Přední segment oka zahrnuje oční víčka, řasy a slzný film, rohovku, spojivku, duhovku, zornici, řasnaté tělísko, komorový úhel a oční čočku. Katarakta (neboli zákal oční čočky) a dysgeneze předního segmentu oka jsou značně heterogenní skupinou onemocnění, která jsou z velké části monogenně dědičná. Hlavním cílem této disertační práce byla molekulárně genetická analýza 51 pacientů z 23 rodin s kataraktami a dysgenezemi předního segmentu oka, přičemž byla použita řada moderních i konvenčních metod, jako např. sekvenování nové generace, přímé sekvenování, bioinformatické a funkční analýzy včetně sestřihových vektorů. Identifikovali jsme 24 kauzálních variant v kódujících, nekódujících i regulačních oblastech genů PAX6, FTL, FOXC1, PITX2, FOXE3 a CHRDL1. Potvrdili jsme roli varianty n.37C>T v MIR204 na vzniku chorioretinální dystrofie variabilně asociované s kolobomem duhovky, časně manifestující kataraktou a kongenitálním glaukomem. Pomocí sestřihového vektoru byl prokázán patogenní efekt dvou nekódujících variant v genu PAX6 na sestřih pre-mRNA, vedoucí k vynechání exonů z kódující sekvence. Výsledky této práce napomohly ustanovit molekulární diagnózu u řady pacientů, což následně umožnilo zlepšit poradenství pacientům a jejich příbuzným z hlediska prognózy ztráty zrakových funkcí a rizika vzniku... The anterior segment of the eye includes the eyelids, eyelashes, tear film, cornea, conjunctiva, iris, pupil, ciliary body, chamber angle, and lens. Cataracts and anterior segment dysgenesis are a highly heterogeneous group of diseases exhibiting all types of Mendelian inheritance. The aim of this thesis was molecular genetic analysis of 51 patients from 23 families with congenital and early onset cataracts and/or anterior segment dysgenesis, using a modern as well as conventional methods, such as next-generation sequencing, direct sequencing, bioinformatic and functional analyses including the use of artificial splicing vectors. We identified 24 causal variants in the coding, non-coding and regulatory regions of the PAX6, FTL, FOXC1, PITX2, FOXE3, and CHRDL1 genes. We confirmed the role of the n.37C>T variant in MIR204 in the development of chorioretinal dystrophy variably associated with iris coloboma, early-onset cataracts and congenital glaucoma. Splicing assay confirmed pathogenic effect of two mutations on pre-mRNA splicing of the PAX6 gene.
