[cze][eng] Metronidazole (MTZ) cílí na řadu anaerobních bakteriálních a eukaryotických patogenů, včetně prvoka Giardia intestinalis (Metamonada). Přestože je MTZ hojně a dlouho využíván k léčbě giardiózy, příčiny selhání léčby giardiózy (G-TF) nejsou známé. Většina dosavadních poznatků byla získána pomocí linií s laboratorně navozenou rezistencí nebo z klinických vzorů, ke kterým nejsou dostupná klinická data. Ve spolupráci s Fakultní nemocnicí Bulovka v Praze byla v letech 2020-2023 sledována kohorta 70 pacientů, ve které 25,7 % pacientů vykázalo selhání léčby. Pomocí genotypizace bylo zjištěno, že giardie pacientů s G-TF patřily do stejné genetické skupiny (asambláž B, především BIII) a jednalo se pouze o importované nákazy převážně z Indie. Pro ověření hypotézy, zda může být rezistence spojena s určitým mezi liniemi přenosným genotypem, byly sledovány sdílené polymorfismy v genech zapojených v metabolismu MTZ a detoxifikacích (NR1, NR2, prx1, hmp). U G-TF izolátů v NR2 byly nalezeny aminokyselinové záměny (K42E, K139R), které mohou vést ke změnám ve struktuře proteinu. Zároveň byla testována variabilita počtu kopií genů jako spoluvarianta/příčina lékové rezistence v důsledku duplikací nebo delecí genu hmp. Výsledky z dPCR ukázaly rozdíl v počtu kopií hmp mezi sub- asamblážemi BIII a BIV a menší změnu počtu kopií...Metronidazole (MTZ) targets a spectrum of anaerobic pathogens ranging from bacteria to eukaryotic parasites, including the protist Giardia intestinalis (Metamonada). Despite the long time and widespread use of MTZ, understanding giardiasis treatment failure (G-TF) is incomplete. Most of the knowledge was gained on laboratory-generated resistant lines or on clinical lines without data on the treatment outcome. In cooperation with Bulovka University Hospital in Prague, 70 giardiasis cases were followed between 2020-2023, of which 25.7% exhibited the G-TF. Giardia genotyping revealed that G-TF cases belonged to the same genetic group (assemblage B, mainly BIII); and G-TF cases were all imported, out of which 60% were from returning travellers from India. To test the hypothesis that resistance is genetically determined/associated with a transmissible genotype, genetic polymorphisms shared in Giardia G-TF samples in a set of genes involved in MTZ metabolism and detoxifications (NR1, NR2, prx1, hmp) were examined. The analysis revealed amino acid substitutions (K42E, K139R) in NR2 isolates of G-TF patients, which might affect protein structure.
