Arteriální hypertenze patří mezi nejčastější kardiovaskulární onemocnění a je jedním z nejvýznamnějších rizikových faktorů kardiovaskulární morbidity a mortality. V patologickém mechanismu vedoucím ke strukturálním změnám kardiovaskulárního systému se nad rámec hypertenze uplatňují i další hemodynamické a neuroendokrinní vlivy. V tomto procesu hraje významnou roli také nadprodukce aldosteronu a katecholaminů. Cílem práce je studium vlivu nadprodukce aldosteronu a katecholaminů na remodelaci intimo-mediálního komplexu společné karotidy a myokardu levé komory srdeční u nemocných s primárním aldosteronismem (PA) a feochromocytomem (FEO). V první části práce jsme analyzovali změny v textuře intimo-mediálního komplexu společné karotidy u 33 nemocných s PA, 52 nemocných s esenciální hypertenzí (EH) a 33 normotenzních jedinců. Hodnocení textury jsme provedli analýzou 140 Haralickových příznaků a 10 waveletů, které jsme následně použili pro trénování XGBoost klasifikátoru. V další části jsme zkoumali intimo-mediální tloušťku (IMT) společné karotidy a index hmotnosti levé komory srdeční (LVMi) u 50 nemocných s FEO před a 5 let po provedení adrenalektomie. Výsledky jsme následně srovnali s 50 nemocnými s EH. V rozlišování mezi PA a EH jsme dosáhli v porovnání se správností endokrinologických testů sloužících... Arterial hypertension ranks among the most prevalent cardiovascular disorders and represents one of the most significant risk factors for cardiovascular morbidity and mortality. Beyond hypertension itself, additional hemodynamic and neuroendocrine influences contribute to the pathological mechanisms that induce structural alterations in the cardiovascular system. Of notable importance in this process is the excessive production of aldosterone and catecholamines. The objective of the research has been to study the impact of aldosterone and catechola- mine excess on intima-media complex remodeling in the common carotid artery and left ventricular wall in primary aldosteronism (PA) and pheochromocytoma (PHEO) patients. Texture analysis of the intima-media complex of the common carotid artery was conducted in 33 PA patients, 52 EH patients, and 33 normotensive individuals. 140 Haralick features and 10 wavelets were analyzed and utilized to train an XGBoost classifier. Additionally, the intima-media thickness (IMT) of the common carotid artery and left ventricular mass index (LVMi) were examined in 50 PHEO patients before and 5 years post- adrenalectomy and compared to 50 EH patients.
