bylo zhodnotit přesnost použití CGM na oddělení fyziologických novorozenců jako nástroje pro screening porovnáním párů hodnot CGM a glykémie. Druhým cílem bylo zhodnotit počet odběrů krve při použití CGM oproti běžnému screeningu. Dále jsme hodnotili psychomotorický vývoj našich pacientů pomocí standardizovaného testu Bayleyové pro zhodnocení bezpečnosti CGM. Analyzováno bylo 43 dětí diabetických matek. Výsledky prokázaly, že CGM lze z hlediska měření používat jako... THE EFFECT OF THE PERINATAL GLUCOSE HOMEOSTASIS ON THE EARLY NEURODEVELOPMENT IN INFANTS OF DIABETIC MOTHERS. Abstract This dissertation is presenting a novel approach in a screening of neonatal hypoglycaemia by continuous glucose monitoring (CGM) in neonates of mothers with diabetes. Neonatal hypoglycaemia is one of the most common neonatal pathologies and may adversely affect the neurodevelopment and may also have serious acute complications. Current standard approach for preventing neonatal hypoglycaemia is screening by intermittent sampling of glycemia. Blood lettings are an invasive procedure that disrupts the comfort of the newborn and mother - child dyad, more over that it has local risks, and may miss the blood glucose troughs between the sampling. CGM offers a detailed insight into the adaptation of glucose homeostasis while being less invasive. The main objectives of our study were firstly to measure the accuracy of the CGM used as a screening tool in the well baby nursery. Secondly, we evaluated the number of blood samples required due to the use of CGM compared to conventional screening. Thirdly, we assessed the neurodevelopment of our patients using Bayley scales of infant and toddler development. We analysed 43 infants of diabetic mothers.
